Licensing Program Analysts (LPA) Angelica Boyles conducted an unannounced Required Annual Inspection. LPA was welcomed by, identified herself to, and discussed the purpose of the visit with Administrator Victor Schard. The facility is licensed to serve 3 residents all of whom must be ambulatory. LPA accompanied by Administrator, toured the interior and exterior of the facility, and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Resident bedrooms contained the required furnishings. Doors, windows and screens, toilets, and showers were in working order. Extra linens and hygiene supplies were present. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and resident activities. There was at least 2 days of perishable food, and at least 7 days non-perishable food present, all safely stored. Cooking/dining equipment and utensils were present. Medications were labeled, as required, and stored in locked areas. (CONTINUED ON LIC 809C) (CONTINUED FROM LIC 809) Per Administrator, no firearms or ammunition are kept at the facility. Smoke alarms, carbon monoxide detectors, emergency lighting, and facility telephone were all working. First aid kit(s) were complete and readily accessible. Required licensing postings were observed in visible areas of the facility. LPA reviewed staff and resident files. The reviewed files contained required documents. Confidential records were stored in locked areas. No deficiencies were observed or cited on today's visit. Licensing Program Analyst (LPA) Debbie Correia made an unannounced visit to the facility to conduct an annual licensing inspection. LPA identified herself to Licensee Victor Schard and explained the purpose of the visit. The facility is licensed to serve three (3) residents of whom all three (3) must be ambulatory. During today's visit all three residents were present. During today’s visit, LPA Correia, accompanied by Licensee Schard, toured the interior of the facility. LPA observed the interior of the facility to be sanitary and in good repair. LPA observed required postings, the fire extinguisher was current on inspection, and smoke alarms and carbon monoxide detectors were present and operable. The facility was following disaster drills and had current liability insurance. All residents’ bedrooms had the required furnishings, including padded mattress covers. Toilet and shower were in working order and equipped with non-skid flooring and grab bars. The facility’s ambient internal temperature was 74 F. The facility had an adequate supply of clean linens. LPA observed Personal Protective Equipment (PPE) and First Aid Kit and Manual. Medications, sharps, and toxins were housed in locked cabinets and inaccessible to residents in care. Per Licensee Schard there are no weapons or ammunition kept on the facility premises. LPA also conducted a review of staff and resident records, including P&I and ledger, all were complete, accurate and up to date. No deficiencies were cited during today's visit. Type of visit: Case Management - Annual Continuation

Licensing Program Analyst (LPA) Debbie Correia made an unannounced continuation visit of the required annual inspection. LPA was granted entry into the facility by Licensee Schard, identified herself, and stated the purpose of today’s visit, to inspect the facility to ensure that it is in compliance with the California Code of Regulations, Title 22, Division 6. During today's inspection LPA observed toxins, medications, and sharp object were inaccessible to clients in care. P & I money matched client ledgers and receipts. A review of Facility records revealed staff and client records were complete. Per Licensee Schard there are no firearms, weapons, or ammunition on the facility premises. LPA observed no bodies of water on the premises. LPA observed an adequate supply of PPE, personal hygiene supplies, and observed 2 days of perishable food and 7 days of non-perishable food to meet the clients dietary needs. An exit interview was conducted with Licensee, Schard, and a copy of this report along with the Licensee Rights (LIC 9058) will be provided. During today’s visit, LPA Correia, accompanied by Licensee Schard, toured the interior of the facility. LPA observed the interior of the facility to be sanitary and in good repair. LPA observed required postings, fire extinguisher was up to date, and smoke alarms and carbon monoxide detectors were present and operable. All clients bedrooms had the required furnishings, including padded mattress covers. Toilet and shower were in working order, including non-skid flooring and grab bars. The facility’s ambient internal temperature was 70 F. The facility had an adequate supply of clean linens. An overall inspection of the facility began today. However, due to time constraints LPA was unable to complete the visit and will return later to conduct the remaining portion of this inspection. No deficiencies were cited during today's visit. This report was discussed with Licensee Schard.
