Type of visit: Case Management - Annual Continuation

On 10/06/2025, Licensing Program Analyst (LPA) Daiquiri Boyd made an unannounced follow-up visit to the facility for the purpose of continuing the Annual Inspection visit. LPA asked that Administrator be called to meet with me at the facility. Caregiver stated that Administrator Victor Flauta was at work at his office of medical practice. Caregiver said that she called Administrator Jill Stowell who oversees another of their facilities, and that she was in Bakersfield and couldn't meet me at the facility. LPA asked if there was an LIC308, designating someone to be able to assist when an Administrator is present. Caregiver said she asked Jill Stowell if there was someone designated and was told there was not. LPA reviewed missing documents from residents files and reviewed the staff files. LPA found missing documents in staff files. LPA found issues with maintenance in the backyard area and inside the home in the bathroom area. LPA found that the Administrator they have listed as a backup, is not associated to this facility. Deficiencies were cited on this day. LPA requested the following documents: LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, LIC 610-E the Emergency Disaster Plan and copy of current Administrator’s Certificate to update the facility file. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Daiquiri Boyd arrived at the facility unannounced to conduct an annual inspection. LPA explained the purpose of the visit and was granted in by staff Lorna Daniels. Staff contacted Administrator Jill Stowell who called LPA and said she was with a resident at a doctor appointment and would be here in an hour. AD never showed up to the facility. Caregiver Sandra Jacaba came to the facility from across the street at their other facility, to aid LPA in today's visit. The residence was set at 76 F temperature and free of passageway obstructions inside and outside. Facility is clean and free of odors. Medications were stored in a locked cabinet in the kitchen. Smoke detectors and carbon monoxide detectors were checked and operating. Fire extinguisher was purchased 4/2024. LPA checked water temperature which measured at 107 F. Fire drill was last conducted 7/12/25. Resident records were reviewed. Medications for one resident was reviewed in part. Due to time constraints, LPA will continue this inspection at a later date. Allegation investigated: Staff does not ensure resident is provided with adequate servigs of food while in care.

Licensing Program Analyst (LPA) Daiquiri Boyd conducted the complaint investigation visit to the facility. LPA was greeted at the front door by caregiver staff Roselle Tayag. During this visit LPA reviewed client files and inspected refrigerator and pantry for food supply, as well as interviewed clients. LPA delivered investigation findings regarding the above allegations. The Department has investigated the complaint alleging: Staff does not ensure resident is provided with adequate servings of food while in care. LPA found an ample food supply in the home, resident stated that she has plenty of food and can help herself if she is hungry. LPA observed resident in the kitchen getting a dinner roll, stating she wanted a snack. Based on the interviews conducted and/or records review the above allegation is UNSUBSTANTIATED. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated. Type of visit: Prelicensing

On September 3, 2024, Licensing Program Analyst (LPA) Daiquiri Boyd met with Licensee/Administrator Victor Flauta for the purpose of conducting a Prelicensing visit. LPA toured facility. Common rooms have adequate furnishings and lighting. All five of the resident bedrooms have all the required furnishings and adequate lighting. Hot water temperature in bathrooms measured at 120 degrees F. LPA observed a supply of extra bed linens and personal hygiene and grooming products. Kitchen observed to have dishes, plates, utensils etc. Cleaning supplies are stored in locking cabinets in the garage, under the kitchen sink, under the bathrooms sinks, as well as above the laundry area. Medications are locked in a medication closet in the hallway. A fire extinguisher is present and has a purchase date of 04/01/2024. Smoke detectors and carbon monoxide were operating. Outside of the facility toured. Exits open and free of obstruction. The residence has alarmed exit doors which sounds the indoor alarm. No outside hazards were observed. No pools or bodies of water. All required postings are posted. Resident plans to update the size of the postings by the front door. Facility phone number will be (559)797-4637 and LPA called the number to ensure it is working. Component III was conducted during pre-licensing visit with Applicant. Applicant will make corrections as stated per LIC812 and will submit to LPA by September 10, 2024.
