Prathna Residential Care Facility for the Elderly

On 9/23/2025, Licensing Program Analyst (LPA) Triel Ellen Lindstrom arrived at the facility unannounced to conduct a required 1-year annual inspection. LPA Lindstrom met with care staff provider Helen Retirado and explained the purpose of the visit. The Administrator joined the site visit at 11:00 AM. The Administrator’s certificate # is 7023863740, expiration 11/9/2026. The facility is a four-bedroom, two-bathroom house in a residential neighborhood. There were two on-duty staff and one resident at the facility at the time of the visit. Both staff have criminal background clearances. The current census is four. LPA Lindstrom toured the interior of the facility. The interior was clean, orderly, and odor-free. The required documents were hanging on the wall in the entry way. The living room and dining room contained enough furniture for all residents. The four bedrooms were occupied by single residents. Each bedroom was clean and contained the required furniture. The two bathrooms were clean, odor-free and included grab bars and non-slip flooring. The hot water in the hallway bathroom was 110 degrees Fahrenheit, within the required range. A hallway closet contained extra towels and bedding, as well as cleaning supplies and extra toiletries in a locked cabinet. The kitchen was clean and sanitary. LPA Lindstrom observed a two-day supply of perishable food and seven-day supply of non-perishable food in the kitchen cabinets and refrigerator, as well as a refrigerator in the garage. All sharps, cleaners, and toxins were in locked drawers and cabinets in the kitchen. The LPA observed multiple first aid kits that contained the required items. The fire extinguisher was located in the kitchen and was last serviced on 8/15/2025 by Jorgensen Co. The smoke and carbon monoxide detectors were in working order. The backyard was manicured, had an intact perimeter fence, and included a shaded area with seating. The walkways on the grounds were free of obstruction. (continued on LIC809-C) LPA Lindstrom inspected facility records, including two staff records and four resident records. The staff and resident records were complete. LPA Lindstrom did an audit of two residena's cash resources managed by the facility and verified its accuracy. LPA Lindstrom observed resident medication stored in a locked cabinet. The paper-based medication administration records and centrally stored medication and destruction records were complete and up to date. The facility conducts disaster drills monthly and maintains a log of each drill. The Administrator reviews and updates the infection control plan and emergency disaster drill on a yearly basis. As a result of this inspection, no deficiencies were cited. On 9/16/24 Licensing Program Analyst (LPA) Maja Jensen arrived at facility unannounced to conduct a required 1 year annual inspection. LPA Jensen met with care staff provider Shirley Tudence and explained the purpose of today's visit. LPA Jensen toured the grounds and observed all paths to be free of obstruction. There are shaded areas and patio furniture for client use. There are no bodies of water on the property. The window screens were in good repair. The back yard gate is functional. LPA Jensen observed weeds growing in the gutters. Technical assistance was provided. LPA Jensen toured the interior of the facility. The facility was observed to be sanitary and free of odor. Adequate furnishings and lighting was observed. All toxins, medications and knives were observed to be locked and inaccessible to residents in care. An adequate supply of linens was observed. The facility maintains multiple 1st aid kits which are complete and in compliance. The smoke detector and carbon monoxide detector were observed to be in good working order. The fire extinguisher was last serviced on 8/26/24 and is in compliance. Emergency disaster drills are conducted and logged on a regular basis. The thermostat was set at 75 degrees for the comfort of the residents. The water temperature in the master bath sink was tested and found to be in compliance. There are no leaks in the master bathroom sink. LPA Jensen inspected the kitchen and observed a 2 day supply of perishable food and a 7 day supply of non-perishable food. Fresh fruit and vegetables were available. LPA Jensen reviewed 2 staff files which were observed to be complete with the exception of 1 staff member lacking a criminal background clearance transfer. LPA Jensen reviewed 3 of 3 resident files and observed them to be complete. A random audit on P&I funds was conducted and the accounting was determined to be accurate. Liability insurance and the surety bond were found to be current. LPA Jensen conducted client interviews and clients stated they are satisfied with all aspects of care being received. A deficiency is being cited pursuant to the California Code of Regulations (CCR), Title 22, Division 6. Civil penalties are also being assessed. On 10/26/23 at approximately 9:40am Licensing Program Analyst (LPA) Maja Jensen arrived at facility unannounced to conduct a required 1 year annual inspection. LPA Jensen met with Administrator Shirley Tudence and explained the purpose of today's visit. The administrator holds current certificate # 6057838740 good through 11/9/2024. The current facility census is 4. The facility's plan of operation allows for care of residents with dementia. The fire clearance allows for a capacity of 5 non-ambulatory residents. There is currently no hospice waiver. The annual licensing fees are current. The facility maintains liability insurance for the required policy limit minimums with a current policy period of 3/14/23 through 3/14/24. All facility staff were determined to have criminal background clearance. LPA Jensen toured the grounds and observed them to be maintained with all paths free of obstruction. All window screens were observed to be in good repair. There is outdoor furniture and shaded seating areas for clients to engage in outdoor activities. The facility exit/entrances have accessibility ramps. The interior of the facility was observed to be sanitary and adequately furnished. The thermostat was set at 70 degrees for the comfort of the residents. LPA Jensen toured the kitchen and confirmed that a 2 day supply of perishable food and a 7 day supply of non-perishable food is maintained. There is a menu posted and the food being prepared for dinner accurately reflected what was posted on the menu. Cleaning supplies, toxins and knives were observed to be locked and inaccessible to residents in care. LPA Jensen observed 2 kitchen cabinets missing handles. Technical assistance was provided. LPA Jensen toured the resident bedrooms. The facility has 2 private bedrooms and 1 shared bedroom. The bedrooms were observed to contain all required furniture. Each bedroom was personalized to reflect the interests of the individual resident. The facility maintains an adequate supply of bedding and linens. LPA Jensen observed all required postings to be displayed in areas easily viewed by residents/staff and visitors. The facility maintains adequate supplies for activities to encourage resident engagement. The first aid kit was observed to be in compliance. LPA Jensen reviewed the emergency disaster plan and found it to be in compliance. The fire extinguisher was purchased on 8/29/23 and is in compliance. The smoke detector and carbon monoxide detector were determined to be in working order. The facility maintains 7 day supply of water in the event of an emergency. There is emergency lighting available. LPA Jensen toured the facility bathrooms. The master bedroom bathroom was observed to have a bucket underneath the sink with water in it and appears to be leaking. Technical assistance was provided. The temperature in the master bedroom bathroom was measured at 113 degrees which falls within the required range of 105 degrees to 120 degrees. There are night lights available in the hallways and bathrooms. The bathroom shower/tub areas have non-slip mats. There is an adequate supply hygiene products and towels available. The bathrooms have paper towel dispensers for infection control. LPA Jensen conducted a random medication audit and determined the record keeping to be accurate. A random audit of P&I funds was conducted and the accounting was determined to be accurate. LPA Jensen reviewed 3 of 3 staff files and found all staff files to be complete and in compliance. 4 of 4 resident records were reviewed and found to be complete and in compliance. The facility does not appear to have an updated Infection Control Plan submitted. Technical assistance was provided. Deficiencies are being cited from the California Code of Regulations (CCR) Title 22, Division 6. Failure to correct deficiencies may result in the assessment of civil penalties.
