On 3/16/2026 at approximately 10:00 AM, Licensing Program Analyst (LPA), Angelica Segovia conducted an unannounced annual visit to the facility. LPA was greeted by the caregiver and stated the reason for their visit. LPA spoke with the Administrator, Clifton Von Buck who was not available and designated staff member, Faith Kemanzi to assist with today’s visit. LPA asked for the census, Staff/Resident Roster, and liability Insurance. LPA conducted a physical plant tour at approximately 11:30 AM and the following was noted: The facility is a single unit building with four (4) bedrooms and three (3) bathrooms currently occupying four (4) residents. There is a designated staff room with its own private bathroom. The facility has an approved fire clearance for four (4) ambulatory residents. The facility is vendor through North Los Angeles Regional Center (NLARC) and is being operated at a Level II Residential Home for the Elderly. Common areas: The living room and dining room were observed to be neat, clean, and organized. Both rooms were observed to be properly furnished and in good repair. The facility maintains a comfortable temperature at 70°F. A fire extinguisher is located in the kitchen was observed to be fully charged. Required postings such as See/Say Something, Facility License, and Rights of Individuals with Developmental Disabilities were observed to be located throughout the common areas. A working telephone was observed. Kitchen: The kitchen was observed to be clean and free from pests. Sufficient supplies of seven (7) day nonperishable food and two (2) day perishable foods were observed. The cleaning solutions/toxins and knives/sharps were observed to be kept locked underneath the kitchen sink. The kitchen appliances were observed to be working and in proper condition. (continued on LIC 809-C) Bedrooms: The residents’ rooms are adequately furnished with appropriate furniture and lighting system. Hallways/passageways are lighted appropriately. Extra linens/covers were observed to be stored in cabinets located within the hallway’s passageway. (continued on LIC 809-C) Bathrooms: The bathrooms were checked for cleanliness and proper operation. The hot water temperature was measured within regulations. LPA observed appropriate grab rails and slip-resistant mats to be in proper condition. Laundry Room: The laundry room was observed to be located near the staff room. The laundry appliances were observed to be working and in proper condition. Staff Room: LPA observed the staff room to be locked and inaccessible to residents. Backyard: The backyard of the facility is equipped with a designated shaded area with outdoor furniture for residents. No body of water located at the facility. Garage: The garage is located outside of the facility and was observed to be kept locked. The garage was observed to be used for storage purposes. Extra refrigerator and freezer with additional food for residents was observed. Medications: The medications along with staff and residents’ files were observed to be kept in the storage closet located in the hallway. The medications were observed to be kept in a locked container. Smoke detectors and carbon monoxide observed to be working properly and were tested. Resident/Staff Records: LPA conducted a complete file review of resident records. Resident records appeared to be complete and updated. Staff records: LPA conducted a complete file review of three (3) staff records. Staff records appeared to be complete and updated. There were no immediate health and safety hazards observed during the day of inspection. LPA was greeted by the caregiver and LPA stated the reason for their visit. Administrator, Clifton VonBuck arrived shortly after to assist with today’s visit. LPA asked for the census, Staff/Resident Roster, and liability Insurance. LPA conducted a physical plant tour at approximately 11:00 AM and the following was noted: The facility is a single unit building with four (4) bedrooms and three (3) bathrooms currently occupying three (3) residents. There is a designated staff room with it’s own private bathroom. The facility has an approved fire clearance for four (4) ambulatory residents. Knives, sharps, cleaning solutions and disinfectants observed to be kept in a locked cabinet underneath kitchen sink. Kitchen appliances observed to be working and in proper condition. Bedrooms: The Residents' rooms are adequately furnished with appropriate furniture and lighting system. Hallways/passageways are lighted appropriately. Extra linens/covers were observed stored in a storage closet located in the hallway’s passageway. (continued on LIC 809-C) Bathrooms: Bathrooms were checked for cleanliness and proper operation. Appropriate grab rails and non-slip mats were observed and in proper condition. The hot water temperature was measured within regulations. Garage: The garage is located outside of the facility and is kept locked. LPA observed additional refrigerators and freezer used to store extra food. The garage is equipped with sufficient supplies of Personal Protective Equipment (PPE) along with extra supplies of cleaning solutions and toiletries. Laundry Room: The laundry room is located inside the facility near the staff room. Laundry appliances observed to be working and in proper condition. Staff Room: LPA observed staff room locked and inaccessible to residents. Backyard: The backyard of the facility is equipped with a designated shaded area with outdoor furniture for residents. No body of water located at the facility. Medications: Medications were observed stored in a locked box located in the hallway. First-aid kit observed to be equipped with but not limited to bandages, scissors, digital thermometer, tweezer, and First Aid manual. Smoke detectors and carbon monoxide observed to be working properly and were tested. The Last Fire Drill was conducted on 2/26/25. Resident/Staff Records: LPA conducted a complete file review of resident records. Resident records appeared to be complete and updated. Staff records: LPA conducted a complete file review of staff records. Staff records appeared to be complete and updated. There was no immediate health and safety hazard observed during the day of inspection. An unannounced Required One (1) year visit was conducted on this day by Licensing Program Analyst (LPA) Gary Tan. LPA met with staff Patricia Cowan who called the administrator Clifton Von Buck and explained the reason for the visit. Mr. Von Buck designated Ms. Cowan to sign the report. This is a North Los Angeles Regional Center vendored facility Level II. LPA conducted physical plant tour inside and out at 1 02 PM During the tour, LPA observed that the facility has four (4) bedrooms and three (3) bathrooms. One bedroom and one bathroom is designated for staff use. The swimming pool is appropriately fenced but has no water in it and locked during visit. The front main door is the only entrance being utilized at the facility. There is a sign on the front door that everyone entering at the facility must be screened. Screening area is located immediately before entrance. Sign in sheet, hand sanitizer, gloves and masks are available. The facility had submitted and approved Mitigation and Infection Plan. Signs to wear a mask and other Covid 19 prevention protocol signs were posted indoors. Hand washing, coughing etiquette, physical distancing and other necessary signs were posted in the bathroom and all over the facility. All trash cans were observed to be with cover. The facility has a designated visitors' area at the backyard. The facility has sufficient stock of PPE in the storage. Bedrooms were toured and observed to be clean and appropriately furnished. Bathrooms were observed to be clean, sanitary and with necessary supplies. Hot water temperature measured at a range of 111.3°F to 114.7°F (continued to LIC 809-C) (continued from LIC 809) Physical plant was checked for cleanliness and condition. Facility was observed to be in good repair and clean during today's visit. Living and dining room furniture were also checked for functionality (wear and tear). Furniture was observed to be in good condition. Kitchen area is observed to be clean and sanitary. All disinfectants, cleaning solutions and other toxins were observed to be locked in the hallway cabinet near the main entrance. Laundry area is located adjacent to the dining room by the staff room. Laundry detergent, cleaning agents and other toxins are kept in the locked in the cabinet under the kitchen sink. Food. The facility is observed to have sufficient food supply both perishable and non-perishable for clients. Temperature of facility wall thermostat was set at 74.0°F and observed to be within the required range. Fire extinguisher was observed to be located in the living room. Fire extinguisher was observed to be operable and last inspected on 03/18/24. Fire alarms are hardwired, tested and observed to be operational. There was a carbon monoxide detector installed in the facility. Medication were observed to be locked, inaccessible and stored in the cabinet in the bedroom hallway cabinet. There was a complete first aid kit inside the cabinet. Knives are locked in the cabinet under the sink. Garage is detached to the house and observed to be locked and inaccessible to clients. Garage is also used as frozen emergency food and other supplies storage. Client records. All four (4) client records were reviewed. Clients record are complete and current. Staff records were also reviewed. All staff present records were reviewed, they all have criminal record clearances and associated to this facility. Current training and first aid observed for staff on duty. Administrator's certificate was observed to be current. Disaster drill was last conducted on 10/30/24. Required posting observed in facility (complaint hot line poster, personal rights, etc). There was no health and safety hazard observed during the day of inspection. Exit interview conducted.
