Licensing Program Analyst (LPA) Sanjay Vaid conducted an unannounced annual visit using the CARE Tool. LPA Vaid was allowed entry by caregiver Griselda Cisneros, Caregiver and met with Laura Aguilar (Assistant Administrator) and explained the reason for the visit. The facility is licensed to serve 6 non-ambulatory residents in the age range of 60 and over. The facility has a hospice care waiver for 6 residents. The facility is operating within the scope of its license. A tour of the single-story facility included: living room, kitchen, dining room, 4 resident bedrooms, 2 bathrooms, front porch, backyard, and attached garage. Two private and two shared rooms. Today’s census is three (3) residents. Garage is inaccessible to residents. LPA Vaid and Laura Aguilar toured the facility and the following was observed: the front porch and backyard are well maintained. There is a shaded seating area for the residents located in the backyard. Passageways and exits are free of obstruction. Auditory devices were seen on exit doors which are required for dementia residents and were operating at the time of the visit. The water temperature was tested in both bathrooms and measured at 108.7 degrees F and 110.1 degrees F which is within the required 105 - 120 degrees F. The bathrooms are clean and have the required grab bars in the shower and near the toilet for non-ambulatory residents. Showers also have non-skid materials. Resident bedrooms have the required furniture such as bed frames, dressers, lamps and chairs. Bedrooms also have enough closet space. Resident beds have the required linen, and the linen is in good condition. There are extra clean linen and towels in a hallway closet. Smoke detectors were observed in each room and throughout the facility and were properly operating. There is carbon monoxide in the kitchen/dining area and was properly operating. There is one fire extinguisher located in the kitchen which is fully charged. Kitchen appliances are clean and were operating at the time of the visit. (Continued to LIC 809-C) Sharps, cleaning supplies and toxins are kept locked under the kitchen sink and are inaccessible to residents. Sufficient supply of 2 days perishable & 7 days non-perishable foods was observed in the kitchen and garage freezer. First Aid kit was fully stocked with current manual, and it is kept locked in the medication cabinet. Residents and staff files are centrally stored in the medication cabinet. Residents medication are centrally stored in a locked kitchen cabinet. LPA reviewed medication for all 3 residents and observed that medications are documented properly and given as prescribed. LPA reviewed all 3 resident files and 2 staff files have CPR and 1st Aid certifications. LPA observed administrator certificate for Luis Aguilar with an expiration date of 09/23/2026. LPA interviewed 2 staff and attempted to interview all 3 residents. Liability Insurance expires 02/20/2027. Safety drills conducted 04/06/26. Per California Code of Regulations, Title 22, and California Health and Safety Code, there were Zero (0) deficiencies observed during the visit. Licensing Program Analyst (LPA) Alberto Lopez conducted an unannounced annual inspection visit. LPA met with facility staff Laura Aguilar. The purpose of today's visit was discussed. The facility is licensed to serve six (6) non-ambulatory residents who are ages 60 and above and approved for three (6) Hospice Waivers. Facility had dementia program on file. Currently, two (2) residents are on hospice. Annual licensing fees are current. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following: 1. Infection Control: Infection control practices and Personal Protective Equipment (PPEs) were observed. The facility has submitted a COVID-19 Mitigation Plan and Infection Control Plan. 2. Operational Requirement: Liability Insurance is updated and in the amount of at least ($1,000,000) per occurrence and ($3,000,000) in total annual aggregate is in place. 2) PHYSICAL PLANT The facility is a one-story home with four (4) resident rooms, two (2) bathrooms, front porch, backyard, and attached garage. Resident rooms had the required furniture and in compliance. Bathrooms inspected were clean, operable, with the required grab bars and non-skid materials in the shower. Hot water temperature was measured in a range from 112.6 to 113.1 degrees Fahrenheit. Adequate linen and personal hygiene supplies were observed. Medications were centrally stored, locked and inaccessible to residents in care. Auditory device alarms are operational. Smoke detectors and carbon monoxide detectors were tested and operable. Fire extinguishers were fully charged and replaced when needed. The outdoor area has a shaped area and seating. (continued on 809C) (Continued from 809) 4. Staffing: The facility has sufficient staffing, and the night supervision staff have current CPR/first aid certification. 5. Personnel Record-Training: All the staff files are maintained in the facility. Staff employed are over the age of 18 and are fingerprint cleared and associated to the facility. Some staff have required ongoing training. Administrator Luis Aguilar Certificate expires 09/23/2026 6. Resident Records-Incident Reports: Resident files are maintained at the facility and have the following documents in their files - Admission Agreements, Identification & Emergency Information, Pre-admission appraisal/Appraisal Needs & Services Plan. 7. Resident Rights-Information: The Complaint, ombudsman and CCLD poster are not posted. entry. Visiting hours are included in admission agreement. 8. Planned Activities: Facility has sufficient space to accommodate indoor and outdoor activities. There are sufficient supplies and equipment to meet resident's physical capability. 9. Food Service: The kitchen was inspected and did have sufficient supply of 2-day perishable & 7-day non-perishable food. Kitchen, food preparation area, and storage areas were observed to be very clean and sanitary. Pesticides and cleaning supplies are kept away from the food preparation areas. Kitchen is kept clean and free from rodents. 10. Incidental Medical and Dental: Medication is centrally stored and locked in the medication cabinet near the kitchen. Four (4) centrally stored resident medications were reviewed, which contained 30-day supply of medications. All medications are administered according to physician’s orders, Disaster preparedness: The last fire drill was conducted on 04/28/25. The facility has an Emergency Disaster Plan posted at the facility. 12. Resident with Special Health Needs: 1 resident are receiving home health services. No resident is currently on postural support. Individual Service Plans and Appraisals are on file. No deficiencies observed during today’s visit. An exit interview was held. The facility is licensed to serve six (6) non-ambulatory residents in the age range of 60 and over. The facility has a hospice care waiver for six (6) residents. Annual fees are current. Administrator certificate is current, expires on 09/23/24. During the visit, the CARE tool was used; a tour of the facility was conducted; food supply was reviewed; staff/residents files were reviewed; staff/residents interviews were conducted; and medications were reviewed. The facility was a single family with four (4) resident rooms, two (2) bathrooms, front porch, backyard, and attached garage. Resident rooms had the required furniture and in compliance. Bathrooms inspected were clean, operable, with the required grab bars and non-skid materials in the shower. Hot water temperature was measured in a range from 106.1 to 111.6 degrees Fahrenheit. Adequate linen and personal hygiene supplies were observed. Sufficient supply of two (2) days perishable and seven (7) days non-perishable foods was observed in the kitchen and garage freezer. Smoke detectors and carbon monoxide detectors in residents' rooms were operable. Auditory devices were operable. All mandated documents and signages were posted in common areas. There was shaded patio and garden areas with ample seating. Medications were centrally stored and locked. Resident and staff records inspected were current. Fire/ Emergency drill was conducted on 02/26/24. No deficiencies were observed to be in violation of California code of Regulations, Title 22, Division 6. An exit interview was conducted.
