Licensing Program Analyst (LPA) Cuadra arrived unannounced to conduct a case management visit to follow up on a telephone call received on 1/29/26 from Licensee of the facility, Emilce Deller regarding a change of ownership. Upon LPA arrival, staff granted access to the facility. Licensee was not able to come to the facility due to medical reasons, but was available by phone and gave authorization for staff to sign the report. On 1/29/26, The Licensee contacted LPA via phone to notify the Department that the facility is under a Limited Liability Corporation (LLC). According to the Licensee, they didn't remember that they have switched to a limited liability company (LLC) since 2020, and Emilce Deller will be the Licensee in LLC of this facility. During today's visit, LPA learned that current (LIC309) Administrative Organization form indicates that this facility is in a sole proprietorship of two Licensees. As of today’s date, a new application has not been submitted to the Department. License acknowledges that they are responsible for the operation until the new application is approved and issued. The Licensee agrees to provide documentation to ensure control of property. The Licensee confirms understanding that the current license is not transferable and an application is required to be submitted to the Department for change of ownership. The Licensee agreed to submit a change of ownership application with new corporation or LLC to the Centralized Application Bureau (CAB) for the facility by not later than April 10, 2026. Deficiencies are cited from the California Code of Regulations (CCRs), Title 22, Division 6, Chapter 8 and the Health and Safety Code. Failure to correct the deficiency and/or repeat deficiencies within a 12-month period may result in civil penalties. Appeal Rights Given. Licensing Program Analyst (LPA) Cuadra arrived unannounced for the purpose of reviewing the physical plant for updated fire clearance for designating a bedridden resident and met with Licensee, Emilce Deller. LPA/Licensee toured the facility, including the bedroom involved in the updated fire clearance, which is the room located in the hallway, next to the main bathroom. No new construction was done. The facility sketch designating one room as a bedridden room and indicating the ambulatory status for each room was updated as of 6/20/2025 after the fire safety inspection was approved. Licensing Program Analyst (LPA) Cuadra arrived unannounced to conduct an Annual required inspection and met with staff Merced Ruiz. Licensee, Emilce Deller was unable to come to the facility, but gave authorization for staff to sign the report. Fees are current. There are residents with diagnosis of dementia. LPA initiated a tour of the facility at 9:00 am and made the following observations: Facility was a comfortable temperature and passageways were free from obstructions. Resident rooms were furnished per regulation. Water temperature in resident's bathroom measured at 108.1 and 107.3 degrees F which are within allowable range of 105 to 120 degrees F. Extra hygiene products and linens were available. Bathrooms had required bath mats and grab bars. Toxins were inaccessible to residents in care. Facility has at least two days of perishable and one week of non-perishable foods. Medications were centrally stored and locked. Fire extinguisher was last inspected June, 2024. Smoke detectors and carbon monoxide detectors located throughout the facility were tested and operational. Exit doors have auditory alerts that were functional at time of visit. LPA initiated file review at 9:30 am. Two staff files and six resident files were reviewed. All staff have required 1st Aid/CPR certificates and required training hours. Residents have a current medical assessment and care plan updated. Administrator Certificates for Licensee Emilce Deller, 6010293740, expires on 2/26/26. Licensee agreed to submit updates of the following documents by 6/1/25: Designation of Administrative Responsibility (LIC308), Personnel Report (LIC500) liability insurance and control of property. Licensing Program Analyst (LPA) Cuadra arrived unannounced to conduct a Case Management Visit and met with Licensee, Emilce Deller. On 6/24/24 the licensee have contacted the Department requesting guidance regarding an increase of capacity from six to seven residents. During today's visit, LPA/Licensee have discussed about the possibility of increasing capacity and after the conversation, the licensee stated that they don't want to proceed with the change of capacity process. Also, Licensee requested LPA some guidance regarding possible cost of living increases effective next year. LPA have provided regulations regarding increases and Licensee agreed to submit to the Department any changes prior to become effective for residents in care and their responsible parties. LPA discussed with Licensee Technical Support Program (TSP) that offers advice, guidance, a review of facility operation, discusses best practice, and required regulation/HSC compliance if needed. Resident rooms were furnished per regulation. Water temperature in resident's bathroom measured at 105.3 and 106.1 degrees F which are within allowable range of 105 to 120 degrees F. Extra hygiene products and linens were available. Bathrooms had required bath mats and grab bars. Toxins were inaccessible to residents in care. Facility has at least two days of perishable and one week of non-perishable foods. Medications were centrally stored and locked. Fire extinguisher was last inspected June, 2024. Last disaster drill was conducted on 3/23/24. File review was initiated at 1:30 pm. Three staff files and six resident files were reviewed. One out of three staff do not have required 1st Aid/CPR certificates (technical violation issued). One out of six residents (R1) do not have a current medical assessment within the last 12 months. Three out of six residents (R1, R2 & R3) do not have a care plan updated within the last 12 months as indicated in regulation. Administrator Certificates for Licensee Emilce Deller, 6010293740, expires on 2/26/26. Training records were reviewed. Licensee agreed to submit updates of the following documents by 6/28/24: Designation of Administrative Responsibility (LIC308), Personnel Report (LIC500) and control of property.
