Licensing Program Analyst (LPA) Nadia Shahbazian conducted an unannounced Required - 1 Year annual inspection visit. Upon their arrival, staff on duty was Karine Maloyan/Care Staff and Administrator Elina Melkonian. LPA explained the purpose of the visit. The program is vendorized through the Frank D. Lanterman Regional Center. The Residential Care Facility for Elderly (RCFE) is licensed for 6 non-ambulatory adults, ages 60 and above, one of whom may be bedridden. Facility has hospice waiver for for 6 clients. Current census is 6 clients, two of whom are ambulatory. LPA toured the facility with Administrator at 10:25 am and observed the following: Required postings were observed in the hallway and office area. The hallway leads to an exit door, which is used as the main exit. In addition there is an exit door in the kitchen, leading to the hallway exit. LPA observed functional auditory alarms on all doors. The fire extinguisher is located in the kitchen and was recently purchased on 03/24/2026. Facility conducts quarterly fire and safety drills, the last fire drill was conducted on 02/28/2026. The last earthquake drill was conducted on 04/19/2026. Facility is equipped with two fire doors, one in living room and one in kitchen. The dual smoke alarms and carbon monoxide detectors are hardwired and interconnected. At 10:09 am the smoke/carbon monoxide detectors were tested and observed to function properly. Kitchen: The kitchen appliances consisted of a stove, refrigerator, stove top oven, dishwasher and microwave; all the fixtures were observed to be clean and functional. Knives and sharp kitchen objects are observed to be locked in a kitchen drawer. Chemicals and cleaning detergents were observed to be locked in the patio cabinet, inaccessible to clients in care. LPA found a sufficient supply of perishable foods (2 days) and non-perishable food (7 days) supplies, with sufficient amount of dishes for the census of six (6). (Continued on 809C) Common Areas: The living room is located by the entry door and leads to the dining area. A door connects the dining area to the kitchen. The common areas were properly furnished with ample sitting/tables for appropriate number of clients and staff. A television set and activity/gaming areas were observed in the living room. Facility offers cable, internet access and a land line telephone for client use. Bathrooms: There are two (2) bathrooms designated for staff and clients’ use. All toilets and sinks are maintained in sanitary, operating condition. LPA observed proper grab bars and non-skid mats in both bathrooms. Hot water temperature was measured at 115.2 and 115.9 degrees Fahrenheit. Bedrooms: There are six (6) private bedrooms designated for clients' use. All of the bedrooms were properly furnished with appropriate chairs, beddings. television sets, linens with sufficient lighting. Bedroom #4 has an exit door, leading to the backyard and is designated as the bed ridden room. Laundry Room: Stackable laundry machines were observed in the hallway. All toxins are stored in a locked cabinet in the patio, inaccessible to clients in care. First-Aid Kits/Medications: LPA observed all medications in a locked cabinet in the hallway. A complete first-aid kit with all required supplies and the first aid manual was maintained in the medication cabinet as well. LPA counted medications for all 6 clients and compared to Medication Administration Record (MAR), to ensure administration based on physician orders. Surrounding grounds: No pool or bodies of water were observed at the facility. Entry/exit gates and pathways were free of obstruction. There was a covered patio area with furniture appropriate for outdoor use, sufficient for number of clients. There is a refrigerator for staff use in the patio, along with a locked cabinet for kitchen and laundry detergent/chemicals. A locked shed is located in one corner of the backyard. LPA observed a refrigerator and freezer supplied with perishable and non-perishable food and emergency water in the locked shed. There is an attached garage, in the front, which currently is being used for storage. Resident/Staff Files: A review of resident records was conducted to ensure compliance of licensing forms. LPA counted P&I for six out of six clients and compared to purchase receipts for accuracy of records. Staff files were reviewed to ensure all forms, training and first aid certificates are up to date. Pursuant to Title 22 Division 6 of the CA Code of Regulations, no deficiencies observed during the visit. The Adult Residential Facility is licensed for 6 non-ambulatory adults, ages 60 and above, one of whom may be bedridden. Facility has hospice waiver for for 6 residents. Current census is 6 residents, two of whom are ambulatory. LPA toured the facility with Administrator at 9:15 am and observed the following: Required postings were observed in the hallway and office area. The front entry is the main exit door and there two exit doors leading to the backyard. The physical plant appeared to be very clean and sanitary. The fire extinguisher is located in the kitchen and was recently purchased on 03/21/2025. Facility conducts quarterly fire and safety drills, the last fire drill was conducted on 05/04/2025. The last earthquake drill was conducted on 06/16/2025. Facility is equipped with two fire doors, one in living room and one in kitchen. The dual smoke alarms and carbon monoxide detectors are hardwired and interconnected. At 9:59 am the smoke/carbon monoxide alarms were tested and observed to function properly. Common Areas: Include a living room and dining area. The common areas were properly furnished with ample sitting/tables for appropriate number of clients. Television set and activity/gaming areas were observed in the living room. LPAs found a sufficient supply of perishable foods (2 days) and non-perishable food (7 days) supplies, with sufficient amount of dishes for the census of six (6). Bathrooms: There are two (2) bathrooms designated for staff and residents’ use. All toilets and sinks are maintained in sanitary, operating condition. LPA observed proper grab bars and non-skid mats in both bathrooms. Hot water temperature was measured at 111.2 and 115.7 degrees Fahrenheit. Bedrooms: There are six (6) private bedrooms designated for clients' use. All of the bedrooms were properly furnished with appropriate chairs, beddings. television sets, linens with sufficient lighting. Laundry Room: Laundry machines were located in the hallway. All toxins are stored in a locked cabinet in the patio, inaccessible to clients in care. First-Aid Kits/Medications: There are two complete first-aid kits with all required supplies and the first aid manual. Medications were locked in a cabinet in the hallway. Surrounding grounds: Entry/exit gates and pathways were free of obstruction. The outdoor area was free of visible immediate hazards. No pool or bodies of water were observed at the facility. There is ample patio space for outdoor activities. There was a covered patio area with furniture appropriate for outdoor use sufficient for all clients. There is a refrigerator for staff use in the patio, along with a locked cabinet for kitchen and laundry detergent/chemicals. There is a detached garage which currently is being used for storage. Resident Files: A review of resident records to ensure compliance of licensing forms was conducted. LPA reviewed and counted medications for 6 out of 6 residents and compared to MAR for accuracy of administration based on physician orders. P&I for six out of six residents was also counted for accuracy. Staff Files: Staff files were reviewed to ensure all forms and training certificates are up to date. Pursuant to Title 22 Division 6 of the CA Code of Regulations, no deficiencies observed during the visit. Licensing Program Analyst (LPA) Abeye Duguma met with Elina Melkonian for a Required One (01) Year visit. LPA explained the reason for the visit. A tour of the physical plant was conducted at around 9:15 AM and the following was noted: There is one entrance being utilized at the facility. The facility has a total of six (06) bedrooms and two (02) bathrooms. The facility is fire cleared for six (06) non-ambulatory of which one maybe bedridden. The facility is currently occupying six (06) residents. The facility has outdoor furniture with a covered shaded area for residents and visitors. The facility does not have a swimming pool/body of water. The garage is currently being used for storage. Laundry detergents, cleaning agents and other toxins are locked away. Kitchen is sufficiently stocked with at least two (02) days perishable and seven (07) days non-perishable food. Frozen foods are wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. Knives and sharps are observed to be locked and inaccessible to residents. The living and dining room are neat and clean. The facility maintains a comfortable temperature at 70°F. The smoke and carbon monoxide detectors are hardwired, interconnected and observed to be operational. Fire extinguisher is located in the kitchen, observed to be full and last purchased on 03/26/2024. (continued on LIC 809-C) The residents' rooms are adequately furnished with appropriate lighting system. Hallways are well lit. Residents have enough personal hygiene product provided by the licensee. The bathroom was checked for cleanliness and proper operations. The hot water temperature was measured at 121.9°F. Towels and washcloths are not shared. There was enough clean linen available in the cabinets. LPA observed medication and first aid kit to be locked and inaccessible to residents.
