Licensing Program Analyst (LPA) Imaculada Vasquez and Yolanda Delgado arrived unannounced to conduct an annual inspection. Upon arrival LPA was greeted by facility staff and granted entry by Licensee Emelita Price and Staff Rositas Melendros. LPA began inspection with introduction and visit purpose. Upon arrival LPA learned that one (1) client resides at this facility and there is currently two (2) caregiver present. The facility is a single story structure consisting of four (4) bedrooms and two (2) bathrooms. There is not Infection Control Plan on file. Client Records-Incident Reports/Clients Rights-Information/Dental- LPA began review of client records. One (1) records were reviewed. LPA reviewed for identification and emergency information, admission agreement, medical assessment, and TB test results, needs and service plans - missing, functional assessment, centrally stored medication/destruction records, safeguard for personal property/valuables, and personal rights notification. Personnel Records/Training/and Staffing- LPA began review of employee records- Two (2) records were reviewed. LPA reviewed employee records for first aid certification- missing, criminal record clearance or an exemption, health screening and TB test results, employee rights, training verification, and current administrator certification; expiration date 12/13/2027. Food Service- Food prep areas are clean and organized. Food supply meets the requirement of one (1) week supply of non-perishable and two (2) day supply of perishables - not observed. Emergency food and water supply was not present. There is a locked location for chemicals and sharps in the kitchen. (Continued on LIC809C) (Continued from LIC809) Physical Plant and Safety of Environment/Operational Requirements- LPA toured the facility inside and outside. LPA observed the facility to have pests and exterior window screens and in need of repair. The home is maintained at a comfortable temperature for the clients. Lighting is sufficient for safety and comfort. Water temperature measured 122.0 degrees F. Laundry facilities and a locked cabinet is present for storing laundry soap and other chemicals. All outdoor and indoor passageways are free of obstruction. Emergency lighting is available. There is a telephone working at this location. The LIC 610, emergency disaster plan is maintained. There are no firearms at this home. This home does not have a pool. Medications- are centrally stored. There is a locked cabinet allocated for medication storage. Centrally stored medication and destruction logs are maintained. Medications reviewed appear to have been dispensed accurately. LPA made observations throughout the inspection process to assess if the facility remains in conformity with the State Fire Marshall regulations. Smoke detectors and carbon monoxide detectors were tested and found to be operational. Fire extinguisher was recharged this year. The facility is conducting emergency disaster/fire drills quarterly; last done on 11/2025. Based on the information received during this visit today in the areas reviewed, six (6) deficiencies and one (1) technical violation are being cited per Title 22, Division 6 of The California Code of Regulations. This LIC 809, 809D, Appeal Rights report was reviewed with Licensee Emelita Price at the time of the exit interview. On 02/21/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a 1 year required visit. LPA was greeted and granted entry by caregiver Rositas Melendro. Below are the observations made from today's visit: The facility currently has (1) resident in care that was at program during LPAs visit. There was (1) staff present. The facility is a single story structure consisting of (4) bedrooms and (2) bathrooms. The facility is utilizing video surveillance in the common areas. The following is needed an updated facility sketch, an addendum to the plan of operation, as well as a consent from the resident acknowledging that they agree and were informed that video surveillance will be utilized in the facility. LPA observed for there to be a locked storage food room inside the garage that staff did not have access to, it is not included on the facility sketch. LPA observed for there to be a live cockroach crawling around the kitchen floor, as well as a baby cockroach on kitchen counter. Per Mr. Price the facility is doing their own extermination treatment. In addition LPA observed for there several expired food items of canned peas (4), cranberry sauce(3), refried beans (4), and (6) boxes of cake mix dating back to March 2018. LPA observed for there to be crumbs throughout the refrigerator in addition to resident medication that requires refrigeration. Deficiency cited. The Administrator Mr. Alan Price agreed to bring the small refrigerator to the facility today, Proof is to be submitted to department by 5pm 02/21/25. All staff present were observed to have criminal record clearance and were associated to the facility. The smoke and carbon monoxide detectors were tested and observed to be operable. The water temperature was tested and was found to be within regulatory limits measuring at 106.8 degrees Fahrenheit. The medications (bubble packs) were observed to be locked and inaccessible to residents in care. Based on today's inspection deficiencies were be issued in accordance with the California Code of Regulations (Title 22, Division 6, Chapter 8). On February 13, 2024, Licensing Program Analyst (LPA), Venus Mixson arrived to the Facility unannounced to conduct the required annual inspection and met with Emelita Price, the Administrator and introduced herself. The LPA stated the purpose of the visit. The File review was conducted in the office and additional forms were requested and reviewed at the facility. LPA Mixson toured the facility, along with Emelita, and inspected the inside and outside of the facility. The facility is a single story building located at 2771 CAMBRIDGE AVENUE Hemet, CA. 92545. The facility phone number is (951) 282-2819 and it is operable. Physical Plant: The physical plant, is in good condition, neat, and orderly. Outdoor and indoor passageways are free of obstruction at the time of this visit. The facility rooms have the required furniture; such as tables, chairs, storage space, and sufficient lighting. The building temperatures throughout was per regulations. The bedrooms are equipped with the required items, per Title 22. The hot water temperature was tested, and within the range required for regulations. The restrooms were equipped with liquid soap and paper towels. LPA Mixson toured the kitchen it was clean and free of orders. The facility has emergency food and water. LPA Mixson inspected the common areas. The fire extinguisher was in the green and the facility takes the extinguisher for service. Carbon monoxide alarms, along with smoke detectors were observed. There was a locked and centralized storage area for medications. Medications are contained in bubble packs, and supplied by the Medicine Shoppee Pharmacy. The facility had a designated area for resident and staff files, and it was locked. Emergency disaster plans, personal rights, and complaint procedures were posted in a prominent area. There was adequate seating in the common areas and sufficient space for activities. LPA Mixson reviewed staff and resident files, and conducted staff interviews and no resident interview due to the resident being at the Day Program. There were no regulation violations observed during todays visit.
