Type of visit: Required - 1 Year
On 06/02/2026, Licensing Program Analyst (LPA) Lavinia Muscan arrived unannounced to conduct an annual inspection. LPA met with Administrator Brian Dejeu and explained the purpose of today's visit. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. There are no residents currently at the facility. LPA and Administrator toured the facility together to ensure there are no residents. The areas toured included resident rooms, bathrooms, kitchen, and common areas and outside area. The food supply is within compliance, 2 days of perishable and 7 days worth of non-perishable food items. Grab bars were present at the toilet and in the shower. All exits were unobstructed. There is a side gate for emergency access. LPA checked the kitchen area for the ability to prepare and store food. Knives and Sharp objects found to be locked. LPA observed the area used for medication to be locked and inaccessible to residents. LPA observed smoke detectors and carbon monoxide detector at the care home are operational. Fire extinguisher was ready for emergency use. All required postings were observed. No deficiencies were observed or cited per Title 22, CCR Regulations during this visit. May 29, 2025Facility evaluation reportReport on file
Type of visit: Prelicensing
On May 29, 2025 Licensing Program Analyst (LPA) Kevin Mknelly conducted a Pre-Licensing inspection. LPA met with the Administrator, Brian Dejeu. LPA toured Physical Plant, Common Areas, Bedrooms, Bathrooms, Kitchen and Medication Storage. Fire extinguisher is current and First Aid is fully stocked. Kitchen was clean and good repair. Licensee has knowledge of (7) seven (2) two day supply of non-perishable and perishable, and required emergency shelter in place supplies. Rooms inspected have appropriate items and are in good repair. Water temperatures requirements were reviewed. LPA observed centrally stored medications and toxins are to be kept locked and inaccessible to residents. Staff and resident files are to be set up to contain required documents. Facility will accept total capacity of six elderly residents. There are currently no residents in care. LPA observed this facility appears to be clean, safe, and secured. All common areas appear to be free from hazards, clean and in good repair. As of this date, the Department has received the fire clearance. During this visit, this facility is in substantial compliance and meets the minimum requirements for a RCFE license. Component III was completed. Application is pending further review and approval. Facility Type: RCFE Application Type: INITIAL Capacity: 6 Census (if any clients in care): COMP II Participants: BRIAN DEJEU Interview Method: Telephone interview On 4/4/25, applicant/administrator participated in COMP II. Identification of the applicant and administrator was verified through interview questions based on photo ID and other identifying personal information. During COMP II, applicant and administrator confirmed that they have read and understand community care facility licensing laws included in the Health and Safety Codes and the California Code of Regulations Title 22. Signed LIC 809 with copy of photo ID have been obtained. During COMP II, CAB analyst confirmed Applicant/Administrator’s understanding of following areas: 1.
