Type of visit: Required - 1 Year

Licensing Program Analysts (LPA) B. Miranda conducted an unannounced visit today for the facility’s annual inspection. LPA introduced themselves and was allowed entrance into the facility. LPA met with Artak Matinyan House Manager/Designee. Facility is licensed for 6 residents and has a current census of 4. There are no residents on hospice or home health. Water temperature was checked in the common bathroom which read at 109 degrees Fahrenheit. Fire Extinguisher was purchased June 9, 2025 and is within the safety regulation period. Smoke and carbon monoxide detectors were tested and in working order. Susan McCarty Administrator's Certification expired on March 21, 2026 and is currently pending recertification. Staff files were reviewed, but do not have the correct number of training hours for dementia or medication. Resident files were reviewed, complete, and current. P&I was verified and correct. Facility provided a log for disaster drills that are conducted monthly. First aid kit on site. Toxins and cleaning supplies are locked and inaccessible. There is a locked storage for medications. R1's centrally stored medication log was not complete. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms, bathrooms, medication storage, kitchen, garage and outdoor areas. Bedrooms were clean, properly furnished, with adequate lighting, and in good repair. Food supply is adequate for 2-day perishable and 7-day nonperishable. Deficiencies observed were cited during today's inspection per California Code of Regulations, Title 22. TSP was offered and Artak will follow-up with Susan and left LPA know. LPA's requested the following documents: LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, LIC 610-E the Emergency Disaster Plan and copy of current Administrator’s Certificate to update the facility file. Listed documents shall be sent to Licensing by May, 11, 2026. Type of visit: Required - 1 Year

On 05/28/2025, Licensing Program Analyst (LPA) Daiquiri Boyd arrived unannounced to conduct an annual inspection. LPA introduced self, stated the purpose of the visit and requested to meet with the Administrator. LPA met with Administrator, Susan McCarty, who arrived shortly after. Facility email address and phone numbers were verified. LPA conducted a tour inside and outside of facility. Facility observed to be clean, odor free and at a comfortable temperature of 72 degrees. Common areas were furnished well with adequate seating and lighting available. Resident rooms appeared clean and had required furnishings. Resident bathrooms were properly equipped with securely fastened grab bars in toilet and shower areas, non-skid mats were observed. Hot water measured at 116.7 degrees F. Kitchen toured, appeared clean, food supply was checked. Exterior tour conducted, all exits open and free of obstructions. Swimming pool has a sturdy fence with a locked gate. Two storage sheds used for general storage, were checked and found to be locked with old furniture and garden supplies. All cleaning supplies are locked and secured under the kitchen sink. LPA observed the home to have the required postings located on the wall by the front door. LPA reviewed the homes Disaster and Emergency Plan and found it needed to be updated and was missing some details as to how the plan was to be executed; such as who is making calls to day programs and who is shutting off utilities if an evacuation is eminent. Facility sketch needs to be updated with evacuation details, meeting place, and exit routes, and utility shut off locations. Emergency food supply was observed, but found to be short in supply. Administrator is purchasing emergency food to be kept at facility. LPA observed that there was no variety in the stored food, having many cans of the same type. Fire extinguisher serviced on 06/04/2024. Smoke detectors and carbon monoxide detectors observed operational during today’s inspection. Last fire drill conducted on 05/05/2025. LPA reviewed staff and client records. All staff and client files were found to be in good order. Medications reviewed and observed to have original labels and be administered as prescribed. (continued on next page) Exit interview conducted and a plan for Administrator to submit updated facility sketch and Disaster Plan was reviewed and developed with Administrator. A copy of this report was provided to Administrator, Susan McCarty whose signature on this form confirms receipt of this document. On 04/16/2024, Licensing Program Analyst (LPA) Walton arrived unannounced to conduct an annual inspection. LPA introduced self, stated the purpose of the visit and requested to meet with the Administrator. LPA met with Administrator, Susan McCarty. LPA conducted a tour inside and outside of facility. Facility observed to be clean, odor free and at a comfortable temperature. Common areas were furnished well with adequate seating and lighting available. Resident rooms appeared clean and had required furnishings. Resident bathrooms were properly equipped with securely fastened grab bars in toilet and shower areas, non-skid mats were observed. Hot water measured at 115.6 degrees F. Kitchen toured, appeared clean, food supply was checked. Exterior tour conducted, all exits open and free of obstructions. Fire extinguisher serviced on 06/09/2023. Smoke detectors and carbon monoxide detectors observed operational during today’s inspection. Last fire drill conducted on 04/1/2024. All cleaning supplies are locked and secured under the kitchen sink. LPA reviewed staff and client records. Upon review of P& I records, LPA found resident funds are commingled with facility funds. Medications reviewed and observed to have original labels and be administered as prescribed. A deficiency is being cited in accordance to California Code of Regulations, Title 22, Division 6 on the attached 809D. Exit interview conducted and a plan of correction was reviewed and developed with Administrator. A copy of this report and appeal rights were discussed and provided to Administrator, Susan McCarty whose signature on this form confirms receipt of this document.
