Type of visit: Case Management - Deficiencies

On September 15, 2026, at 4:00 PM, Licensing Program Analyst (LPA) Sulma Lopez arrived unannounced at the facility to conduct a case management visit. LPA Lopez met with the Administrator Jay Warraich and explained the purpose of the visit. The current census was 4 residents. The purpose of the visit, is in response to learned deficiencies during a complaint investigation. Based on observations,interviews, and record reviews, it was learned that the facility recently hired a new staff member. Staff 1 (S1) was hired on September 8, 2026 and has not completed background clearance. Per interviews with S1 and A1, S1 has been training at the facility and can complete fingerprint scans today. S1 and A1 explained that S1 worked at the facility on the following dates: September 8,9,10,12,15. An immediate civil penalty of $500 was issued today and will be continue to assess at $100 per day until corrected. Please see attached LIC 421BG. Plan of correction: Licensee immediately dismissed S1 from the facility. The Licensee agreed to submit fingerprint clearance for S1 to the LPA by POC date 9/16/2026 by 11:59PM.

Type of visit: Required - 1 Year

On 7/31/26 at 2:30pm Licensing Program Analyst (LPA) Kevin Gould arrived at Era at Vintage Park for the purpose of conducting a required 1 year annual inspection. LPA met with Administrator, Jaskaran Warraich and together conducted a tour of the home. LPA and Administrator evaluated the physical plant to ensure the health and safety of the residents in care. Areas inspected are including but not limited to the kitchen, resident bedrooms; resident bathrooms, living and dining room and outdoor areas. LPA observed the facility to be free of odor, clean and in good repair. LPA observed that all rooms are equipped with the required furniture and sufficient lighting throughout the facility. LPA observed used syringes stored in a container that did not meet regulations Advisory not issued and Licensee is aware of the container type required. Licensee was able to describe appropriate disposal procedures. LPA also advised the complaint and ombudsman posters present should be a larger size and directed licensee where to obtain posters. LPA measured the water temperature, temperature measured at 106 degrees F which meets the 105-120 degree Fahrenheit regulation. LPA observed sufficient seven day non-perishable and two day perishable food supplies. LPA advised licensee to obtain some additional canned goods as the supply was nearing replenishment. Fire extinguishers and smoke detectors are current and in compliance with fire safety. LPA notes the facility had the required carbon monoxide detectors. First aid kit was checked and is complete. LPA observed centrally stored medications secure from residents. Per California Code of Regulations, Title 22 there were no deficiencies cited during today's inspection. COMP II by CAB successfully completed Method: Phone Call at CAB Applicant/administrator participated in COMP II at CAB telephone call with analyst at CAB. Identification of the applicant and administrator was verified by presenting photo ID via phone. During COMP II, applicant and administrator confirmed the understanding of Title 22. Component II was successfully completed. Applicant and administrator were advised to email/fax signed LIC 809 with copy of photo ID to CAB. Type of visit: Prelicensing

On July 2, 2025 at 9:02 AM, Licensing Program Analyst (LPA) Cynthia Tamayo arrived announced to conduct a Pre-Licensing Inspection of the facility to ensure compliance with Title 22 regulations. LPA met with applicant, Jaskaran Warraich (Licensee/Administrator) and Nischay Grewal (Licensee) assisted LPA in today’s inspection. This Applicant is seeking a facility license for a Residential Care Facility for the Elderly (RCFE) with a capacity of six (6). The facility is one story home with five bedrooms and two bathrooms. The facility will not have live in staff and will provide 24/7 care to residents. Bedroom #1-4 is approved for non-ambulatory residents and bedroom #5 is approved for one (1) bed ridden residents .There were no residents at this time. A brief interview with applicants was conducted. Jaskaran Warraich will be the Administrator of this facility. The facility administrator’s certificate # is 6066509740 and will expire 6/30/2026. The facility has an infection control plan and an emergency disaster plan completed and provided to Licensing for approval. LPA toured the facility and inspected the kitchen area. Cabinets and drawers were opened and reviewed at this time. Silverware, plates, and utensils were observed to be sufficient to meet the needs of six residents. Knives were observed to be made inaccessible to the residents at this time. The food storage units and facility refrigerator was observed to be functional and in good repair at this time. LPA observed enough food supplies for 2-day perishables and 7-day non-perishable quantities. Continued on LIC 809-C LPA observed cleaning supplies and laundry supplies were stored inside locked cabinets in the laundry room as well as the garage which is kept locked and inaccessible to residents. The kitchen facucet water temperature measured at 107.8, which is within the required regulation of 105 to 120 degrees Fahrenheit. There was a water temperature of 108.1 degrees Fahrenheit for bathroom #1 and bathroom #2, which is within the required regulation of 105 to 120 degrees Fahrenheit. The common area and dining area were observed to be furnished with enough seating to meet the needs of the 6 residents. LPA observed a cell phone (707) 699-0775 made available to residents in the common area; The phone rang with an option to leave a voice message. The facility smoke detectors and carbon detectors were observed to be in good condition. The fire extinguisher is current and located in the laundry room next to the kitchen. Linen closet was observed in the hallway and had enough supply of sheets, bedding, pillowcases, and blankets to meet the needs of six (6) residents. Residents’ bedrooms were toured, and furniture and furnishings were observed to meet the needs of the residents. LPAs observed the centrally stored medication areas to be locked near the dining area. LPA inspected the first aid kit, and it was complete. LPA observed there was enough supply of hygiene items on the premises made available to residents in care. LPA observed facility has a designated area for residents and staff files, which is kept locked. LPA also observed required posters posted. LPA toured the garage and observed washer and dryer and freezer in the garage with no accessible hazards. LPA toured the exterior physical plant and observed the back yard to have enough shade and furniture made available for residents. LPA observed a locked shed where gardening supplies and spare equipment is kept. LPA Tamayo observed there were two exit gates in the yard with self-latching mechanisms. Licensee/Administrator will ensure to notify LPA once their first resident moves in. A Component III was completed at this time with Nischay Grewal and Jaskaran Warraich. The applicant has passed the pre-licensing component of the application process. LPA Tamayo will notify the Central Application Bureau (CAB) that the pre-licensing has been completed and passed.
