May 12, 2026Facility evaluation reportReport on file

Type of visit: Case Management - Other

Licensing Program Analyst (LPA) Simi Rai conducted an unannounced Case Management visit. LPA Rai met with Administrator, Bani Kaur and stated the purpose of today's visit. LPA Rai observed 1 staff and 4 residents at the facility. The purpose of the visit was to follow up on an exception request for a Nephrostomy Tube for resident R1 that the facility submitted to the Department on 04/24/2026. During today's visit, LPA Rai spoke with Administrator (ADM) Bani. ADM stated R1 was admitted to the hospital on 11/28/2025 due to pain and was later transferred to the skilled nursing facility after discharging from the hospital. ADM stated on 04/13/2026, R1 was discharged from the skilled nursing facility back to the facility with Nephrostomy Tube. ADM stated the hospital told her that the Nephrostomy Tube was temporary and R1 had a scheduled surgery on 04/20/2026 to remove the Nephrostomy tube. ADM stated a licensed home health agency oversaw R1's medical supervision from 04/14/2026 to 04/20/2026. ADM stated after 04/20/2026, the resident has not returned back from the hospital and has submitted an exception request on 04/24/2026. ADM stated at this time, R1 does not have the Nephrostomy Tube and will notify the Department to rescind the exception request. ADM stated R1 will discharge back to the facility after assessment. Continuation on LIC 809-C, Page 1 of 2. Page 2 of 2. During today's visit, LPA Rai provided PIN 22-24-ASC - Collaborating With Home Health Agencies And Hospice Agencies To Provide Care To Residents. LPA Rai advised Administrator (ADM) Bani to review PIN with staff to ensure providing continuing care and supervision to residents in their RCFEs and must work collaboratively with HHAs and hospice agencies to ensure residents receive such care. Deficiencies were cited per California Code of Regulations, Title 22, please see LIC 809-D. Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment. Feb 27, 2026Complaint investigation reportUnfounded

Allegation investigated: Residents were left unsupervised at the facility.

Licensing Program Analyst (LPA) Simi Rai conducted an unannounced visit to conclude the complaint investigation. LPA Rai met with the Administrator (ADM), Bhupinder (Bani) Kaur and stated the purpose of today’s visit. On 1/20/2026, the Department received a complaint with the above allegation. On 1/27/2026, the Department conducted an initial investigation at the facility. It was alleged that on the evening on 1/19/2026, facility staff left the facility and the residents unsupervised. Continuation on LIC 9099-C, Page 1 of 2. Unfounded Page 2 of 2. On 1/27/2026, LPA Rai interviewed 3 staff (S1-S2) including Administrator (ADM) Bhupinder (Bani) Kaur. ADM stated there are staff present at the facility for care and supervision. ADM stated the evening on 1/19/2026, staff S1 was leaving the facility and said bye to the residents and staff S2 was present at the facility to oversee the residents. ADM stated the residents may have misunderstood thinking S1 left the facility and S2 was not present in the facility. S1 stated he/she left the facility on 1/19/2026 after the shift and S2 was present at the facility. S2 confirmed S1’s statement by stating he/she was present at the facility after S1 left the facility on 1/19/2026. On 1/27/2026, LPA Rai interviewed 5 residents (R1-R5). 4 Out of 5 residents did not want to be interviewed or did not provide any information regarding the allegation. R1 stated there was a misunderstanding on the evening 1/19/2026 when S1 left the facility after shift. R1 stated there was another staff S2 present at the facility and the residents were not left alone and unsupervised. Based on the review staff schedule for January 2026, LPA Rai reviewed two staff (S1 &S2) were scheduled for the AM/PM shift on 1/19/2026. LPA Rai confirmed with interviews with S1 and S2 that they were present at the facility during the evening of 1/19/2026. The Department has completed the investigation of the above allegation. Based on interviews conducted and record reviews, the Department has found that the above allegation were UNFOUNDED, meaning that the allegation were false, could not have happened and/or are without a reasonable basis. No deficiencies cited from California Code of Regulations, Title 22. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Steve Chang conducted an unannounced annual inspection visit, and met with Administrator (ADM) Bani Kaur. LPA observed 5 residents (R1-R5), 2 staff (S1, S2) in the facility. LPA toured the facility inside and out with ADM. License, Administrator Certificate, and personal rights posters were observed in the facility. LPA reviewed 3 resident file and 3 staff files. 1 out of 3 staff was observed with first aid certificate. ADM stated he/she has it somewhere at home. ADM stated he/she will send to LPA. Living room, kitchen, dining area, 2 restrooms, a office area, a staff live-in room, 5 resident rooms, and garage were inspected. Two day perishable food supplies and seven day nonperishable food supplies were observed sufficient. Medication closet was observed locked. Knives closet was observed locked. Dish washing soap solution was observed on the sink in kitchen, ADM put the dish washing soap solution in the cabinet under the sink and locked it. Room temperature was at 74 degree F, and hot water temperature was at 106 degree F in facility. Fire extinguisher was serviced on 12/02/2024. The facility was equipped with fire alarm system, smoke and carbon monoxide detectors. Carbon monoxide detector was tested, and was observed out of battery. ADM installed new batteries and Carbon monoxide detector was working. First aid box, night lights, and flash lights were observed in the facility. . Front yard and backyard were inspected. 1 storage rooms was observed at backyard. The last time the facility conducted the emergency and fire drill was on 9/3/2025. Type of visit: Prelicensing

Licensing Program Analysts (LPAs) Simi Rai and Marcela Yanez conducted a unannounced pre-licensing inspection for a change of ownership. LPAs met with Applicant, Bhupinder Kaur. The home currently has 5 residents. The facility is approved for Fire Clearance for 6 Non-Ambulatory residents. LPAs toured the facility inside and out. The facility is equipped with smoke and carbon monoxide detectors in working condition. Fire extinguisher observed in the home which were all serviced on 10/25/2023. The kitchen, dining and living room were observed in good repair. Resident bedrooms were observed in good repair, furnished, with clean linens and adequate lighting. Bathrooms were observed clean and equipped with grab bars. The water temperature range for facility bathrooms were 108 - 108.7 degrees F. The water temperature in the kitchen was measured at 106.6 degrees F. Centrally stored medication cabinet with first aid kit was observed locked. 2-day perishable and 7-day non-perishable food were observed in the kitchen. All outdoor and indoor passageways were observed clear and free of obstruction. The backyard was inspected, no bodies of water observed. LPAs observed laundry room and it is not being used as living quarters. Component III orientation was completed with applicant. No issues noted during this pre-licensing inspection. LPAs observed the facility is ready to be licensed. However, this report will be submitted to the Central Application Bureau (CAB) and a final review of the application will be conducted. This facility is not yet licensed and is subject to final approval by CAB. Additional requirements may still be required.
