On 08/26/2026, Licensing Program Analyst (LPA) M Vega arrived to the facility unannounced to conduct the required Annual Inspection Visit. LPA was greeted by staff, stated the purpose of the visit and was allowed entry into the facility. Licensee was contacted and arrived a short time later. Licensee an Administrator on record is Laura Edwards. LPA toured the facility inside and out and observed the house to be a 4 bedroom, LPA observed 4 residents in care at the time of visit. 3 out 4 residents were observed in the living room, sitting in individual recliners, watching TV. The facility temperature read at 76 degrees F. Resident bedrooms were observed to have the required lighting/furnishings and are free from odor and passageway obstruction or fire hazards. Mattresses and linen were observed in good condition. A supply of clean linens/towels were observed in a closet off the hall. Bathrooms were observed to have operational lights, running water, required grab bars and non-slip floors. Hot water temperature tested at 115.8 degrees F. Cleaning supplies and knives were observed to be locked cabinet located in the laundry room. LPA toured the kitchen observed the required 7-day supply of non-perishable food and 2- day supply of fresh perishables to be properly stored. Menus for the facility were observed. LPA observed fire extinguisher with a service date of 08/04/2026. Smoke alarm detectors and carbon monoxide detectors were observed to be operational. Last Disaster Drill was an Earthquake on 07/26. First aid kits were observed to contain all required items. Medications were observed to be in a locked cabinet located the laundry room. Continuation on LIC 809C Residents’ files were reviewed and observed to have update documents. A sample of staff files were also reviewed. Staff files were observed to have current First Aid/CPR. Staff are fingerprinted clear and associated to the facility. LPA observed the Emergency Disaster Plan and Infection Control Plan. Type of visit: Case Management - Incident

On February 6, 2026 Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to conduct a case management for a visit previously reported incident. LPA was allowed entrance into the facility by Caregiver Maria Avila Ochoa. LPA spoke with House Manager Katelyn Hernandez via phone. LPA observed the facility to be clean, odor free, and clutter free. LPA explained the reason for the visit was due to an incident that was reported to the Dept. about R1 waking up and having bruising on their face. LPA spoke with House Manager Katelyn via phone. Katelyn stated R1 has been more agitated towards staff. Katelyn explained R1 might have hit their head/face on the bed rails which may have caused the bruising. Katlyn stated the family was inform and the bed rails have been cushioned with blankets. LPA did not observe any bruising on R1's face during the visit. Caregiver Maria stated R1's agitation has increased. LPA did not observe any deficients at this time and no citations were issued. On 09/15/25, Licensing Program Analyst (LPA) L. Salazar arrived to the facility unannounced to conduct the required Annual Inspection Visit. LPA was greeted by staff stated the purpose of the visit and was allowed entry into the facility. Administrator on record is Laura Edwards. LPA toured the facility inside and out and observed the house to be a 4 bedroom, 3 bathroom house. LPA observed 4 residents in care at the time of visit. 3 out 4 residents were observed in the living room, sitting in individual recliners, watching game shows on TV. 1 out of 4 residents are receiving Hospice care services. 1 out of 4 residents are receiving home health services. 2 out of 4 residents are receiving Regional Center services. The facility temperature read at 76 degrees F. Resident bedrooms were observed to have the required lighting/furnishings and are free from odor and passageway obstruction or fire hazards. Quarterly Emergency Disaster Drill logs were observed for staff. LPA observed on the LIC 610E (Emergency Disaster Plan) with emergency numbers and evacuations locations was posted in the hallway. An exit interview was conducted with Administrator. A copy of this report was discussed and provided at the time of visit. No deficiencies cited on today's visit. Allegation investigated: Staff are mishandling the residents medications Staff have expired food for the residents. Staff are using a manual locks for the exit doors

On 11/12/24, Licensing Program Analyst (LPA) L. Salazar arrived to the facility unannounced to deliver findings on the above allegations. LPA was greeted by caregiver, stated the purpose of the visit and was allowed entry into the facility. LPA called Licensee/Administrator to discuss findings over the phone however, Licensee/ Administrator was unavailable. During the investigation, LPA conducted a facility tour, reviewed records and conducted interviews. Based on the information received, medications were observed to be stored and documented according to physician's orders. A 7-day supply of non perishable's and a 2-day supply of perishables were observed to be in good quality and quantity. Manual locks were not observed being used and were removed on 08/06/24. Although the allegations may have happened, there is not a preponderance of evidence to prove that the alleged violations occurred, therefore the allegations are Unsubstantiated. LPA was greeted by caregiver, Rosa Garcia, stated the purpose of the visit, and was allowed entry in the facility. Administrator (ADM) on record is Laura Edwards. LPA spoke to ADM via telephone and was allowed to begin the annual inspection with caregiver until Administrator's arrival. Administrator arrived to the facility shortly after the telephone call. LPA toured the facility inside and out and observed the house to be a 5 bedroom, 3 bathroom house. LPA observed 4 residents in care at the time of visit. 3 out 5 residents were observed in the living room, sitting in individual recliners, watching game shows on TV. 2 out of 5 residents are receiving Hospice care services. 1 out of 5 residents are receiving Regional Center services. The facility temperature read at 71 degrees F. Resident bedrooms were observed to have the required lighting/furnishings and are free from odor and passageway obstruction or fire hazards. LPA reviewed a sample of residents' medications, MARS, and Centrally Stored Medication and Destruction Record (CSMDR) and observed medication to be logged according to physician's records. A covered seating area under the patio was observed. Backyard gate is self-closing and self-latching. LPA is requesting the following updated forms to be sent to the Regional Office by 08/19/24. Bathrooms were observed to have operational lights, running water, and non- slip floors. Hot water temperature tested at 110 degrees F. Cleaning supplies were observed to be locked cabinet located in the garage. LPA toured the kitchen observed the required 7-day supply of non-perishable food and 2- day supply of fresh perishables to be properly stored. Menus for the facility were observed. Carbon monoxide detectors were observed to be operational. A fire extinguisher was observed with a service date of 01/09/23. First aid kits were observed to contain all required items. Medications were observed to be locked cabinet located the laundry room. Quarterly Emergency Disaster Drill logs were observed for staff. A sample of Resident and Staff files were reviewed and observed to have the required forms.
