Allegation investigated: Illegal eviction
On 5/20/26 at around 8am Licensing Program Analyst (LPA) K. Nguyen arrived unannounced to conduct an investigation and deliver findings on the above allegation. LPA explained the purpose of the visit to the administrator (ADM) Grace Reano- Aquino. Allegation: Illegal Eviction: Unsubstantiated During the investigation, the Licensing Program Analyst (LPA) conducted interviews with Resident 1 (R1) and staff members and reviewed relevant resident records and facility documentation regarding the allegation that the facility was conducting an illegal eviction, including but not limited to R1's physician report, needs and service plan, Kaiser notice of action letter, invoices, and communication between the facility and Kaiser. Report continued on LIC 9099c… Unsubstantiated Report continued… During an interview with LPA, R1 stated that Staff 1 (S1) personally provided R1 with a written eviction notice and explained the reason for the eviction. R1 reported that S1 informed R1 that there was an outstanding balance of approximately $30,000 owed to the facility after Kaiser reportedly discontinued payment for R1’s care following approximately four and a half years of residency at the facility. LPA confirmed that the Department received a copy of the eviction notice from the facility on 04/27/2026. LPA reviewed the notice and supporting documentation and determined that the facility had followed the required eviction procedures in accordance with applicable regulations. Additionally, R1 stated that facility staff continued to provide care and assistance during the eviction process and reported that staff were actively helping R1 secure alternative placement. Specifically, R1 stated, “Staff still assist me and are helping me find a place to stay.” During an interview with S2, LPA learned that the facility had been coordinating with a placement agency to assist R1 in locating and transitioning to another appropriate care facility. Documentation and staff interviews indicated that R1 was scheduled to relocate to another facility on 05/26/2026. The reporting party also alleged that R1 required assistance with the AWL Waiver. However, information obtained during the investigation revealed that coordination of those services was being managed through Kaiser’s Case Management (CM) services and was not related to the eviction process itself. Based on interviews conducted and records reviewed, there was insufficient evidence to support the allegation that the facility conducted an illegal eviction. Therefore, the allegation is Unsubstantiated. On 4/29/26 at around 9 am, Licensing Program Analyst (LPA) K. Nguyen arrived unannounced to deliver findings for the above allegations. LPA explained the purpose of the visit to the administrator (ADM), Grace Reano-Aquino. Allegation: Facility is not maintained at a comfortable temperature: Unsubstantiated During the course of the investigation, LPA reviewed residents’ files and interviewed 6 residents, ADM, and 5 staff. It was alleged that the facility is not maintained at a comfortable temperature. Based on interviews conducted with six (6) residents and five (5) staff members, as well as observations made during the visit, the facility maintained a comfortable temperature at 70 degrees Fahrenheit. R1 stated that “the previous place that I’ve stayed at is always cold, not like here, very comfortable.” Residents provided mixed responses regarding the temperature within the facility. Several residents stated that the facility temperature was comfortable and that they could request adjustments if needed. Staff reported that the heating and cooling systems were functioning properly and stated that efforts are being made to maintain a comfortable indoor environment for residents. During the visit, LPA did not observe any conditions indicating that the facility temperature posed a health or safety concern. Allegation: Staff do not treat residents with dignity and respect: Unsubstantiated During the course of the investigation, LPA reviewed residents’ files and interviewed 6 residents, ADM, and 5 staff. It was alleged that the staff does not treat residents with dignity and respect. Based on interviews conducted with six (6) residents and five (5) staff members, as well as observations made during the investigation. Residents provided mixed responses regarding staff interactions; however, the majority stated that staff are respectful, assist with their needs, and communicate appropriately. R1 stated, “I was talking about the old place that I’ve stayed at, which did not respect me, and yelled at me”. Staff interviewed stated that residents are treated with dignity and respect, and that they are expected to maintain professional, respectful interactions with all residents. During the visit, LPA observed staff interacting appropriately with residents in a calm and respectful manner. Report Continue on LIC 9099c1... Report Continued LIC9099c1.. Based on interviews conducted with six (6) residents and five (5) staff members, as well as observations made during the investigation. R1 stated, “At the old facility, staff don’t come and help me. Staff here come when I ask them for help”. Residents provided mixed responses regarding staff response times; however, several residents stated that staff generally respond when assistance is requested and that their needs are addressed appropriately. Staff interviewed reported that they make efforts to respond to residents’ requests as promptly as possible and provide assistance based on residents’ care needs. During the visit, LPA observed staff responding to resident requests and providing assistance throughout the facility. Report continued on LIC 9099c2... Report continued LIC9099c2... Allegation: Facility is not wheelchair accessible: Unsubstantiated During the course of the investigation, LPA reviewed residents’ files and interviewed 6 residents, ADM, and 5 staff. It was alleged that the facility is not wheelchair accessible. Based on interviews conducted with six (6) residents and five (5) staff members, as well as observations made during the investigation. Residents interviewed provided mixed responses regarding accessibility within the facility; however, several stated they can move throughout the facility and access common areas without issue. R1 stated, “At the old facility, I could not go anywhere around because the place didn’t have a ramp for me to access. This facility, I use my wheelchair with no issue”. Staff reported that the facility is maintained to accommodate residents’ mobility needs and stated that accommodation is provided as needed. During the visit, LPA observed accessible pathways throughout the facility, including common areas and resident living spaces. No observations indicated that the facility was inaccessible to wheelchair users or that residents were prevented from accessing areas of the facility due to mobility barriers. Although the allegation may have occurred or be valid, there is not a preponderance of evidence to prove whether the alleged violation did or did not occur; the above allegations are unsubstantiated. Type of visit: Case Management - Deficiencies
On 4/2/26 LPA K. Nguyen conducted case management as a result of an observation during a complaint visit (15-AS-20260325154357) during the facility tour. THE FOLLOWING DEFICIENCIES WERE OBSERVED DURING THE VISIT: At 11:44 AM, LPA observed an exit door in the facility having a sliding Bolt lock on the exit door. LPA was not able to open the exit door. Staff stated that they lock it for resident safety. A civil penalty of $500 is assessed. At 12 PM, LPA observed that the door leading out to the patio is not working properly. The Facility was cited from the California Code of Regulations, Title 22, and/or Health and Safety Code. Failure to correct deficiencies by the POC date may result in additional Civil Penalties. Type of visit: Required - 1 Year
On 2/5/2026 at 8:30 AM, Licensing Program Analyst (LPA) K. Nguyen arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Administrator, Grace Reano- Aquino and explained the purpose of the visit. The administrator certificate number: 7000015740. LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of 19 bedrooms in total bedrooms which 18 bedrooms are occupied by the residents, and 0 bedroom is occupied by staff. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for residents is maintained at 75-degree Fahrenheit. LPA observed lighting in all rooms is adequate for the comfort and safety of the residents. Hot water temperature in the shared resident’s bathroom was measured at 110.7-degree Fahrenheit. All toilets, hand washing and bathing are safe, sanitary and in operating condition. The supply of extra hygiene was available for clients. There is a minimum of one week supply of nonperishables and 2-day perishables food supply. Report Continued on LIC 809c... Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher was last serviced on 03/18/2025. Emergency Disaster Drill was last posted on 2/3/25. First aid kit was observed to be complete. Fire drill was last conducted on 2/3/26. Liability Insurance effective on 11/28/25 to 11/28/26. At 9:30am, 6 resident records were reviewed. At 11am, 5 staff records were reviewed and 5 of 5 have current first aid training and are associated with the facility. A sample of 4 client’s medications were reviewed. LPA met with Grace Aquino, administrator (ADM), and informed the reason for visit. LPA interviewed Administrator regarding the incident. Administrator called and made police report regarding the unwitnessed physical altercation between R1 and R2. R1 stated R2 was hitting R1 with R1 mug yelling that R1 is a “Chinese Grandma Prostitute in Chinese and started hitting R1.” ADM called the police R2 got sent to John George. R1 came back from the hospital and is recovering. R2 is back from John George and are scheduled to see the psychiatric on 9/19/25. Type of visit: Required - 1 Year
On 2/12/2025 at 10:00am, Licensing Program Analyst (LPA) K. Nguyen conducted an unannounced 1-Year Required inspection. LPA met with Grace Reano-Aquino, Administrator. The Administrator currently holds a certificate (#6000611740) that expires on 07/07/2025. The facility’s fire clearance was approved for thirty-five (35) residents. LPA toured the facility with Administrator including but not limited to bedrooms, bathrooms, kitchen, common area, back and side yard. The facility consists of nineteen (19) total bedrooms, and six (6) bathrooms. No bodies of water observed. A comfortable temperature is maintained at 70 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 105.7 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars. Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher was last serviced on 3/29/2024. Emergency Disaster Plan was posted and updated on 2/3/2025. Liability Insurance effective date from 11/28/24 to 11/28/2025. First aid kit was observed to be complete. LPA reviewed four (4) staff files and seven (7) resident file which were all found to be complete. Continued on LIC809C... LPA observed the following deficiency: · At 12:30pm, LPA observed dish wash detergent, Lysol, Clorox wipes unlocked underneath kitchen sink, and inside unlock storage outside in the backyard area. (Deficiency Clear during visit) The deficiencies were observed (see LIC809D) and cited from the California Code of Regulation, Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiency within a 12-month period may result in civil penalties. Type of visit: Case Management - Incident
On this day, September 5, 2024, Licensing Program Analyst (LPA) K. Nguyen arrived unannounced to conduct a case management due to an SOC 341 between resident on resident. LPA met with Grace Aquino, administrator, and informed the reason for visit. LPA interviewed Administrator regarding the incident. Administrator called and made police report regarding the unwitnessed physical alteration between R1 and R2. R2 was the one that provoke R1, so R1 cannot control R1 anger, but after R1 apology to R2 and everyone that R1 cannot control anger issue. R2 is still in the hospital, due to R2 needs higher level of care. Administrator and R2 family were discussing the higher of care for R2 prior to this incident, because R2 tend to have aggressive combative behavior, and server dementia. R1 is back from John George and continue to take medication daily. Requesting: Police report or an incident case number from police No deficiency observed on this day.
