Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to conduct an annual required inspection. LPA Valerio met with facility staff, and explained the purpose of the visit. LPA was shortly met by Administrator Okay Onwuli. LPA Valerio and Administrator toured the facility to ensure compliance with Title 22 regulations. LPA observed the common area to be fully furnished and organized. Residents were sitting in their preferred lounge chair watching a sports channel and eating snacks. The dinning area was observed to be clean, fully furnished, and no hazardous materials. The kitchen was observed to have work appliances. The facility was observed to have an adequate supply of food. The facility was observed to have an adequate food supply. Medications, sharps, and toxins were locked and inaccessible to residents in care. Resident bedrooms were observed to clean, organized, and free from odors. Resident bathrooms were fully stocked with hygiene supplies and sanitary. The backyard was observed to have an area for seating. The shed was utilized for storage. Walkways were clear from obstructions. The fire extinguisher was fully charged and within compliance. LPA Valerio reviewed facility files. Resident files were observed to be up to date with required documentation. The date of the last fire drill was in March 05 2026. LPA Valerio requested the following be sent to CCLASCPSacramentoSouthRO@dss.ca.gov: LIC 500, LIC 308, LIC610, and copy of liability insurance. On 3/27/25 at 9:00am Licensing Program Analyst (LPA) Kevin Gould arrived at Diamond Oak Senior Care for the purpose of conducting a required 1 year annual inspection. LPA met with Administrator, Okay Onwuli and together conducted a tour of the home. LPA and Administrator evaluated the physical plant to ensure the health and safety of the residents in care. Areas inspected are including but not limited to the kitchen, resident bedrooms; resident bathrooms, living and dining room and outdoor areas. LPA observed the facility to be free of odor, clean and in good repair. LPA observed that all rooms are equipped with the required furniture and sufficient lighting throughout the facility. LPA conducted file review for three residents and two staff members, LPA observed both the still files reviewed did not show sufficient training hours documented to meet the annual training requirements of 20 total hours of which 8 hours should be dedicated to dementia care. LPA measured the water temperature, temperature measured at 121 degrees F which meets the 105-120 degree Fahrenheit regulation. LPA observed sufficient seven day non-perishable and two day perishable food supplies. Fire extinguishers and smoke detectors are current and in compliance with fire safety. LPA notes the facility had the required carbon monoxide detectors. First aid kit was checked and is complete. LPA observed centrally stored medications secure from residents. LPA Requested the following documents for facility file: LIC 308 Designation of Facility Responsibility, LIC 500 personnel report, LIC 9020 client roster and current administrator certificate. Per California Code of Regulations, Title 22 the following deficiencies are cited during today's inspection. Allegation investigated: Staff mismanged resident's social security funds

Licensing Program Analyst (LPA) Victoria Brown arrived unannounced to conduct an investigation of the above mentioned allegation on 5/3/24 at 1p. LPA met with Caregiver Perlita Lasap who contacted Administrator regarding todays visit. LPA reviewed Resident #1 (R1)'s file, and Interviewed CEPS representative during this visit. LPA received a copy of the email sent to Administrator stating the SSI Overpayment is required to be refunded as R1 was hospitalized on Janurary 31, 2024. LPA observed that a friend of R1 removed the belongings on 4/22/24 as R1 was relocated due to a need of higher level of care. Although the Licensee would be entitled to the bedhold monies until all belongings were removed, this complaint is in regards to overpayment of monies ($916.73) which were not due to the resident and it is due back to SSI through way of check to CEPS. The Title 22 regulations states under Admission Agreement (8) (A) General facility policies...All facility policies...shall not violate any applicable rights, laws or regulations. Substantiated Based on interview, confirmation the refund has not been received, and review of email sent to Administrator the allegation is deemed Substantiated. The above allegation is SUBSTANTIATED. A finding that the complaint allegation is Substantiated means that the allegation is valid because the preponderance of the evidence standard has been met. Licensing Program Analyst (LPA) Victoria Brown arrived unannounced to conduct a Required - 1 Year visit on 2/13/24 at 1:45pm. LPA observed the Administrator Certificate expires 9/25/25 for Okay Onwuli. LPA met with Liz Magpiong, Caregiver and stated the purpose of todays visit. Caregiver contacted the Administrator who was unable to assist with the visit. The facility is licensed for a capacity of 6 non-ambulatory residents of which 6 may receive hospice care services in rooms 2,3,4,and 5. There is 0 residents receiving hospice care services during this visit. LPA toured and inspected the physical plant inside and outside to ensure there are no safety hazards to residents. LPA observed 2-day perishables and 7-day non-perishables. The most current disaster drill was conducted on 1/6/24. LPA observed LIC308 Designation of Responsibility during this visit. Facility staff roster provided during this visit. The temperature inside the facility was observed to be at 73*F which is within the required range of 68-85*F. The hot water temperature was measured at 118.7 *F which is within the required range of 105-120*F. LPA observed fire extinguisher(s), smoke and carbon monoxide detectors, and central heating and air in the facility. LPA observed the centrally stored medications area to be locked and inaccessible to residents. The first aid kit contained the required items such as sterile dressings, bandages, adhesive tape, scissors, tweezers, thermometers, antiseptic solution and guide. LPA reviewed 1 staff and 1 resident files and conducted interviews during this visit.
