Licensing Program Analyst (LPA) Daniel Konishi conducted an unannounced Required-1 year visit and met with Direct Support Professional, Leticia Aguilar Echeverria and LPA explained the purpose of the visit. Administrator Hector Vargas and House Manager, Wendy Vasquez, and Staff, Patricia Wong, arrived shortly after and LPA explained the purpose of the visit. The facility is licensed to care for (6) non ambulatory residents age 60 and over. The initial annual visit was conducted on 09/23/2025. During the initial visit the following nine (9) Compliance and Regulatory Enforcement (CARE) tool domains were observed and reviewed: Infection Control, Operational Requirement, Physical Plant Environmental Safety, Resident Right-Information, Planned Activities, Food Services, Incidental Medical and Dental, Disaster Preparedness, and Resident with Special Health Needs. During today’s annual visit, the following three (3) Compliance and Regulatory Enforcement (CARE) tool domains were observed and reviewed: Staffing, Personnel Records-Training, and Resident Records-Personnel Reports. Staffing: A total of nine (9) staff members including the Administrator provide care and supervision to the client. Staff employed are over the age of 18 and have criminal background clearance, fingerprint cleared, have training and are associated to the facility. LPA reviewed (4) staff files which includes: Personnel Record, Health Screening, TB Clearance, fingerprint background clearance, Employee Rights, valid 1st Aid/CPR training, and ongoing staff training. Administrator certificate expires on 06/23/2026. Resident Records-Incident Reports: LPA reviewed five (5) resident files which include: Face Sheet, Admission Agreement, Physician's Report (including TB and Ambulatory Status), Appraisal and Needs Services plan, Pre-Admission Appraisal (Physical/Social Assessment), and Resident Personal Rights. Per California Code of Regulations, Title 22, and California Health and Safety Code, no deficiencies observed during the visit. Licensing Program Analyst (LPA) Daniel Konishi conducted an unannounced Required-1 year visit and met with House Manager, Wendy Vasquez. LPA explained the purpose of the visit. The facility is licensed to care for (6) non ambulatory residents age 60 and over. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following: Infection Control: Infection control practices and Personal Protective Equipment (PPEs) were maintained. Bathroom has hygiene items such as paper towel, hand soap and toilet paper. LPA reviewed the Infection Control Plan in place. Operational Requirements: The fire clearance is in place. LPA observed a valid Liability Insurance policy and a valid Surety Bond Insurance in place. Last Fire/Disaster Drill was conducted on 07/09/2025. Physical Plant/Environment Safety: The facility is a single-story home located in a residential neighborhood which consists of (3) resident bedrooms, (3) bathrooms, living room with fireplace, kitchen, dining area, laundry/storage area, side yard and backyard. The interior and exterior physical plant was inspected. Resident bedrooms were toured. Each bedroom has a smoke detector, linen, light, chair and sufficient closet space. Resident beds have the required linen, and the linen is in good condition. The bathrooms were observed to be clean and operational. Extra linens and towels are located in the hallway closet. Side yard and backyard were inspected and have a shaded and sitting area. There are (2) fire extinguishers in the facility which was last serviced on 04/15/2025. Carbon monoxide detectors were tested and operable. There are no firearms or weapons stored at the facility. No open bodies of water. Cleaning supplies are locked and inaccessible to residents. Fireplace is covered and inaccessible to residents. Physical Plant/Environment Safety [Cont.]: LPA tested hot water temperature and measured readings were 106.1 deg. F in bathroom #1, 123.6 deg F in bathroom #2 and 125.03 deg F in bathroom #3 which are not within Title 22 Regulations. Resident Rights-Information: Resident personal rights, complaint hotline information and visitors’ policy posters are posted. Facility provides internet services to all residents and they have access to the facility phone. Planned Activities: Sufficient space to accommodate both indoor and outdoor activities was observed. Food Service: Kitchen, food preparation area, and storage areas were observed to be clean and sanitary. There is sufficient food supplies of 2-day perishable and 7-day supplies of non-perishable items. The food is properly stored in the refrigerator. There are no residents with special diets residing at this facility. Pesticides and cleaning supplies are kept away from the food preparation areas. Health Related Services: The medications are centrally stored and in their original containers and bubble packed. LPA reviewed medication for (6) residents. The facility uses the Medication Administration Record (MAR) log to document medications given. Medications are administered as prescribed by the Physician. First-aid kit has been reviewed and has all required items. Incidental Medical Services: There are no residents in the facility with incidental medical services nor have a restricted health condition. Disaster Preparedness: The facility has a complete Emergency Disaster and Mass Casualty Plan. Residents with Special Health Needs: Per House Manager, there is one (1) resident that receives home health services and no residents that receive hospice services. No residents have prohibited health conditions. ***Due to time constraints, LPA was not able to complete the annual inspection for this facility. LPA will do a continuation of this inspection at a later date.*** Per California Code of Regulations, Title 22, and California Health and Safety Code, the deficiencies observed during the visit are documented on the LIC809-D. Licensing Program Analyst (LPA) Bennette Pena conducted an unannounced Required-1 year visit. LPA arrived at the facility and rang the doorbell, but no one answered. LPA called the facility and spoke with Patricia Wong who contacted another staff who will be coming to the facility to assist. At 10:04am, Marta Ramos, Asst. Wellness Coordinator arrived but had the wrong keys and could not open the door. At 10:20am, Wendy Vasquez, DSP Supervisor arrived to let LPA in and explained the purpose of the visit. At 10:45am, Hector Vargas arrived and assisted with the inspection. The facility is licensed to care for (6) non ambulatory residents age 60 and over. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following: Infection Control: Infection control practices and Personal Protective Equipment (PPEs) were maintained. Bathroom has hygiene items such as paper towel, hand soap and toilet paper. Operational Requirements: A current Plan of Operation was reviewed. The Infection Control Plan has been added to the Plan. A fire clearance is in place. Liability Insurance policy is valid and will expire on 07/15/2025. Surety Bond Insurance with bond amount of $3750 is valid and will expire on 02/28/2025. Last Fire Drill was conducted on 10/22/2024 and according to the Administrator, emergency drills are conducted on a quarterly basis. Physical Plant/Environment Safety: The facility is a single story home located in a residential neighborhood which consists of (3) resident bedrooms, (3) bathrooms, living room with fireplace, kitchen, dining area, laundry/storage area, side yard and backyard. LPA observed that the coating on the bathtub in bathroom #1 was chipping and worn out. Side yard and backyard were inspected and have a shaded and sitting area. There are (2) fire extinguishers in the facility which was last serviced on 04/24/2024. Smoke alarms and carbon monoxide were tested and operable. There are no firearms or weapons stored at the facility. Water temperature reading measured within the required 105 - 120 degrees Fahrenheit. Water readings were 112.2 deg. F in bathroom #1, 109.5 deg F in bathroom #2 and 110.6 deg F in bathroom #3. *****REPORT CONTINUED ON LIC809-C***** Staffing: A total of nine (9) staff members including the Administrator provide care and supervision to the client. Administrator certificate expired on 06/23/2024, but sent renewal to CDSS on 05/13/2024. Resident Rights-Information: Facility provides internet services to all residents and have access to the facility phone. Planned Activities: Sufficient space to accommodate both indoor and outdoor activities was observed. Food Service: There is sufficient food supplies of 2-day perishable and 7-day supplies of non-perishable items. The food is properly stored in the refrigerator. Kitchen was inspected and the milk stored in the refrigerator expired on Nov. 17. Kitchen is kept clean and free from rodents and other vermin. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Ashley Calderon conducted an unannounced site visit for the Required - 1 Year inspection. Upon arriving at the facility, LPA discovered there was no one present. LPA called facility phone number and shortly after Staff member / Medical Coordinator/ Martha Ramos arrived and later Wendy Vasquez/ DSP Supervisor arrived at the facility and assisted with the visit. The facility is licensed to serve six (6) Non-ambulatory Residents ages 60 and over. The facility is vendored with the San Gabriel/ Pomona Regional Center, level RCFE 4C. LPA used CARE Inspection tool to conduct todays visit, a tour of the physical plant was conducted alongside with Martha Ramos, medication was reviewed with Wendy Vasquez, staff and resident file review and staff interviews were conducted. No resident's present during visit therefore no resident interviews were conducted. The facility is located in a residential area. A tour of the single-story facility includes: (3) resident bedrooms, (2.5) resident bathrooms, living room/ office area, dining room, kitchen, laundry room/ pantry, (2) backyards observed and no large body of waters observed. Kitchen and food supply was observed, operable equipment and thermometers in place for the ability to prepare and serve food. LPA observed an appropriate food supply of two (2) days of perishables and one week (7 days) of non-perishables, facility has a pantry with food supplies. All storage areas for cleaning solutions, toxins, knives, and hazardous items are in a secured cabinet and inaccessible to clients. Kitchen Cabinets a total of two drawers are not on track, un-leveled and not sturdy, deficiency noted. There is a functioning telephone and internet on the premises. Continuation 809-C... The bathrooms are clean and operational. Hallway Bathroom were observed water stains / marks , brown and black spots observed on the wall and ceiling and stains and cracks observed on the door casing frame, and paint coming off the wall. Uneven tiles with missing door threshold between the hallway floor and bathroom fall, causing a tripping hazard, deficiency cited. The hot water temperature was tested throughout the facility and warning sign for water temperature that exceeds over 120F posted above the kitchen sink (used staff only). -Hallway was observed to have water stain on ceiling and on door casing frame leading to the pantry room, deficiency cited. -Flooring was observed to have uneven, piling and indented and cracked titles in dining room, living room and in pantry room, deficiency cited. Residents bedrooms had required furniture, lighting and bed sheets/linen. Room #2 was observed to have ceiling water stains and broken door casing frame, deficiency cited. Resident bedrooms were checked and closet/drawer space to accommodate each resident comfortably was available. The backyard is free of debris/hazards and the outdoor and passageways are free of obstruction. The outdoor activity area is free of visible hazards and debris and the trash cans have covered lids. There are no security bars or weapons on the premises. All medications for residents are kept locked and inaccessible to other residents. LPA reviewed medication and Medication Administration Record book and found no deficiencies. The first-aid kit is fully stocked w/First-aid Manual. LPA reviewed resident medications. Administration Certification for Hector Vargas expires: 6/23/2024 Liability Expires:7/15/2025. Smoke detectors and carbon monoxide detectors are operable and in compliance. The fire extinguishers were observed to be fully charged and in compliance. Emergency/ Fire Drill Expires: 09/10/2023 Per Title 22 Regulations, deficiency cited, see LIC809-D and TA was given for window/wall cleaning.
