Type of visit: Case Management - Other
Licensing Program Analyst (LPA) Evelin Rios conducted a case management visit to confirm the closure of this facility. LPA contacted the licensee and later met with Emmanuel A Manabat II at the facility. On 07/06/26, LPM Naira Margaryan received an email from the licensee regarding the facility closure. During today's visit, LPA conducted a physical plant tour and confirmed that there were no residents at the facility. LPA obtained the License. The licensee initiated the closure of the facility. LPA will submit the file for closure upon returning to the Woodland Hills South Regional Office. The effective closure date is 07/21/2026. LPA will email the closure survey. Jun 4, 2025Facility evaluation reportReport on file
Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Melissa Spaeth conducted an unannounced visit and was greeted a caregiver and the Administrator, Emmanuel Manabat. LPA stated the purpose of the visit is to conduct an annual inspection. The facility is licensed for six non-ambulatory of which two may be bedridden. The Administrator confirmed there are four residents. LPA and the Administrator toured the facility at 10:30 am until 11:00 am. Kitchen - LPA observed a two day supply of perishable food and a seven day supply of non perishable food items. The knives were safely locked in a kitchen cabinet. The cleaning supplies were locked under the kitchen sink. Common Areas –The living room contained comfortable seating. The dining room contained table and chairs. The family room also contained comfortable seating and a television. Residents’ Rooms – The rooms contained a bed, linens, night stand, lamp, chest of drawers and a closet. Bathroom – The two resident bathrooms contained grab bars, slip resistant mats, paper towels and a covered trash can. The staff bathroom was neat and clean and contained the required paper towels and trash can. Continued on 809-C Water Temperature - The water temperature was tested in resident bathroom one at 10:20 am and was 113 degrees F. Backyard –The backyard contained a covered patio with comfortable seating. The gate leading from the backyard to the front yard was not locked. Medication Closet - The medications were locked in the hallway closet. The first aid kit was also locked in the closet. Laundry Room – The room was locked and contained additional cleaning solutions, the washer and dryer, and laundry detergent Garage - The garage was locked and contained an additional refrigerator. LPA did not observe any safety issues when viewing the garage. Egress - There are three exits leaving the facility. LPA observed the egress doors were properly working. Smoke/Carbon Monoxide Detectors - The smoke/carbon monoxide detectors were tested at 10:30 am and were operable. Residents’ Records -LPA reviewed residents' records at 11:00 am until 11:30 am and reviewed resident's medications at 11:30 until 11:40 am. Staff Records - LPA reviewed staff records at 11:45 am until 12:15 pm. LPA observed a staff member has not completed the CPR/First Aid training. Based upon LPA's review of the staff records, the following deficiencies were issued. Allegation investigated: Resident sustained an unexplained injury while in care Staff are providing poor food service Staff are not meeting resident's diapering needs Staff are billing residents for services not provided Staff leave residents in bed for an extended period of time
On 7/12/2024 Licensing Program Analyst (LPA), Melissa Spaeth conducted a subsequent complaint investigation at the above facility to address the following allegation(s). LPA Spaeth was met by Caregiver. The Administrator arrived at 9:30 am. LPA explained the purpose of this visit was to conduct interviews and present findings. LPA Spaeth interviewed the three (3) of the four (4) caregivers via phone call at 8:00 am until 8:30 am. LPA Spaeth interviwed the Administrator at 12:00 pm until 12:30 pm. The investigation consisted of the following: On 10/27/2023 LPA Spaeth initiated a complaint investigation. LPA reviewed resident files and interviewed a resident. LPA toured the facility with the caregiver at 9:30 am until 9:45 am. At 9:30 am, LPA observed a seven-day supply of perishable food and a two-day supply of non-perishable food. LPA did not observe any health or safety issues. Continued - 9099C Unsubstantiated LPA Spaeth reviewed residents’ files 9:45 am until 10:15 am. LPA received copies of the residents' documents, the resident roster, the staff work schedule and a copy of the facility menu. Regarding the allegation: Resident sustained an unexplained injury while in care. It’s being alleged that a resident’s leg is bruised and bleeding and the Administrator does not properly document regarding what happened to the resident’s leg. Three (3) of the four (4) staff members (S1, S2, S3) stated there are no residents who currently have wounds and stated there has been no injuries. The Administrator stated a previous resident had a skin tear which was treated by a Kaiser wound nurse during the month of October, 2023. LPA received the facility sign in sheet and observed the nurse had provided wound care to the previous resident. The Administrator stated they had been advised to change the bandages each day and stated the wound was not an injury. Therefore the allegation is unsubstantiated. Regarding the allegation, Staff are providing poor food service. It’s being alleged the meals are lacking, are small, food is not nutritious, and there is not an adequate supply of food in the facility. During LPA's facility tour, LPA observed an adequate supply of food which included dairy, fresh fruits and vegetables, and frozen meat. LPA interviewed one resident (R1) out of the three residents (R2 - R3). R1 stated they are always provided enough food to eat and they like the food that is prepared. R2 and R3 were unable to answer LPA's questions. The menu was posted in the kitchen and LPA observed nutritious food is provided at each meal. S1 - S3 stated there is always an adequate supply of nutritious food. The Administrator stated they purchase food two times a week. Therefore the allegation is unsubstantiated. Regarding the allegation, Staff are not meeting resident’s diapering needs. It’s being alleged residents are left in bed for an extended period of time, and residents diapers are not regularly changed. R1 stated their diaper is changed every two hours and even during the night. R2 and R3 were unable to answer LPA's questions. S1 - S3 unanimously stated they change the resident's diapers every two hours. Therefore the allegation is unsubstantiated. Regarding the allegation, Staff are billing residents for services not provided. It’s being alleged that a resident left the facility due to the facility staff not providing care needed but still charged the resident for care Continued 9099-C and supervision. LPA spoke to a resident's family member at 12:00 noon who stated the facility has never charged for services that were not provided. The family member stated the staff provide the care needed and the facility has never overbilled the resident. The Administrator stated family members have not complained about the services provided and the facility has not overcharged for services rendered. Therefore the allegation is unsubstantiated. Regarding the allegation, staff leave residents in bed for an extended period of time. It’s being alleged the staff are forcing the residents to go to bed at 7pm and the staff leave the resident in bed until 9:00 am. S1 - S3 stated they never force residents to go to bed at 7:00 pm and never leave them in their bed until 9:00 am. R1 stated they make the decision when they go to bed. R2 and R3 were unable to answer LPA's questions. The Administrator denied the allegation. Therefore the allegation is unsubstantiated. Licensing Program Analyst (LPA) Melissa Spaeth conducted an unannounced visit and was greeted by the caregiver. The Administrator, Emmanuel Manabat arrived at 9:30 am. LPA stated the purpose of the visit is to conduct an annual inspection. The facility is licensed for six non-ambulatory of which two may be bedridden. LPA and the Administrator toured the facility at 9:35 am until 10:00 am Kitchen - LPA observed a two day supply of perishable food and a seven day supply of non perishable food items. The dining room contained table and chairs. The family room also contained comfortable seating and a television. The staff bathroom was neat and clean and contained the required paper towels and trash can. The water temperature was tested in resident bathroom one at 9:45 am and was 112 degrees F. Backyard –The backyard contained a covered patio with comfortable seating. The gate leading from the backyard to the front yard was not locked. Cont’d 809-C Medications - The medications were locked in the hallway closet. The first aid kit was also locked in the closet. Laundry Room – The room was locked and contained additional cleaning solutions, the washer and dryer, and laundry detergent Garage - The garage was locked and contained an additional refrigerator. LPA did not observe any safety issues when viewing the garage. Egress - There are three exits leaving the facility. LPA observed the egress doors were properly working. Smoke/Carbon Monoxide Detectors - The smoke/carbon monoxide detectors were tested at 9:55 am and were operable. Residents’ Records -LPA reviewed residents' records at 10:10 am until 10:30 am. Staff Records - LPA reviewed NUMBER staff records at 11:00 until 11:20 am. There are no deficiencies to report at this time. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA), Tihesha Smith conducted an unannounced Required 1-year inspection at this facility 10:03 am LPA was greeted by staff and disclosed the purpose of the visit. Staff contacted the administrator. LPA conducted a tour of the physical plant at approximately 10:20 am to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations. Common areas were observed for the ability to safely serve the needs residents. These included the living room, dining and family room combination, and kitchen. The common areas were checked for cleanliness and furniture was checked for functionality. Common areas observed to have adequate seating for residents. LPA reviewed the food service areas, food storage and supply (perishable and nonperishable foods). The kitchen food supply was observed and sufficient for the three (3) residents currently residing there. Two (2) days of perishable food observed. The freezer is stocked with meats and frozen vegetables. However, the meat was not properly stored in containers. Resident medications and first aid kit/supplies are stored in hall closet near laundry room. Medications observed to be locked and inaccessible to residents in care. Sharps are locked in kitchen drawer and observed to be in accessible to residents. Toxins are stored and locked in laundry room. Toxins observed to be inaccessible to residents. There is one (1) fire extinguisher attached to wall in kitchen and observed to be charged. Laundry room is located near garage. The appliances observed to be clean and functional. (Cont. from 809) The facility has a total six (6) bedrooms and (3) bathrooms. There is one (1) room for staff room. The resident bedrooms were properly furnished with at least one chair, nightstand, and sufficient lighting for each resident. The bedrooms had appropriate and adequate bedding and linens such as sheets, pillowcases, mattress pads, and blankets. LPA observed a supply of linens in hall cabinets. Bedroom #5 had one (1) bed with a brace attached to prevent falls. Each bathroom has the following items available: hand soap, paper towels, and trash cans. The hot water temperature was measured for the three (3) bathrooms to ensure it is within the required range for residents’ comfort and safety. The water temperature range was between 115.1, 115.9 and 117.3 -degrees Fahrenheit. Backyard has the following: Covered patio with tables and chairs. Patio furniture observed to be in good repair. Attached Garage: Used for PPEs, toxins, and storage. Shed: Locked shed used to store equipment Smoke detectors/carbon monoxide detector were tested and operable at time of visit. Facility grounds were free of hazards. At approximately 11:20 am LPA Smith discuss with Administrator over phone the availability of facility staff records. Administrator reveals he has staff records and currently updating them. At 1:50 pm: LPA reviewed three (3) resident files. Three (3) resident files reviewed included medical assessments and appraisals.
