Allegation investigated: Staff did not follow resident's hospice care plan appropriately Resident's records were not properly maintained Facility has pests
On December 18, 2025, Licensing Program Analyst (LPA) Brandon Lopez made a telephone call to Licensee (LI) Hanh Dao to deliver the complaint findings. However, the LI did not answer and LPA left a voicemail explaining the reason for his call. Additionally, LPA attempted to contact the LI multiple times during this investigation including on December 4, 2025, December 5, 2025, and December 18, 2025, due to the facility being closed effective October 13, 2025. Regarding the allegations that, staff did not follow resident's hospice care plan appropriately and resident's records were not properly maintained, the following has been concluded: It was alleged that Resident #1 (R1) hospice care plan was not followed appropriately and R1's records were not properly maintained. R1 was admitted to the facility on February 19, 2020 and moved out of the facility on January 25, 2022. The Department received the complaint on February 11, 2022. Due to the facility being closed effective October 13, 2025, the Department was unable to review or obtain any relevant documents to this complaint such as facility records, resident records, staff records, or any records for R1. Continued on LIC9099-C Unsubstantiated In an attempt to obtain relevant documents for this complaint, a collateral visit was conducted on December 4, 2025, to the current licensed facility at the location, Amore Senior Living II - 306006698. During the collateral visit on December 4, 2025, the Department conducted three resident interviews. However, the three residents interviewed were all admitted to the facility on August 2023, or later. Furthermore, the three residents interviewed were unfamiliar with R1 or any incidents that occurred prior to their admission. During the collateral visit, the Department also conducted two staff interviews. Both staff interviewed started employment with the facility on October 2023, or after. Both staff interviewed were unfamiliar with R1 or any incidents that occurred prior to the start of their employment. Regarding the allegation that, facility has pests, the following has been concluded: The three residents interviewed during this investigation denied ever seeing any pest since they have been admitted to the facility. However, the three residents were unable to provide any information about any issues prior to their admissions to the facility. The two staff interviewed during this investigation denied pest ever being present since they have worked at the facility. However, both staff were also unable to provide any information about issues prior to their employment with the facility. During the collateral visit conducted on December 4, 2025, to the current licensed facility at the location, Amore Senior Living II - 306006698, LPA conducted a tour of the interior and exterior portions of the facility. LPA inspected areas such as the resident bedrooms, resident bathrooms, the kitchen, the living room, the garage, and observed them to be free of any pest. Based on the limited information gathered during the investigation, the Department is unable to ascertain if the allegations occurred as reported. Although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove or refute the alleged violations occurred; therefore, all three allegations are deemed UNSUBSTANTIATED. The facility was closed effective October 13, 2025. Attempts to reach Licensee Hanh Dao to conduct an exit interview were unsuccessful. Allegation investigated: Staff did not seek medical attention for resident in care in a timely manner
Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to deliver the findings of the complaint investigation into the allegation listed above. LPA met with staff Fnu Lawrence and explained the reason for the visit. The investigation into the allegation, staff did not seek medical attention for resident in care in a timely manner, revealed the following. It was alleged that Resident 1 (R1) had a pressure injury, and no action was taken by the facility to have R1 treated by a wound care specialist. R1 moved into the facility on September 17, 2024. R1 had a stage 2 pressure injury at the time of move in. Witnesses interviewed verified this information. A review of R1’s records show R1 received wound care on September 17, 2024, November 27, 2024, and January 20, 2025. Witnesses interviewed could not verify if R1 received would care on any other days. R1 could not verify the days they received wound care. The Administrator reported that wound care was provided to R1 weekly, but the facility does not have records to show the days it was provided. The hospice company would not respond to requests for information. Substantiated The wound care progress notes for R! for November 27, 2024, show the pressure injury was noted to be stage 3. There is no documented evidence to show R1 received wound care after September 17, 2024, and before November 27, 2024. There is no documented evidence to show R1 received wound care for the time period between November 27, 2024, to January 20, 2025. Based on the evidence gathered, the preponderance of evidence standard has been met, therefore the following allegation, staff did not seek medical attention for resident in care in a timely manner, is deemed SUBSTANTIATED. Deficiencies are being cited per Title 22, Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to deliver the findings of the complaint investigation into the allegations listed above. LPA met with Administrator Chet Khay and explained the reason for the visit. The investigation into the allegation, staff are not able to communicate with residents in an effective manner due to language barrier, revealed the following. LPA interviewed the Administrator and 4 staff members who reported no issues communicating with staff. LPA interviewed 4 out of 5 residents, who reported that they had no issues communicating with any of the staff. One resident was asleep and not interviewed. Two former staff members contact information is no longer valid and could not be reached for interview. The Administrator reported that all staff members and the 2 former staff members are bilingual and had no issues communicating with anyone. None of the evidence gathered supports the allegation, therefore the allegation is deemed unsubstantiated, although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. Unsubstantiated The investigation into the allegation, Licensee is not ensuring that facility is kept in a sanitary condition, revealed the following. It was alleged that the facility dog who lives at the facility tracked feces into the entry way of the facility and there was a urine soaked pad on the floor in the living room/main entry of the facility. LPA interviewed 4 out of 5 residents who could not recall any such incident. LPA interviewed 4 staff members and the Administrator who reported such an incident never happened. The Administrator and staff reported that the facility dog goes to the bathroom outside and has not tracked anything inside. No evidence was provided to support the allegation, therefore the allegation is deemed unsubstantiated, although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA explained the reason for the visit. Facility is licensed for a capacity of 6, of which 5 can be non-ambulatory and 1 bedridden in bedroom 1 only and a hospice waiver for 3. Facility is operating within it's license. Facility has 6 resident rooms, 1 staff room, 3 bathrooms, living room with a screened fireplace, dining room, kitchen and a 2 car garage. Facility has a new phone number, 949-503-2404. LPA and staff toured the facility. 4 residents and 2 staff were present during the visit. LPA observed a 2 day perishable and a 7 day non-perishable food supply on hand in the kitchen. Knives are kept locked in a kitchen drawer. LPA observed medications are kept locked in a kitchen cabinet. LPA observed there is no See Something Say Something poster (PUB 475) posted in the facility. The Administrator observed all the occupied resident rooms (4) had the required furnishings and bed linens. Smoke detectors/carbon monoxide detectors tested operational. The garage is kept locked and used for storage. LPA and staff toured the backyard. No bodies of water observed. There is shaded seating for the residents to sit outside. Both exit gates are operational. During the visit the Administrators Ruth and Chet Khay arrived. Ruth Khay's Administrator's Certificate expires on 1/19/2026. LPA reviewed 2 staff files. Both staff members are background cleared and associated to the facility. Both staff members have the required training including CPR/First Aid training. LPA reviewed 4 resident files and medications. No discrepancies observed. LPA consulted with the Administrators regarding reporting requirements. Allegation investigated: Licensee abandoned facility
On this Day, Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced visit to deliver complaint findings. LPA was greeted and granted entry into the facility by Caregiver Laurence Laurence and explained the reason for the visit. Administrator Chet Khay was available by phone. The Department received a complaint on 01/03/2024 and LPA Mendivil conducted the initial visit on 01/11/2024. LPA Mendivil interviewed staff and current Licensee. LPA Mendivil obtained copies of rental agreements, management agreements, and liability insurance. Regarding the allegation Licensee abandoned the facility, the investigation revealed the following: It was alleged that the current Licensee Hanh Dao has abandoned the facility. Based on interviews with Hanh Dao, the facility is currently operated by Administrators Ruth Khay and Chet Khay as they are in the process of applying for their own license. Unfounded Per review Licensee Hanh Dao is still currently licensee until Chet Khay and Ruth Khay have their application approved. Current Licensee Hanh stated she is in contact with the perspective Licensee monthly and is updated on issues with the facility. Administrator Chet Khay stated he is in the process of submitting an application to the Department for Change of Ownership as of 2/23/2024. Therefore, based on a preponderance of evidence through interviews and records reviewed the allegation that licensee abandoned facility is determined to be UNFOUNDED, meaning that the allegations were false, could not have happened and/or is without a reasonable basis. This agency has investigated this complaint. No deficiencies cited. Allegation investigated: Facility staff do not answer the telephone
Licensing Program Analyst (LPA) Celine De Perio conducted an unannounced 10-day visit to the facility for the complaint received on 11/29/23 and to deliver the findings. LPA De Perio explained the purpose of today's visit, was greeted, and granted entry by staff on duty (S1), who contacted facility administrator (AD) Chet Khay. During the investigation, LPA De Perio toured the physical plant of the facility, conducted interviews, and requested copies of pertinent records reviewed. It was alleged that facility staff do not answer the telephone. During the physical plant of the facility, LPA observed that the facility phone is operational by observing the phone ringing, and staff answering it. A total of 6 interviews were conducted with staff and residents who did not corroborate with the allegation by stating that the facility staff does answer the phone, and that the phone is working. An interview was conducted with the reporting party (RP) and it was revealed that RP had the wrong phone number to the facility. Unfounded Based on LPA’s interviews which were conducted, review of documents obtained, and observations, this allegation was UNFOUNDED, meaning that the allegation was false, could not have happened and/or is without a reasonable basis.
