Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Nadia Shahbazian conducted an unannounced Annual Required visit and inspection of the facility. LPA met with Administrator - Christine Lekhlyan. LPA asked for copies of staff and resident rosters and files. The purpose of the visit was explained. The Residential Care Facility for the Elderly (RCFE) is licensed to serve 6 (six) non-ambulatory residents with hospice waiver for three (3) residents. Current census is three (3) and all residents receive services from the Frank D. Lanterman Regional Center. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools. At 11:45am LPA toured the facility with the Administrator and observed the following: Required postings were observed in the dining room and office area. The facility is located in a residential area and consists of 7 bedrooms, 2.5 bathrooms, living room and dining room. The physical plant appeared clean, sanitary and comfortable with no visible immediate hazards. There are several exits doors throughout, including the kitchen, living room and bedroom#6, but facility uses the living room exit as the main exit. There are two fully charged fire extinguishers, one in the kitchen, one in the dining room, both purchased on 02/05/2025. All exits were equipped with functional alarms. Facility conducts monthly fire and safety drills, the last fire drill was conducted on 12/01/2025 and the earthquake drill was conducted on 12/16/2025. Smoke alarms and carbon monoxide detectors are interconnected and were tested at 11:50am and observed to function properly. Facility is equipped with cable, internet and land line telephones. Continued on LIC 809C Common Areas: Include a living room, dining room and locked hallway closet for storing chemicals and linens/supplies. The office area is located near the living room. Laundry washer and dryer are located in the office area but the laundry chemicals are locked in the hallway closet. Dining room is furnished with a table, eight (8) chairs and three (3) small refrigerators. LPA observed a properly screened, non-functional fireplace in the dining room. The living room had a table and ample sitting for all the residents and staff and has an exit door leading outside. Television set and activity/gaming areas were observed in the living room. Kitchen: There is a refrigerator, stove, microwave and toaster oven in the kitchen. Knives, cutlery and other sharp kitchen utensils are stored and locked in the kitchen cabinet. Food storage and preparation areas are clean and sanitary. LPA found a sufficient amount of perishable (2 days) and non-perishable (7 days) food supplies, properly stored in the kitchen and also in office area cabinet. Cleaning detergents were observed to be locked in the hallway cabinet. Bedrooms: There were six (6) bedrooms designated for clients' use and one (1) bedroom for live-in staff but there is always awake staff at all times. All bedrooms in use by residents were properly furnished with appropriate beds, chairs, closets and linens with sufficient lighting. Currently bedrooms #1, #4 and #5 are vacant. Bathrooms: There are two (2) full bathrooms and one (1) half bathroom designated for staff and residents. All bathrooms were properly equipped with required grab bars and non-skid mats and functional fixtures. Hot water temperature was tested between 107.2 and 109.6 degrees Fahrenheit. Surrounding grounds: Entry/exit gates and pathways were free of obstruction. The outdoor area was free of visible immediate hazards. No bodies of water were observed at the facility. There was a covered patio area with furniture appropriate for outdoor use sufficient for number of clients. Facility has a detached garage with it's own separate exit. The garage was converted to a permitted Accessory Dwelling Unit (ADU), which is currently rented. Administrator will submit copies of the permits and a new facility sketch to LPA; a Technical Assistance advisory was provided. Continued on 809-C Resident Files: A review of resident records to ensure compliance of licensing forms was conducted at 11:05am. All required admission reports and current physician reports were observed in the files. Medication: First aid kit, first aid manual and medications are kept locked in a kitchen cabinet. Medications records for all three (3) residents were also verified for accuracy of administration based on physician orders. Medications for all three (3) residents were counted and compared to medication rosters. Staff Files: Staff files for four (4) staff were reviewed to ensure all forms and training certificates are up to date. Pursuant to Title 22 Division 6 of the CA Code of Regulations, no deficiencies observed during the visit. A Technical Assistance advisory was provided to submit ADU permits and a new facility sketch. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Nadia Shahbazian conducted an unannounced Annual Required visit and inspection of the facility. Upon arrival at 10:15 am, LPA met with Administrator/Christine Lekhlyan. LPA asked for copies of staff and resident rosters and files. The purpose of the visit was explained. The facility is licensed to serve 6 (six) non-ambulatory residents with hospice waiver for three (3) residents. Current census is five (5) and all five (5) residents receive services from the Frank D. Lanterman Regional Center. At 10:45 am, with the assistance of Administrator, Christine Lekhlyan, LPA toured the physical plant area inside and outside to ensure there are no health and safety hazards. The walls, floors and general area was in good repair with good lighting. Mandated postings were observed in the office area and were readily available for review. Smoke alarms and carbon monoxide detectors were tested to be functioning properly. There were two (2) fire extinguishers, both purchased on 01/16/2025, one in the kitchen and one in the dining area. Facility conducts monthly fire drills and LPA was provided a log of all the drills. The last fire drill was conducted on 01/12/2025. There are four (4) exits, the main entry door, one in bedroom five (5), one in the kitchen and the main fire exit is in the living room. All exits were equipped with functional alarms. The following was observed: Structure: The facility is a single story house located in a residential area. The house has seven (7) bedrooms, one (1) staff room and six (6) private resident bedrooms, two (2) full bathrooms for residents and a half bathroom for staff. There is a kitchen, living room, dining room. Laundry machines are located in the office area with chemicals and detergents locked in a hallway closet. Cont. in 809-C Common Areas: These included separate living room and dining area. The common areas were properly furnished. LPA observed ample chairs/sitting for six (6) residents and staff. There is an non-functional screened fire place in the dining room. LPA observed a gaming area, with a small refrigerator and a freezer in the dining room. Living room has sofas, table and a television set. Surrounding Grounds: Entry/exits passageways were free of obstruction. There is a side yard with covered patio, with appropriate furniture for outdoor use with ample seating for six (6) residents. The outdoor area was free of hazards; no body of water observed. The main exit door was observed to be unlocked from inside. Kitchen: The kitchen is equipped with a refrigerator, stove/oven, dishwasher, microwave. All appliances and fixtures were observed to be clean and in functional condition. LPA found sufficient amount of perishable and non-perishable food, sufficient for all six (6) residents for at least 10 days. Knives and sharp objects were stored in a locked drawer in the kitchen. Properly labeled medications were locked the kitchen cabinets. There were two (2) First Aid Kits with First Aid Manual, one locked in kitchen cabinet and the other in hallway closet. The kitchen cabinets were observed to have ample dishes and cook ware. Bathrooms: There are two (2) bathrooms designated for residents' use and a half bath for staff. Both resident bathrooms were sanitary and had the required grab bars and non-skid mats and functional fixtures. Hot water temperature was measured at 106.8 degrees Fahrenheit. Bedrooms: There were six (6) private bedrooms designated for residents' use and one (1) for night staff. All bedrooms in use by residents were properly furnished with appropriate beddings and linens with sufficient lighting. Each room was equipped with chairs, cabinets and extra bedding. Cont in 809-C The medication, chemicals and first aid are locked and secured in the kitchen cabinet. There is also a first aid manual and two (2) First Aid Kits with scissors, tweezers and supplies, one in the kitchen and the other in the hallway closet. Resident Files: LPA conducted a file review of resident records to insure compliance of licensing forms. Staff Files: LPA also conducted a file review of staff records to insure forms and training certificates are up to date and compliance with licensing forms. Medications: Medication and Medication Records were reviewed for proper documentation. LPA counted the medications and ensured proper dosage were given to residents. Pursuant to Title 22 Division 6 of the CA Code of Regulations, no deficiencies observed during the visit. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Antonia Alvizar conducted an Annual Required visit and inspection of the facility. Upon arrival, LPA met staff Susanna Hayrapetyan who assisted with the visit. Shortly after Administrator Christine Lekhlyan arrived. The purpose of the visit was explained. The facility is licensed to serve 6 (six) non-ambulatory residents. Hospice Waiver for (3) three residents. Residents receive services from the Frank D. Lanterman Regional Center. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools. At 9:25 am, with the assistance of Administrator, Lekhlyan and LPA toured the physical plant area inside and outside to ensure there are no health and safety hazards. Required postings and readily available for review were observed in the office area. Smoke alarms and carbon monoxide detectors are interconnected also tested and function properly. There was a fire extinguisher observed to be fully charged in the dining area. The last fire drill was conducted on 12/06/2023. The following was observed: Structure: The facility is a single story house with six (6) bedrooms, one (1) Staff room, two (2) full bathrooms, one (1) half bathroom, kitchen, living room, dining room, laundry machines in office space area, side yard for shade with table and chairs. There's no bodies of water on the premises. All outdoor and indoor passageways are free of obstruction. Kitchen: The kitchen appliances and fixtures were functional. Knives, cutlery and other sharp kitchen utensils are stored and locked in the kitchen cabinet. Bedrooms Residents: There were six (6) bedrooms designated for clients' use. All bedrooms in use by residents were properly furnished with appropriate beddings and linens with sufficient lighting. Hygiene for each resident observed, ample supply of clean linen and storage space. One (1) bedroom is in use by staff. Bathrooms: There are two (2) full bathrooms and one (1) half bathroom designated for staff and residents. All bathrooms were properly supplied with required grab bars and non-skid mats and functional fixtures. Hot water temperature was tested between 110 to 115 degrees Fahrenheit. Linens & Hygiene Supplies: Beds have the required linen/supplies which include, pillowcase, mattress pads, fitted sheet, blanket and bedspreads. Adequate supply of linen stored in resident bedrooms and hallway closet. Food Service: LPA found a sufficient amount of 2 days perishable and 7 days non-perishable food at the facility properly stored. There is one (1) full size refrigerator in the kitchen, two (2) small refrigerators, one (1) freezer in the dinning area and additional food storage in the office area. Dishes, cups and flat ware are stored in the kitchen cabinet, inspected and in good repair. Toxins: All the cleaning solutions are locked and stored in the hallway closet. Medication and First-Aid Kit: Properly labeled medications and centrally stored and locked in a kitchen cabinet and is only available for staff to use but inaccessible to clients. LPA reviewed the first-aid kit, it had all required supplies. Common Areas: These included the living room, and dining area. All furnishings are in good repair, lighting is good, walls, ceiling and floors are also in good repair. Surrounding Grounds: Entry/exits were free of obstruction. The outdoor area was free of hazards. No bodies of water were observed at the facility. There was a shaded patio area with functioning furnisher. Resident Files: LPA conducted a file review of resident records to insure compliance of licensing forms. Staff Files: LPA also conducted a file review of staff records to insure forms and training are up to date and compliance with licensing forms. Medications: Medication and Medication Records were review for proper documentation. They are locked and stored in a kitchen cabinet. Staff and some residents were also interviewed using the CARE Tools questionnaire. Pursuant to Title 22 Division 6 of the CA Code of Regulations, no deficiencies observed during the visit.
