On 08/04/26, Licensing Program Analyst (LPA) M. Yang conducted an unannounced case management visit regarding incident that had occurred. LPA met with staff Rosanne Salazar. Administrator Itaska Mask was called and unable to attend meeting. The purpose of today’s visit is to follow up on incident that was reported by the Central Valley Regional Center where R1’s medication administration record (MAR) was observed not signed for dates 07/02/26 to 07/09/26. During the visit, R1’s medications were checked, MARs and Centrally Stored Medication List were reviewed, and records were reviewed. No deficiency issued during visit. Exit Interview conducted. On 11/24/25 Licensing Program Analyst arrived at the facility to complete an unannounced annual visit. LPA met with Direct Support Professional, Kye-Shanna Manns, explained reason for visit and was permitted entry into the facility. Administrator, Itaska Mask arrived a short time later. LPA completed a tour of the facility inside and out. A health and safety check was completed on residents in care. 1 resident was present during time of visit. Pathways and doors were clear and free from obstruction. Facility was without odor. Common areas were adequately furnished, and adequately lit. Smoke detectors and carbon monoxide detectors were present and operational at time of visit. Fire extinguisher last serviced on 2/25/25. Last fire drill on conducted on 10/16/25. Resident rooms observed to have the required furnishings and with adequate lighting. Medications were located in locked hallway cupboard. LPA observed sufficient seating under covered patio areas. The following issues were observed during today’s visit: 1 of 1 resident file reviewed did not have the required documentation. Facility did not implement measures to protect residents in care from items posing a danger. Deficiencies cited per California Code of Regulations, Title 22. Deficiencies are being cited on the attached 809D. LPA requested the following documents to be submitted to CCL by 12/5/25: current copy of Administrator’s Certificate, Administrator Organization (LIC 309), Designation of Administrative Responsibility (LIC 308), Emergency Disaster Plan (LIC 610-E), Affidavit regarding Resident Cash Resources (LIC 400), Personnel Report (LIC 500), Register of Facility Clients/Residents (LIC 9020) in order to update the facility file. Exit interview was conducted with Administrator, Itaska. A plan of correction was developed by Administrator and reviewed by LPA. On 11/21/24 Licensing Program Analyst (LPA) M. Garza arrived unannounced for an annual inspection visit. LPA was met by Direct Support Professional, Vanndy Nhey, explained reason for visit and was permitted entry into the facility. Administrator, Itaska Mask was contacted and arrived a short time later. LPA completed tour of the facility inside and out. Residents not present at time of visit. Pathways and doors were clear and free from obstruction. Facility was clean and without odor. Common areas were clean, adequately furnished, and adequately lit. Smoke detectors and carbon monoxide detectors were present and operational at time of visit. Fire extinguisher last serviced 03/19/2024. Last fire drill on 11/12/2024. Water temperature measured 112.4 degrees F in resident bathroom #2. Resident rooms observed to have the required furnishings and with adequate lighting. Linen supplies are kept in linen closets. LPA observed sufficient seating under covered patio areas. The following issues were observed during time of visit: Staff restroom missing hand washing sign. Chemicals observed in staff restroom unlocked and accessible. Scissors observed on counter top of medical station. 4 of 4 residents bedrooms observed without box springs. Kitchen drawer with sharps observed unlocked and accessible. Chemical/items posing a hazard to residents in care observed in hallway closet unlocked and accessible. Bedroom #3 exit door broken and in need of repair to bottom of door. Touch up paint needed around thermostat. Right side gate latch in need of repair/replacement (gate does not latch). Concrete lifted at back corner of house (potential tripping hazard). Fence boards on right and left side of facility in need of fixing/replacement. Debris on left side walkway in need of removal. CONT... CONT... On 11/17/23, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and requested to meet with Administrator. LPA met with staff Monica Leon and staff Erika Pierce. Administrator Itaska Mask was called and arrived during inspection. Two clients were present upon inspection. During inspection 1 client left to day program and 1 client left on outing. Area Director Troy Rice arrived later during inspection. LPA toured facility with Administrator. The facility was observed to be at a comfortable temperature at 72 degrees F, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. Fire extinguisher was observed with a service date of: 09/13/23. Fire drill last completed on 10/26/23. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at -1.6 degrees F and refrigerator temperature was maintained at 33 degrees F. Medications were checked and observed kept locked in hall shelf. MARS was reviewed. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured. All bathrooms were observed operating and functioning during inspection. Non-skid mat and grabbed bars were observed. Hot water temperature was tested 105.3 degrees F. in bathroom 1 and 112.1 degrees F. in bathroom 2. Carbon monoxide and smoke detectors were tested and observed to be operational. Outside of facility toured. Side gate was self-closing and self-latching. Adequate outdoor seating available for residents and free of debris. All residents’ file reviewed to have required documents. LPA reviewed two staff files to have all required documents, fingerprinted cleared and associated to the facility. No deficiency was cited during inspection. Exit Interview conducted. LPA received copy of current Administrator certificate, Lic 308, Lic 400, Lic 402, Lic 500, Lic 610E, Lic 9020, current liability insurance, and current Administrator Certificate.
