On 03/16/2026, Licensing Program Analyst (LPA) M Vega arrived unannounced at the above facility to conduct an Annual Inspection. LPA introduce self to Staff 1 (S1), stated the purpose of the visit, and was granted entry to the facility, Administrator was called over the phone and arrived few minutes later. LPA toured facility with Staff 1. The facility was observed to be at a comfortable temperature, of 70 degrees F. Facility is free of debris, in good repair, and no passageway obstructions or fire hazards were observed. Common areas were properly furnished and well-lit throughout. LPA observed some residents in common area after watching television, others in their rooms resting. Department phone number and infection prevention information signs were posted thought the facility. At the time of inspection there are currently 8 residents at the facility. LPA observed rooms for residents were properly furnished, Clean and in good repair. All residents’ bedrooms were observed to be with comfortable temperature. Bathroom water temperature was measured at 113.5 degrees Fahrenheit. Inspecting kitchen LPA observed the required 7 day supply of non-perishable food and 2 day supply of fresh perishables to be properly stored. An emergency disaster supply was observed. Medications observed to be locked in kitchen area. LPA reviewed medication records to be accurate at the time of inspection. Allegation investigated: Staff member physically abused resident in care. Staff are chemically restraining resident(s) in care.

On 02/11/2025 at 1:00 PM, Licensing Program Analyst (LPA) met with Administrator - David Murchison to deliver the findings for the above allegations. The department received a complaint on 08/30/2024 alleging that, Staff member physically abused resident in care. And Staff are chemically restraining resident(s) in care. During the investigation, LPA interviewed Responsible party of Resident 1 (R1). Conducted interviews with R2 and R3. During the course of this investigation facility files were reviewed. It was determined that facility does provide activities to residents, food quality is not poor and there is recorded adequate food supply at the facility. Continuation on LIC 9099C Unfounded Based on LPAs observations and interviews which were conducted Staff 1 of 3 and record review(s), Interviewed staff, conclusion was Staff was able to demonstrate correct actions when submitting forms to CCLD. Postings Posted on wall to the right of the entry way. Therefore allegations, that Staff did not report incident involving resident in care as necessary and Licensee is not prominently posting required information in areas accessible to residents, representatives, and the public., is unsubstantiated. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated. An exit interview was conducted where a copy of this report was provided. This agency has investigated the complaint alleging: Staff member physically abused resident in care. And Staff are chemically restraining resident(s) in care and We have found that the complaint was unfounded, meaning that the allegation was false, could not have happened and/or is without a reasonable basis therefore we have dismissed the complaint. There were no citations issued during this visit and exit interview was conducted. LPA toured facility with Administrator David and Colin Murchison. The facility was observed to be at a comfortable temperature, of 70 degrees F. Facility is free of debris, in good repair, and no passageway obstructions or fire hazards were observed. Common areas were properly furnished and well-lit throughout. LPA observed some residents in common area after watching television, others in their rooms resting. Department phone number and infection prevention information signs were posted thought the facility. At the time of inspection there are currently 8 residents at the facility. LPA observed rooms for residents were properly furnished, Clean and in good repair. All residents’ bedrooms were observed to be with comfortable temperature. Bathroom water temperature was measured at 108.6 degrees Fahrenheit. Inspecting kitchen LPA observed the required 7 day supply of non-perishable food and 2 day supply of fresh perishables to be properly stored. An emergency disaster supply was observed. Medications observed to be locked in kitchen area. LPA reviewed medication records to be accurate at the time of inspection. Cleaning supplies were observed to be in a locked cabinet in the laundry room. Fire extinguisher was observed with a service date of 03/2024. Continuation on LIC 809C LPA inspected outside the facility. There was shaded seating for residents as well and in good repair. No obstructions to fire exits. LPA reviewed Staff and Resident files. Resident files observed to have updated information. No deficiencies were observed and cited. Exit interview conducted. Type of visit: Prelicensing

On 03/14/2024 at 11:00 AM, Licensing Program Analyst (LPA) V Gorban conducted an unannounced Pre-licensing visit. LPA identified himself and discussed the purpose of the visit. LPA conducted the inspection with the facility Administrator (AD) David Murchison. An inside and outside tour of the facility was conducted with AD. Resident’s each room have adequate furnishings and lighting and all the required furnishings. Mattress and linen appeared to be in good condition. Home is fire cleared for 10 (ten) bed ridden residents. LPA observed a supply of extra bed linens. Bathrooms were properly equipped, and trash cans had a fitting lid. Hot water temperature was observed to be 116 degrees F. Kitchen observed to have dishes, plates, utensils. Sharps/knives and medications are locked in the kitchen. Cleaning supplies are stored. First aid kit contains all the required items. Fire extinguisher is present and was serviced on 03/14/2024. Smoke detectors and carbon monoxide are combined in one unit and were operating properly. Outside of the facility toured. Exits open free of obstruction. Gate is self-latching. No outside hazards were observed. All required postings are posted. Facility phone number will be 559- 314-8849. Component III conducted during pre-licensing inspection. LPA have found that applicant has met all pre licensing requirements. LPA will submit documentation to CAB in Sacramento for final review prior to license being issued. Pre-Licensing is complete, and this facility has no deficiencies.
