Licensing Program Analyst (LPA) Christina Valerio arrived to the facility to meet with Licensee. On 07/15/2026, the Regional Office (RO) received a letter stating the licensee is voluntarily surrendering the license and their anticipated closure date would be effective 07/13/2026. According to the licensee, The facility did not have residents in care. The facility submitted their original copy of the license to the RO. LPA Valerio observed the facility and observed zero (0) residents in care. On 10/02/25, Licensing Program Analyst (LPA) Kimberly Viarella made an unannounced visit to this facility to conduct the annual inspection. No one was home upon arrival so LPA called the Administrator, Maria Lourdes Bachiller who arrived shortly after. LPA stated the purpose of the visit and a brief interview followed. There were no residents in care at the time of this inspection. LPA toured the facility with Bachiller and noted that all 4 resident rooms had the required furniture, furnishing and lighting to be in compliance at the time of this inspection. There was a locked cabinet for future resident medications, as well as locks for the sharps in the kitchen to be kept inaccessible to future residents in care. LPA observed Bachiller's Administrator certificate posted: # 6072671740 and that it expires on 08/15/2026. LPA also observed the Ombudsman information, See Something, Say Something, posters along with Resident Rights, Theft and Loss policy, Emergency Disaster Plan, and Resident Council information were posted in the front room and accessible to future residents and visitors. LPA viewed the grounds and noted that the property was well-maintained and in compliance at the time of this LPA provided consultative services to the Administrator regarding grab bars in one shower and future communications with Community Care Licensing. LPA provided the general regional office email: CCLASCPSacramentoRO@dss.ca.gov. LPA also reminded the Administrator that they were required to notify Community Care Licensing when they accepted their first resident. Failure to do so will result in a citation. Licensing Program Analyst (LPAs) Vincent Moleski and Holly Williams arrived announced to conduct a pre-licensing inspection on 10/08/24. LPAs Moleski and Williams met with Licensees Michael Bachiller and Maria Bachiller and explained the purpose of the visit. LPAs Moleski and Williams conducted a presentation of Component III for Residential Care Facilities for the Elderly (RCFEs). LPAs Moleski and Williams reviewed pre-made resident files, and other application documents. LPAs Moleski and Williams toured and inspected the physical plant inside and outside to ensure there are no safety hazards to residents in care. LPAs Moleski and Williams observed rooms to have required furniture.The temperature inside the building was observed to be at 79 degrees Fahrenheit which is within the required range of 68-85 Fahrenheit. The hot water temperature was measured at 120 degrees Fahrenheit which is within the required range of 105-120*F. LPAs Moleski and Williams observed the centrally stored medications area to be locked and inaccessible to residents. LPAs Moleski and Williams observed the fire extinguisher(s) and first aid kits were up to date. LPAs Moleski and Williams observed smoke and carbon monoxide detector(s) were in good repair. Based on a review of this property during this Pre-licensing visit, it was determined that the applicant was found to be in compliance at this time. Applicant has completed Component III. LPAs Moleski and Williams will notify the Central Application Bureau (CAB) that the pre-licensing has been completed and passed. Administrator/Licensee participated in COMP II via telephone call with CAB analyst. Identification of the Administrator/Licensee was verified by confirming driver’s license number. During COMP II, Administrator/Licensee confirmed the understanding of Title 22. Component II was successfully completed. Administrator/Licensee was advised to email signed LIC 809 with copy of photo ID to CAB. During COMP II, CAB analyst confirmed Administrator/Licensee's understanding of following areas: 1.
