Allegation investigated: Clients hygiene care needs are not being met Personal Rights Violation

Licensing Program Analysts (LPAs) Alviso and Stevenson delivered findings of the complaint investigation, on 12/5/24 at approximately 9:20am, and met with Administrator Emma Mutunga. LPA reviewed client C1 records, facility records, and other information obtained in the investigation. LPA toured the facility. The facility was observed to be clean and orderly. The food supply was observed to be sufficient. The reporting party (RP) alleges that client’s hygiene care needs are not being met, and client’s personal rights are being violated. C1 was admitted 8/15/24 to the facility; C1 does attend a day program, as of 8/19/24. Per review of records, information obtained, and interviews with staff, and other related parties, the investigation revealed the following: C1 has had some history of hygiene needs and concerns, and C1 does need assistance with hygiene care, including showering, grooming, and dental hygiene. Allegation investigated: Neglect/lack of care & supervision leading to hospitalization of the resident

Licensing Program Analyst (LPA) Alviso delivered findings of the Departments complaint investigation, on 2/13/24 at approximately 9:00am, and met with Administrator Emma Mutunga. The reporting party (RP) alleges that staff (S1) neglect/lack of care & supervision led to hospitalization of the resident. R1 was transported to the hospital for increased generalized weakness. R1 was medically assessed and found to have a cloth in their severely contracted right hand that may have been there for several days. There was concern of an infection, and a foul odor from R1’s fingers on their right hand. R1 had the tip of two right hand fingers partially amputated. The Department reviewed records and conducted interviews regarding the allegation of “neglect/lack of care & supervision, leading to hospitalization of the resident. The investigation revealed R1 has a diagnosis of severely contracted hands. R1 has a history of reoccurring urinary tract infections. Licensing Program Analyst (LPA) Alviso delivered findings of the Departments complaint investigation, on 2/13/24 at approximately 9:00am, and met with Administrator Emma Mutunga. The reporting party (RP) alleges that staff (S1) caused injury to resident’s (R1) elbow. R1 was observed to have bruising and swelling to elbow. RP believes the injury may have occurred when staff assisted R1 in routine arm exercises. RP alleged C1s elbow may be fractured. The Department reviewed records and conducted interviews regarding the allegation of “staff caused injury to resident.” The investigation revealed R1 has range of motion limitations and a complex medical history. Interview conducted on 12/27/2023 with placement agency service coordinator provided no information revealing concern of R1s care residing at current placement. A medical professional was interviewed with knowledge of R1’s history and injury observed by RP.
