Allegation investigated: Physical Abuse

LIcensing Program Analyst (LPA) Nakagawa arrived unannounced to deliver findings regarding the above complaint and met with Katherine Santos, Caregiver and Luningning Sikat, Licensee via phone. The c omplaint alleges that staff, S1 grabbed a resident by their arms during an argument resulting in the resident being bruised. During investigation, LPAs conducted interviews and made observations. Interview with another staff, S2 revealed that staff S1 also had a physical altercation with S2 the same day as their argument with R1. When Licensee came to facility the following day to address the staff altercation, R1 reported to the Licensee that S1 had grabbed their arms. S1 was terminated and escorted outside. The police were called and responded to the facility where they conducted interviews and arrested S1 for elder abuse. On 11/30/2023, Licensing Program Analyst (LPA) Tobola arrived unannounced for the purpose of delivering complaint investigation findings and was greeted by Lead Staff, Leonida Arinzana. LPA toured the facility, interviewed staff and residents, reviewed staff and resident records and made observations during the course of the investigation. Complaint alleges, staff do not ensure resident records are properly maintained. Based on a review of resident records, LPA found all resident records to be in order and current. However, upon a review of staff records, LPA found that staff (S1) & (S2) do not have current Health Screening Reports on file. Based on an interview with Administrator, it was indicated that staff (S1 & S2) have attempted several times to update their documents but have been unsuccessful. Complaint alleges, Licensee does not ensure staff are in good physical, mental health to perform assigned tasks safely for residents in care. Allegation investigated: Facility sustained a resident with prohibited condition

On 10/4/2023, Licensing Program Analyst (LPA) Tobola arrived unannounced for the purpose of delivering complaint investigation findings and was greeted by House Manager, Arinzana Reyes. LPA toured the facility, reviewed resident records, interviewed staff and outside parties and made observations. Complaint alleges facility sustained a resident with prohibited condition. Based on interviews with Licensee and staff (S1), it was stated that resident (R1) had sustained prohibited pressure injuries prior to the facility change of ownership on February 12, 2023. LPA conducted an interview with outside party (I1) also indicating that R1 sustained wounds prior to the new licensee taking control of the facility. Although both parties’ statements are consistent, there is no documentation indicating when R1’s wounds developed. The Licensee failed to properly seek appropriate emergency medical attention for R1 after observing wounds on R1’s heels on February 12, 2023.
