Allegation investigated: Facility staff did not provide adequate supervision resulting in resident eloping from facility.
Licensing Program Analysts (LPAs) Nakagawa and Contreras arrived uannanounced at 2:00 PM on 06/19/2025 to open a complaint investigation and deliver findings regarding the above complaint allegation and met with Eugenie Broussard, Facility Manager. Facility Administrator was not on site. There were 3 additional care staff and 20 residents on site at the time of visit. Complaint alleges that Facility staff did not provide adequate supervision resulting in resident eloping from facility. Per statement filed by reporting party, resident (R1) who has a diagnosis of Dementia was found wandering on the sidewalk outside without assistance or supervision at approximately 1:30 PM by the reporting party and assistance given and facility staff notified. ****This is an amended report*** On April17, 2025, Licensing Program Analyst (LPA) Nakagawa arrived unannounced for the purpose of delivering complaint investigation findings regarding the above allegations and was greeted by Licensee Sukhjit Sandhu.. LPA Nakagawa toured the facility, conducted interviews, reviewed records, and made observations during the course of the investigation. The complaint alleges that resident (R1) was illegally evicted. The complainant states that the Licensee called the family of R1 stating that R1 had not paid and needed to move out in three days. LPA interviewed the Licensee who stated that they had contacted the family of R1 for payment but had not threatened to evict. Allegation investigated: Lack of supervision resulting in elopement
Licensing Program Analyst (LPA) Nakagawa arrived at American Assisted Living facility on 01/27/2025 to conclude an investigation and deliver findings regarding the above allegation. LPA met with the Licensee/Administrator Sukhjit Sandhu. During the course of the investigation, LPA toured the facility, made observations, conducted interviews with staff and outside parties, reviewed records includig Suisun City Police Reports. It is alleged that due to a Lack of Supervision Resulted in Elopement of a resident. The Department learned that resident (R1) eloped two times, 8/9/24 and 1/11/25. Allegation investigated: Staff allowed resident in care to leave the facility unassisted
****THIS IS AN AMENDED REPORT**** (Amended to include Civil Penalties) Licensing Program Analyst (LPA) Jill Nakagawa arrived uannanounced at 10:38 AM on 9/21/2024 unannounced to deliver findings regarding the above complaint allegation and met with Staff, Charity Bulter. Administrator,Sukhjit Sandhu was available by phone. There were 2 additional care staff and 26 residents (2 receiving hospice services) on site at the time of visit. Complaint alleges that staff allowed resident in care to leave the facility unassisted. Per LPA review of self-reported incident report dated 9/19/2024 and statement filed by reporting party, resident (R1) left the facility unassisted and without the knowledge of the staff on 9/16/2024. Reporting party (RP) found R1 walking at 6:21 PM and rendered assistance and found someone who could speak R1's language. R1 was confused and RP was aware of facility nearby. RP notified police. On 7/31/2024, Licensing Program Analyst and Administrator, Mantu Sandhu met for an announced office meeting and delivered complaint investigation findings. LPA gathered resident documents during the course of the investigation. Complaint alleges Licensee did not ensure that a medical assessment was performed on resident (R1) prior to acceptance of resident. Based upon a review of R1's records, it was found that R1's physician's report is on file dated 5/26/2024 prior to resident admission into the facility. This agency has investigated the complaint allegation listed above. We have found that the complaint allegation was UNFOUNDED, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. No deficiency cited. On 7/3/2024, Licensing Program Analysts (LPA's) Tobola and Mutialu arrived unannounced for the purpose of complaint investigation and was greeted by Lead Carestaff, Charity Butler. Administrator, Mantu Sandhu arrived later in the visit. LPA's toured the facility, interviewed staff and residents, reviewed records and made observations during the course of the investigation. Complaint alleges staff do not ensure that residents are provided a diet that is of the quality and in the quantity necessary to meet the needs of the resident(s) in care. Based upon multiple tours of the facility and LPA observations, there appeared to be a sufficient amount of fresh fruits, vegetables and a general amount of food supplies sufficient in quality and quantity. Resident dietary restrictions are also properly posted in the kitchen area. Due to a lack of corroborating evidence the allegation is found to be unsubstantiated. On 7/2/2024, Licensing Program Analysts (LPA's) Tobola and Mutialu arrived unannounced for the purpose of initiating complaint investigation and was greeted by Lead Carestaff, Charity Butler. Administrator, Mantu Sandhu arrived later in the visit. LPA's toured the facility, interviewed staff and residents, reviewed records and made observations during the course of the investigation. Complaint alleges licensee did not ensure that a pre-admissions assessment was performed on resident prior to acceptance of resident. Based upon file review, LPA's found that 3 out of 3 residents (R1, R2, R3) did not have proper pre-appraisal assessment on file. In addition, upon interview, Administrator confirmed that the pre-appraisal assessments were not completed prior to residents' admissions. On 9/28/2023, Licensing Program Analyst (LPA), Tobola arrived unannounced for the purpose of conducting investigation and delivering complaint investigation findings and was greeted by Staff, Violetta Tafalla. LPA conducted tour of the facility, interviewed outside parties and made observations. Complaint alleges staff do not provide adequate supervision, resulting in residents wandering away from the facility. Based on several interviews with outside parties (I1, I2, I4 & I5) it was indicated that there have been multiple witnessed incidents in 2023 where resident(s) were observed wandering outside of the facility without staff supervision. It is also indicated that during two separate incidents; unsupervised resident(s) were assisted back to the care facility due to wandering without staff care or supervision. Due to multiple accounts supporting, the allegation is found to be substantiated. Allegation investigated: Licensee does not ensure facility grounds are maintained.
Licensing Program Analyst (LPA) Araceli Canela arrived unannounced to initiate a complaint investigation regarding the above allegation and met with Administrator, Sandhu Sukhjit. During visit LPA spoke with staff and made observations. It was alleged Licensee does not ensure facility grounds are maintained in that the faclity side yards fence is broken, there are several household items and gargabe on the premises. Based on LPA’s observations during todays inspection 9/22/2023, LPA observed the yard fence, tilting, missing wood panels and in need of all around repair. LPA also observed the left side of the backyard with two unfolded tarps laying around and the back area out side the resident rooms needing to be raked to remove debri.
