On 10/28/2025, at 9:00am, Licensing Program Analysts (LPAs) Haner-Tomasko and Rankin arrived unannounced at the facility to conduct a subsequent complaint visit to deliver findings to the above allegation. LPAs met with Licensee/Administrator Dexter Price and explained the purpose of the visit. On 11/26/2024 LPA Rankin conducted the initial complaint investigation, collected documentation, and conducted interviews. On 10/9/2025 LPA Haner-Tomasko conducted a collateral visit for additional interviews and collection of other documents. On 10/9/2025 LPA also conducted an additional visit to this facility to further investigate the complaints listed above, collect additional documents, and conduct interviews. Licensing Program Analysts (LPAs) Rankin and Haner-Tomasko conducted a subsequent complaint visit to issue final findings on this investigation. LPA met with Dexter Price and explained the purpose of the visit. During the investigation, LPA conducted an initial visit on 10/18/25, where LPA conducted interviews with administrators and obtained relevant documents. Collateral visit was made on 10/17/25 and additional documents were reviewed and relevant documents collected. Interviews with witnessing parties were held on 10/17/25, 10/21/25, and 10/23/25. Licensing Program Analyst (LPA) Olson conducted an unannounced subsequent complaint visit to issue final findings on the allegation above. LPA Chavez conducted the 10 day visit and requested relevant documents on 4/3/23. LPA Olson interviewed reporting party on 3/28/23, Administrator on 4/4/23 and 5/4/23, and witnesses on 3/27/23, 3/28/23, 4/20/23, and 4/24/23. LPA Olson met with Staff and explained the purpose of the visit. On the allegation: Staff denied resident visitors. It was alleged that on 1/1/23, a visitor was denied access to visit Resident 1 (R1). Interviews with Administrator revealed that they were told by APS and the police that due to the visitor’s behavior and the incident that occurred on 10/5/22, the visitor was banned from the facility. Administrator stated APS directly told the visitor they were not allowed to come back and the visitor didn’t come back for 3 months, so they thought they were able to not allow them to visit. LPA met with Dexter Price, Administrator/Licensee and explained the purpose of the visit. LPA reviewed documents and interviewed Hospice Nurse on 10/14/2022, R1’s family member on 5/27/2022, and staff on 10/14/2022. On the allegation: Resident is being over medicated. Reporting party was concerned that the facility was over medicating Resident 1 (R1) due to their behaviors. LPA Olson interviewed staff on 10/14/2022 who stated the facility follows doctors orders and can’t prescribe medications or give the resident extra doses of medications. Staff follow resident’s prescriptions and count medications often. Staff stated R1 had a nurse come twice a week and oversaw R1’s care and would have noticed or said something if they believed the resident was over medicated. LPA met with Dexter Price, Administrator/Licensee and explained the purpose of the visit. LPA reviewed documents and interviewed Hospice Nurse on 10/14/2022, R1’s family member on 5/27/2022, and staff on 10/14/2022. On the allegations: Staff hit resident in care and Staff handled resident in a rough manner while in care. It was alleged that Resident 1 R1 informed their Hospice Nurse that a male staff member was hitting R1 and told R1 no, and put their hand over R1’s mouth so R1 couldn’t talk and had difficulty breathing. LPA Olson interviewed R1’s Hospice Nurse. The Hospice Nurse stated that they have no suspicions of abuse and the resident showed no physical signs of abuse at that time.
