**REPORT WAS AMENDED ON 06/16/25 AND THE AMDENDED COPY WAS EMAILED TO LICENSEE** Licensing Program Analyst (LPA) Liliana Silveira conducted a virtual meeting on Microsoft Teams with Licensee Xiaoqing “Leslie” Wang to deliver complaint findings. LPA stated the purpose of the meeting with Leslie. The investigation consisted of observations, interviews and a records review. On January 30, 2025, it was alleged that the Licensee had abandoned the facility. A Licensing Program Analyst (LPA) from the Department conducted an unannounced Health & Safety check on January 31, 2025. The LPA observed that there were caregivers present, changing residents and preparing dinner for the residents. The LPA also observed that the facility was clean. Interviews with the current acting Administrator revealed that in June 2024, two Administrators had quit and left the residents with care staff. Licensing Program Analyst (LPA), Sabel Martinez, conducted an unannounced complaint investigation visit to conduct additional interviews and deliver findings. The LPA introduced himself and disclosed the purpose of the visit to House Manager Jushua Mendoza. Throughout the investigation, the Department secured pertinent records and conducted interviews with internal and external sources. It was alleged the staff did not safeguard a resident's personal property. On 11/22/23, it was reported to the Department staff had removed one hundred dollars from a resident’s room. An interview with Resident # 1 (R1) revealed R1 left cash in a drawer, and some of the cash had gone missing. R1 did not witness anyone removing the cash. Interviews with internal and external sources did not reveal evidence corroborating the cash was removed by staff. Licensing Program Analyst (LPA) Dawn Segura conducted an unannounced visit to deliver investigative findings. LPA was granted entry into the facility and met with Jushua Mendoza, Staff, to whom LPA disclosed the reason for the visit. Community Care Licensing (CCL) has investigated the above listed complaint allegation. The investigation consisted of interviews of staff and outside sources. It was alleged that the licensee interfered with residents’ right to select their own hospice agency. Based upon interviews conducted during the investigation, it was determined that Resident 1 (R1) was admitted into hospice services upon moving into the facility. During the course of his/her stay, R1 received hospice services from two hospice agencies at different times. It was determined that Resident 2 (R2) also received services from two different hospice agencies during his/her time in the facility.
