Licensing Program Analyst (LPA) Sarina Ramirez conducted an unannounced visit to the facility to deliver findings on a complaint investigation regarding the above allegations. LPA met with Administrator Eileen Sanchez and discussed the purpose of the visit. Regarding allegation #1, LPA conducted interviews with seven (7) residents. Four (4) residents confirmed that staff assist them with their medication daily and that there have been no instances of medication mismanagement. Two (2) residents reported that they are not currently prescribed any medication; however, one (1) of these individuals stated they have not heard of any medication being mismanaged by staff. One (1) resident indicated they manage and store their own medication and likewise reported no knowledge of staff mishandling medications. Licensing Program Analyst (LPA) Sarina Ramirez made an unannounced visit to the facility to conduct a complaint investigation on the above complaint allegation. LPA met with Executive Director Eileen Sanchez, and discussed the purpose of the visit. Regarding the allegation, Facility is in disrepair, it was alleged the facility's second floor carpet was lifting and causing a fall hazard. LPA observed the second floor carpet and it is lifting throughout the entire hallway. The Executive Director stated that a maintenance person will be at the facility on 5/19/25 to replace the carpet with hard flooring. Based on observation and interviews, the allegation is Substantiated. Substantiated meaning that the allegation is valid because the preponderance of the evidence standard has been met. An exit interview was conducted where reports (LIC9099&LIC9099-D) were discussed and provided with appeal rights to Executive Director Eileen Sanchez at the conclusion of the visit. Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit to initiate and deliver findings for the mentioned allegation. LPA Allen met with Administrator Eileen Sanchez who was informed of the purpose of the visit. LPA Allen conducted interviews with three (3) staff members, four (4) residents and records were reviewed. The interviews conducted with 3 staff members stated that the facility has followed the guidelines and regulations related to the infection control, prevention, and mitigation for communicable diseases in the facility. LPA attempted to interviewed two (2) residents R1, R2, who were unable to confirm or deny that they were exposed to Covid-19. LPA interviewed Resident 3 and Resident 4 who stated that there was Covid-19 cases in their unit and guidance was followed by the staff. Licensing Program Analysts (LPAs) Anna Bueno and Amber Coleman conducted an unannounced visit to the facility to investigate the above mentioned complaint allegation and deliver findings. LPAs identified themselves to marketing director Logan Harrison who was notified of the reason for today’s visit and the elements of the allegation. Administrator Brooke Abrego-Huerta arrived shortly and was informed of the complaint visit. The investigation included interviews and records review. The allegations are that the Facility does not have hot water and the Facility staff did not ensure that residents received showers while in care. On the evening of 11/9/2022, it was discovered that the facility water was only warming intermittently. Two vendors that came out to the facility on 11/10/22 and informed Administrator that the boiler went out and part needed for repair will not be available until two to fours weeks.
