Licensing Program Analyst (LPA) Victoria Brown arrived unannounced to deliver complaint investigation findings on 8/19/2021 at 2:00pm. LPA was met by Donna Bautista-Colmenares, Administrator and stated the purpose of the visit. This visit is to deliver the findings of the complaint investigation. Regarding allegation, “Staff failed to provide medical attention in a timely manner after resident fell”, the investigation revealed that Resident #1 (R1) fell several times in 2020, February, April, and July, each time an Interim Service Plan (ISP) was completed with alert charting. The ISP also stated that R1 can be forgetful and confused, needs to assist with re-orienting, reminding, and activities of daily living (ADL) to be checked every 1-2 hours. Facility failed to move R1 to the memory care unit once resident presented a decline and change in condition, In addition, the reappraisal and care plan was not initiated based on the change of condition.
