On 11/10/2025 at 8:30 AM, Licensing Program Analyst (LPA) Shakaricka Hughes arrived unannounced to this facility to conduct a complaint visit. LPA met with facility administrator Lita Vinluan and explained the purpose of the visit. The purpose of this visit is to deliver complaint findings for the allegations above. The current census is 4. A brief interview with conducted with Lita. Allegation: Staff hits a resident It was alleged that staff hits a resident in care. This investigation consisted of interviews with facility staff and residents in care. On 10/28/2025 LPA Hughes conducted a visit to the facility, LPA spoke with 1 facility staff who stated that they have never harmed a resident in care. Additional interview with 1 out of 4 residents in care stated that they have never had a facility staff harm them, or observed a facility staff harm another resident in care. LPA met with facility administrator Lita Vinluan and explained the purpose of the visit. The purpose of this visit is to deliver complaint findings for the allegation above. The current census is 4. A brief interview with conducted with Lita. Allegation: Staff kicked a resident It was alleged that a facility staff kicked a resident. This investigation consisted of interviews with facility staff and residents in care. On 10/28/2025 LPA Hughes conducted a visit to the facility, LPA interviewed one facility staff who denied the allegation and stated they have never harmed or kicked any resident in care. Interview with 1 resident stated they had no concerns with being kicked or harmed by facility staff. LPA was unable to interview additional residents in care due to their non-verbal status.
