Allegation investigated: Medication: Facility staff are neglecting resident care needs
Licensing Program Analysts (LPA) Kevin Gould made an unannounced inspection to Timeless Treasures RCFE on 10/28/25 at 10:30am to conclude the investigation of the above allegation and to deliver the findings. LPA Gould met with Administrator Aaron Fintel and together discussed the investigation details. Based on the interviews and documentation obtained during the investigation process, the allegations are substantiated. The department has determine based on statements and review of R1's physician report, R1 lacks the physical/mental ability to completed the self administration of blood glucose testing and corresponding injections. The department has concluded the facility staff are administering injections for R1 without an appropriately skilled professional. The Department has determined, based on the preponderance of the evidence obtained during this investigation, that the allegation of Medications are substantiated. Report Continued on LIC 9099-C. On 08/07/2025, Licensing Program Analyst (LPA) Pang Lee arrived unannounced to this facility to conduct a complaint visit. LPA Lee met with Administrator Aaron Fintel and explained the purpose of the visit. The purpose of this visit is to deliver complaint findings for the above allegations. The current census is 4. A brief interview conducted with Administrator Fintel. It was alleged that staff did not ensure residents had sufficient hygiene products. The investigation included observations and interviews with staff, residents and three family members of the residents in the home. During an inspection on 07/02/2025, LPA Lee observed that the facility had an adequate supply of hygiene products, including shampoo, conditioner, lotion, toothbrushes, toothpaste, mouthwash, toilet paper, and wipes sufficient to meet residents’ needs. During today’s visit, 08/07/2025, LPA Lee again observed that there were adequate hygiene supplies available for the four residents in care. This was an amended report. On 4/7/2025 Licensing Program Analysts (LPAs) Holly Williams and Charlie Yang arrived unnannounced at this facility to deliver the findings to this complaint. LPAs conducted a brief interview with Aaron Fintel and explained the purpose of this visit. Facility census was 4 residents. Allegation: Facility is in financial distress. On 3/4/25 it was alleged that this facility was in financial distress. During the course of this investigation, LPA Williams reviewed records and conducted interviews. LPA obtained 3 months worth of facility bank statements and 3 months worth of facility utility bills from the facility licensee. The regional office investigated this allegation. Allegation investigated: Staff did not meet a resident's podiatry needs while in care. Staff allowed a resident to be in soiled briefs and clothing for extended periods of time.
Licensing Program Analysts (LPAs) Holly Williams and Kevin Gould made an unannounced visit to conclude the investigation of the above allegations and to deliver the findings. LPAs Williams and Gould met with facility administrator Aaron Fintel and together discussed the investigation details. This investigation consisted of interviews, observations, and record review. LPA Williams interviewed Fintel. 3 staff members (S1-S3), 5 residents (R1-R5).. In an interview, the home health aide (HHA) said they have cared for R1 from 7/1/2024 until R1 passed away on 8/2/24. In an interview, HHA said they would go to see R1 on Mondays and Wednesdays every week and those days eventually included Fridays. In an interview, the HHA said they would find feces and urine-soaked diaper, bedding, clothes, hospital gown, and blanket and the diaper would be deteriorating. Allegation investigated: Due to staff neglect, resident's wound worsened
Licensing Program Analysts (LPAs) Holly Williams and Kevin Gould made an unannounced visit to conclude the investigation of the above allegations and to deliver the findings. LPAs Williams and Gould met with facility administrator Aaron Fintel and together discussed the investigation details. According to Kaiser Permanente Sacramento Medical Center medical records, R1 presented in the emergency department (ED) with four wounds located on R1’s right back, coccyx area, left foot, and left posterior heal. According to Kaiser Permanente Sacramento Medical Center medical records, the stages of the wounds were not listed, and it was only noted as, “deep tissue.” In an interview, administrator Aaron Fintel said that Fintel first noticed “discoloration” on R1’s lower back when R1 was sent to the hospital on 6/28/24. However, staff members denied seeing any pressure injuries on R1’s body prior to R1 being sent to the hospital on 6/28/24.
