On 10/31/2023 at 12:15 PM Licensing Program Analyst (LPA) Pang Lee arrived unannounced to this facility to conduct a complaint visit. LPA met with Interim Resident Coordinator, Editha Mccullough and Resident Care Coordinator, Sarah Nichols and explained the purpose of the visit. The purpose of this visit is to deliver complaint findings for the allegations above. The current census is 59. A brief interview with conducted with Editha Mccullough and Sara Nichols. LPA Lee toured the facility at 2:00 PM, and observed resident in the living room with a guest singer and his guitar singing. Drinks and snacks was offered to residents. The living room was filled with residents. Residents appeared to be in clean clothing and enjoyed the guest singer and each other. On 10/13/23, Licensing Program Analyst (LPA) Tung Truong arrived at the facility unannounced to conclude the investigation of the above allegations and to deliver the findings. LPA met with Administrator Josef Dunham and explained the purpose of the visit. Throughout the course of the investigation, LPA conducted interviews and reviewed records. Based on LPA observations of facility, record reviews, and staff and resident interviews there has been no direct knowledge of facility staff not adhering to resident’s care plan. Residents who were interviewed did not express any concerns with care being provided. Regarding the allegation that staff did not ensure that resident in care was provided their medication(s), LPA reviewed resident's medication log and observed no irregularities. Allegation investigated: Staff behavior poses as a risk to the residents while in care

On 8/23/23, Licensing Program Analyst (LPA) Tung Truong arrived at the facility unannounced to conclude the investigation of the above allegation and to deliver the findings. LPA met with Administrator Romerico Foz and explained the purpose of the visit. Throughout the course of the investigation, LPA conducted interviews and reviewed records. Based on statements obtained, it was determined that there is insufficient evidence to support the allegation mentioned above. LPA conducted interviews with 4 residents and 5 staff members, 2 staff stated that staff (S5) has alcohol smell while in the facility; however, LPA found no evidence to prove that S1 was intoxicated or poses as a risk to residents in care. LPA interviewed S5, S5 denied having drinking alcohol or being intoxicated while in the facility. Regarding Medication, LPA reviewed resident's medication log and observed no irregularities. LPA conducted interview with resident who expressed concern the staff were still using an old prescrition when a new medication had been filled (same medication). LPA explained to resident the staff are using her own existing supply and had ordered new medications before the current bottel had ended to ensure resident had enough medications. LPA Johnson arrived unannounced to complete the investigation of the above allegation. Based on records reviewed the facility did not follow the doctors order for R1's medication. On 11/10/2020 the facility noted on the medication administration record that medication (Rivaroxaban) for R1 was discontinued. The previous records indicated that the medication was not started or refilled, because the resident's insurance did not cover the medication. It was discovered by this LPA that the facility has no record to confirm that the medication was discontinued by the Primary Care Physician, LPA was able to confirm through records review that the medication in question was attempted to be refilled on multiple occasions, however, the record show that the medication in question was not covered by the residents insurance and a requested was made to have an alternative medication in-place of the non-covered medication. LPA met with Administrator Maricar Venegas and explained the purpose of today's visit. Regarding the allegation Staff allows resident to walk outside without shoes, the Department found the following; based on interview, it was determined that Resident 1 (R1) did have on slippers while being outside. This may not have been the proper footwear but R1 did have on footwear. We have found that the complaint was unfounded, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. Regarding the allegation of Residents are not being provided activities, the Department found the following: based on interviews and record review, it was determined that due to Covid-19 the normal activities were put on pause to prevent further breakouts, but Administrator did provide residents with in room activities such as word puzzels, word searches, reading, and bingo. Regarding the allegation of Food services are inadequate, the Department found the following: based on interviews, it was determined Resident 1 (R1) often did not want to eat what was prepared. Licensing Program Analyst (LPA) Anthony Tuck arrived at the facility location on 10/22/2021 unannounced. LPA was met by Administrator Maria Ciscoe. LPA explained the purpose of the visit obtain additional resident and staff interviews in regards to the complaint allegations listed above. LPA conducted interviews with 5 additional residents and 1 staff during today's visit on 10/22/2021. LPA interviewed R4, R5, R6, R7, and R8. LPA conducted 1 interview with S3 during today's visit. LPA conducted a total of 8 resident interviews and 3 staff interviews. LPA reviewed a copy of work order invoice for HVAC system. LPA reviewed pertinent resident documents for R1 and R2. LPA learned that 7 out of 8 residents interviewed indicated the room temperature is fine. LPA learned from 3 staff interviews that the facility maintains a comfortable room temperature throughout the day. On 8/23/2021 at 9:05am Licensing Program Analyst (LPA) Chris Hopkins arrived unannounced to deliver complaint investigation findings. LPA met with Administrator Maria Ciscoe and stated the purpose of the visit. This visit is to deliver the findings of the complaint investigation. Regarding the allegation Insufficient staff resulting in resident wandering out of facility and sustaining an injury, the Department found the following; based on Interview and record review it was determined that Resident 1 (R1) fell outside in the gated back parking lot ( still on premises, where residents and staff park their cars), with no staff supervision. R1 is 101 years old. The cook who witnessed this fall happened to be outside at the time and notified care staff of the incident. Administrator stated that R1 had a previous incident where R1 was hiding behind the bush in the 2nd courtyard patio, and staff went looking for her and couldn't find her for some time (staff checked courtyard patio 3 times).CDSS inspection report, August 23, 2021 · control 27-AS-20210813112347

On 8/12/2021, Licensing Program Analyst (LPA) Tung Truong conducted an unannounced visit to this facility to deliver the complaint findings. LPA identified himself and discussed the purpose of the visit and the elements of the allegation(s) with Administrator Maria Ciscoe. Throughout the course of the investigation, LPA conducted interviews and reviewed records. Based on the investigation, the facility heater was observed to be in working order. The facility temperature was observed at 73 degrees Fahrenheit. Residents (R2) and (R6) stated during the Winter the facility is hot when the heater is on. LPA Lusby arrived on Friday July 30, 2021 to conclude the complaint investigation regarding the above allegations. Prior to the visit, LPA completed required COVID-19 testing protocols, and a daily self-screening questionnaire for symptoms of COVID-19 infection to affirm no COVID-19 related symptoms; LPA ensured she applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: N-95 mask. Throughout the course of the investigation, LPA interviewed staff and reviewed R1's physicians report, medication administration record, resident care notes, incident report and other relevant documents. LPA observed over 25 unsigned medications between December 2020 and January 2021. There were no records to explain why medications were missed.
