On 08/18/2023 at 1:30 PM, Licensing Program Analyst (LPA) Pang Lee arrived unannounced at this facility to conduct a complaint visit. LPA Lee met with Licensee/administrator, Edna Clegg and explained the purpose of the visit. The purpose of this visit was to deliver complaint findings for the allegations above. Throughout the course of this investigation, the Department conducted interviews, reviewed facility files, and reviewed medical documents. Allegation: Facility staff withheld mail from resident It was alleged that the facility withheld mail from resident. LPA Lee conducted 7 out of 8 resident interviews. LPA Lee was not able to interview one resident. It was learned that 6 out of 7 residents had no concerns regarding facility staff withheld mail from residents. It was also learned that the mail man place mails in the locked mailbox. Licensee has the key to the mail and distributes residents’ mails as they are delivered to the facility. On 07/18/2023 at 8:00 AM, Licensing Program Analyst (LPA) Pang Lee conducted an unannounced facility visit to open a complaint investigation. LPA Lee met with Edna Clegg and explained the purpose of today's visit. During today's visit, LPA Lee conducted 6 out of 7 resident interviews. It was learned that one resident is out visiting family memebers. At 9:40 AM, LPA Lee toured the facility and two facility bathrooms. LPA Lee also checked the facility water temperature in both bathrooms. Facility bathroom #1 sink and shower water was measured at 112.3 degrees Fahrenheit and facility bathroom #2 sink and shower was measured at 150.3 degress Fahrenheit. Licensee did not maintain bathroom #2 resident sink and shower water temperature to be within regulatory range of 105 to 120 degrees Fahrenheit. LPA Lee advised licensee to place a visable warning sign stating that the water temperature is hot at both the resident sink and shower. On 07/18/2023 at 8:00 AM, Licensing Program Analyst (LPA) Pang Lee conducted an unannounced facility visit to open a complaint investigation. LPA met with Licensee Edna Clegg and explained the purpose of today's visit. During today's visit, LPA Lee conducted 6 out of 7 resident interviews. It was learned that one resident is out visiting family memebers. 6 out of 6 residents had no concerns regarding facility shower being hazardous and facility staff not ensuring that residents are being provided with toilet paper. Based on LPA Lee observations, at At 9:30 AM, LPA Lee toured the facility bathroom #1 and bathroom #2, LPA Lee observed both bathroom in good working condition. LPA Lee did not observed any hazardous in both bathroom showers. LPA Lee also observed both bathroom having two toilet paper rolls ontop of the toilet. As a result, the allegations are unsubstantiated.
