Allegation investigated: Staff did not ensure sufficient care and supervision was provided to resident Staff did not seek medical treatment in a timely manner for resident in care
On 10/1/24, Licensing Program Analyst (LPA) Kevin Mknelly, arrived to deliver complaint findings and met with Abigail Budoc . On 8/5/24, the department received a complaint regarding R1, fall on 8/4/24, alleging Staff did not ensure sufficient care and supervision was provided to resident and Staff did not seek medical treatment in a timely manner for resident in care. Additionally, on 8/5/24, the department received an incident report, from the Administrator, for the same incident. The incident report stated that after the Administrator left the facility, R1 had a fall and appeared to hit their head. The caregiver, S1, noticed bleeding from where R1 had hit their head. S1 moved R1 to a comfortable position and attempted to call Hospice to report the fall/ injury. While S1 was attempting to call hospice, family of R1 arrived, as R1 was “bleeding slightly”, family of R1 called 9-1-1. Allegation investigated: Staff restrained resident Staff prohibited resident visits
On 2/21/24, Licensing Program Analyst (LPA) Kevin Mknelly spoke toIldiko Soropan, lead caregiver, to deliver complaint findings for the above allegation. The administrator arrived to assist. LPA reviewed resident records, facility records and conducted extensive interviews. LPA finds that the allegations cited above are substantiated. Staff restrained resident- Statements by the licensee, caregiver (S1), family and hospice all found that a weighted blanket was used with R1 which was tucked and tied in place to limit R1’s movements and try to reduce R1’s restless agitation. This was not discussed with hospice or family as a viable strategy and when found by hospice, it was discontinued. This constituted a restraint by S1 of R1. Staff prohibited resident visits- Family statements and hospice records found that family and some hospice personnel were at times discouraged from visiting or visits by appointment were enforced. On 12/5/23 , Licensing Program Analyst (LPA) Kevin Mknelly LPA Mknelly arrived and met with Administrator to deliver investigation findings. LPA conducted interviews with residents, staff and resident families. LPA finds that facility met Tittle 22 requirements. All interviews and observations conducted found the home is clean and well staffed. Interviews found all to be very happy with care and interactions. This complaint is therefore unfounded. This agency has investigated the above complaint allegations. We have found that the complaint is UNFOUNDED, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. We have therefore dismissed the complaint.
