Licensing Program Analyst (LPA) Cynthia Chan conducted a subsequent visit to deliver findings. LPA met with Administrator Silvia Valdez and explained the reason for the visit. LPA obtained copies of the resident and staff rosters, documents for Resident #1, and interviewed five (5) Staff and seven (7) Residents. LPA also interviewed a Mental Health Therapist on 3/9/26. The investigation revealed the following: Allegation - Staff refused to accept the resident back into the facility upon hospital discharge.
