Licensing Program Analyst(s) (LPA) Mary Flores conducted an unannounced complaint investigation visit regarding the above allegation(s). LPA Flores met with Elmer Malang caregiver and explained the reason for the visit. The investigation consisted of the following: LPA Flores requested copies of staff and resident roster. LPA conducted interviews with Resident #1(R1), #2(R2), #3(R3), #4(R4), #5(R5), #6(R6), administrator, staff #2(S2) and #3(S3). LPA reviewed medication for R1,R5, R6, and requested copies of physician's report, centralized stored medication records for R1, R5, R6, trainings on personal rights and medication for staff. The investigation revealed the following: Regarding allegation: Staff are not treating residents with dignity and respect. It is alleged caregiver tossed boxes of resident's personal hygiene supplies at resident's feet while lying in bed. Licensing Program Analyst (LPA) Alberto Lopez visited this facility as an unannounced 10- Day Complaint Visit to investigate the allegations noted above. LPA Met with DSP Elmer Manalang and Administrator Tina Arutyunyan arrived a short time later. The investigations consisted of obtaining personnel and staff rosters and interviewing Administrator, staff 1 -2 (#1,#2) W#1-W#4, W (W1-W4) and residents 1-6 (R!-R6) Regarding Allegation: Facility staff did not meet resident's bathing needs. It is alleged that resident has not been bathed in 2 weeks. Administrator stated that resident is given bed bath 2 times per week and that R1 chooses to have Home Health employee bathe him. 6/6 residents could not collaborate the allegations. This addendum is written to correct and add 2 deficiencies on LIC 9099D and does not change the complaint investigation report findings recorded on 7/14/2022. Subsequent visit on 10/20/2022 A complaint investigation was conducted by LPA Alberto Lopez at the time of the visit, LPA met with Elmer Manalang Caregiver and explained the purpose of the visit. Elmer stated that Administrator was out of the country. LPA reviewed medications and medication records for Resident #1. LPA also conducted an interview with R1-R2 and R3. R4 and R5 could not answer questions. Based on interviews and medication record review, LPA observed the following: Resident's #1's medication Ativan is labeled as a PRN and caregiver stated he gives one every night to resident because resident ask for. An Informal Meeting was held at the Monterey Park Adult and Senior Care (CCLD) Regional Office to deliver findings on the above allegation and discuss the Trust Audit completed on 3/8/2022 by CCLD Audit Section. The purpose of the meeting was explained to Licensee Tina Arutyunyan and Assistant Administrator Akop Ekimyam. Attendees present were: CCLD Regional Manager Araceli Ramirez, Licensing Program Manager (LPM) Lisa Hicks, Licensing Program Analysts (LPAs) Noemi Galarza, and Mary Flores, Woodland Hills ASC staff, and Auditor Jacqueline Juarez. The investigation consisted of the following: On 9/1/2021, LPA inspected the facility and interviewed staff (S2) & residents (R1-R5). R6 was not interviewed due to cognitive impairment. The following documents were obtained: ID/Emergency Information, Admission Agreements, Appraisals, Physician's Report, LA County Department of Mental Health/Brilliant Corners Flexible Housing Subsidy Pool (FHSP) Program Agreement.
