On today's visit at 2:30 p.m. LPA Mosley was greeted by staff and Administrator Designee and informed them of the visit. LPA met with Administrator Designee, Michael Custodio and explained the reason for the visit. The purpose of today’s visit is to deliver findings for the above allegations. Entrance interview. On 11/26/2024, the Department received a complaint regarding the following allegations, Staff do not keep the facility free of pest, Staff do not provide adequate food service, and Staff do not ensure that sharp objects are inaccessible to residents in care.
