Licensing Program Analyst (LPA) Alberto Lopez conducted subsequent complaint visit to address the above allegation. LPAs met with Desiree Eudave, Supervisor and Keith Kasin, Administrator. LPA explained the reason for the visit. 08/16/2024 On this date, LPA interviewed six (6) staff six (6) residents from Assisted Living and LPA also obtained staff and resident roster and reviewed and obtained two (2) residents' relevant medical documentation, and current training documentation. LPA asked for R6 Woods discharge paperwork and updated LIC602 to be emailed and proof of emergency disaster training for night shift staff. 10/15/2024 LPA interviewed a total of ten (10) staff including staff previously interviewed on earlier visit. LPA interviewed one (1) additional resident. LPA interviewed six (6) residents total and LPA obtained R6 file and 08/2024 and 10/2024 staff schedule. Licensing Program Analyst (LPA) Mary Flores conducted an unannounced complaint investigation visit regarding the above allegations. LPA met with Desiree Eudave and explained the reason for the visit. The investigation consisted of the following: LPA requested copies of staff/resident roster, conducted a tour of 9 assisted living resident rooms; room #170, 174,176,178,233,236,237,243, and 278. LPA interviewed 7 residents and 7 staff. LPA requested copies of assisted living staff schedule. The investigation revealed the following: Regarding allegations: Resident’s refrigerator is in disrepair and Facility does not have sufficient maintenance support staff over the weekend. It is alleged resident’s refrigerator was not working, it is unknown how long the refrigerator was not working, and maintenance staff is not available on the weekends to assist. Licensing Program Analyst (LPA) V. Maldonado made an unannounced initial visit to the facility for the purpose of investigating the above-mentioned allegation. LPA Maldonado met with Keith Kasin- Executive Director and Desiree Eudave- Director of Resident Care and explained the purpose for the visit. During the visit, LPA obtained a copy of the resident and staff roster for the memory care unit, conducted a tour of the memory care unit (physical plant), and obtained the following records for Resident# 1 (R1): Facesheet, Physician's Report, Pre-Placement Appraisal, Current Appraisal, LIC624-Incident report for 2/28/23, internal incident report for 2/28/23, Hospital Discharge documents, Plan of Operations- Absentee Notification Plan for Missing Residents, and current care plan. LPA also conducted interviews with Staff# 1 (S1) in person, Staff# 2-5 (S2-S5) telephonically, due to staff not present in the facility at the time of the visit, and attempted to interview R1. The following documents were requested; staff and resident roster, staffing schedules for the months of 11/2020 and 01/2021, documentation on registry staff usage from 12/2020 -03/2021, communication staff logs for 1/2020 and internal investigation report for Resident #1 conducted by the facility. LPA also requested files for Residents #1-3. On 10/15/2021 LPA Alvarez and Mora interviewed Matthew Neeley and Sue Fairley. LPA requested staff and resident roster, and additional documents. The investigation consists of the following: LPAs interviewed the director of Social work, CEO of Hillcrest, four residents from Assisted Living and two resident from Memory Care unit and LPA also obtained staff and resident roster and reviewed two residents' medication from Memory Care Unit The investigation revealed of the following: Allegation#1 " Resident's hygiene needs were not met." LPA interviewed all four residents in Assisted Living and all denied the allegation and reported all staff take good care of them and all the staff are wonderful and all their needs are being met.
