Allegation investigated: Staff do not ensure that refrigerator is maintained at an appropriate temperature
On 08/07/25, Licensing Program Analyst (LPA) L. Salazar arrived at the facility unannounced to deliver findings on the above allegation. LPA was greeted by staff, stated the purpose of the visit and was allowed entry into the facility. Administrator arrived to the facility shortly after LPAs arrival. The Department investigated the above allegations. On 07/18/25 at 10:51 AM, LPA Vega toured the facility and observed 2 refrigerator's, one refridgerator thermometer temperature read 53 degrees F and the 2nd refridgerator read at 56 degrees F. Refrigerator was observed to have a supply of perishable fruits and vegetables, 2 containers of ricotta cheese equaling approximately 6 pounds and 5 containers of salad dressing (thousand island and ranch) that had been opened and covered with a lid. Based on LPAs observations during facility tour, the preponderance of evidence standard has been met, therefore, the above allegation is found to be Substantiated. On 10/20/23 at 8:55 AM, Licensing Program Analyst (LPA) Malia Thao arrived unannounced to deliver findings. LPA explained reason for inspection and was granted entry. LPA met with Administrator Nicole Morehead and Licensee Jonathan Johnson. The Department investigated the allegation that resident sustained severe bruising due to staff neglect and based on interviews and records reviewed, there was not a preponderance of evidence to prove or disprove the allegation occurred, therefore the allegation is unsubstantiated. This is an amended report. On 12/20/2023, Licensing Program Analyst (LPA) Walton arrived unannounced to deliver findings. LPA met with Administrator, Nicole Morehead. The Department investigated the allegations of staff not treating resident with respect and staff making inappropriate comments towards a resident and based on interviews and records review, the allegations are substantiated. One deficiency is cited on the attached 9099D. Appeal rights provided and exit interview conducted. On 08/30/23, Licensing Program Analyst (LPA) L. Salazar arrived at the facility unannounced to conduct the required 10 day inspection. LPA was greeted by facility manager, stated the purpose of the visit and was allowed entry into the facility. LPA conducted records review of Resident R1's file, facility visitor policy and house rules. LPA conducted interviews with Staff S1 and facility manager. Based on the information received, the complaint allegation is Unfounded, meaning that the allegation is false, could not have happened and or is without reasonable basis, therefore, we have dismissed the complaint. Exit interview conducted. No deficiency cited. A copy of this report was provided at the time of visit.
