On this date, Licensing Program Analyst (LPA) M. Yang delivered findings for the above allegations. LPA stated the purpose of the visit and met with Administrator Zainab Younes. The Department toured the facility food supply, reviewed records, and observed residents around the facility. Based on the observations conducted, there was an adequate supply of food with good quality, residents were observed without any concerns about going to bed early, and resident’s responsible parties arrange medical transportation. Records were reviewed R1 is no longer reside at the facility and medications were no longer at the facility to be reviewed. Licensing Program Analyst (LPAs) K. Kaur and K. Brown arrived at the facility unannounced for a subsequent inspection. LPA discussed the purpose of the visit and the elements of the allegations with administrator via telephone. The Department investigated the allegations listed above. During the visit LPAs conducted interviews and record review. Based on review of the discharge summary, from skilled nursing facility (SNF) R1 did not have a pressure injury upon re-admission to the facility though skin related concerns were noted. Per Home Health agency wound care was started on 10/17/22 for a stage 2. Based of interviews R1 was provided bed baths by facility care givers because facility staff were unable to get R1 out of bed. Licensing Program Analyst (LPA) Katie Brown arrived at the facility to conduct a subsequent complaint inspection as well as deliver investigation findings. Administrator (AD) Elizabeth Dyer was contacted and unable to come to the facility. AD authorized Staff (S1) Joyce Torres to meet with LPA and sign reports. During the visit, LPA toured the facility and conducted interviews. LPA observed unsecured cabinets contacining chemicals and tools which were accessable to residents. LPA observed resident rooms with dirty floors, dusty furniture and broken closet doors. LPA observed broken plate with sharp edges laying in a sink which was accessible to residents. LPA observed dead insects and multiple rat traps in common area, accessible to residents. LPA observed unlocked storage room in the facility full of furniture and miscalaneous items. Licensing Program Analyst (LPA) Darius Williams conducted an unannounced follow up visit. LPA Williams met with Staff Joyce Torres and discussed the purpose of the visit. LPA Williams spoke with the Administrator via phone and disccused the purpose of the visit. In regards to the allegation, staff engaged in a physical altercation with resident in care resulting in resident sustaining injuries, on a date near the end of August 2022, Witness 1 (W1) observed bruises on Resident 1 (R1) which was attributed to a fall. On 9/5/2022, the Administrator contacted Emergency Services (EMS) as R1 was becoming combative. W1 arrived to the facility as EMS arrived and reported no new marks were observed on R1. Licensing Program Analyst (LPA) Darius Williamd conducted a follow up visit. LPA Williams met with Staff, Joyce Torres and discussed the purpose of the visit. Administrator, Elizabeth Dyer was reached by phone. At approximately 10:10 a.m. LPA Williams observed Resident 1 (R1) outside the facility gates standing next to a tree near Gettysburg Avenue street. LPA Williams did not observe any staff outside with R1. According to R1’s physician report, dated March 23, 2022, he has wandering behavior and is unable to leave the facility unassisted. Licensing Program Analyst (LPA) Shawna Doucette Licensing Program Manager (LPM) Sergiy Pidgirny and conducted an unannounced complaint visit to deliver findings. LPA and LPM met with Administrator Elizabeth Dyer and discussed the purpose of the visit. The Department has investigated the allegation: Resident’s are unsupervised, Resident’s are not provided activities and Facility is dirty. On 6/11/22, while at the facility, LPA observed R2, who has a dementia diagnosis to be outside on the sidewalk in the front of the facility near the public street with no staff supervision. On this date census was 13 with one staff on duty. On 6/11/22, while reviewing records and touring the facility, LPA was unable to obtain or locate activities available to residents in care.
