Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Melissa Spaeth conducted an unannounced visit on 10/29/2025 and was greeted by two staff members. The Administrator David Salvador was called. LPA stated the purpose of the visit was to conduct an annual inspection. The caregivers confirmed there are five residents. The facility is licensed for five (5) non-ambulatory residents and one bedridden resident. LPA reviewed resident files at 11:15 am until 11:45 am. LPA reviewed staff files at 11:45 am until 12:10 pm. LPA reviewed resident's medications at 12:10 pm until 12:25 pm. LPA Spaeth and the caregiver toured the facility at 12:25 pm until 1:00 pm. Common Areas – The family room, dining room, and kitchen are combined. The family room was furnished with comfortable seating and a television. The dining room area contained a dining room table and chairs. Kitchen - LPA Spaeth observed a two-day supply of perishable food and a seven day supply of non-perishable food. The knives were locked. The cleaning solutions were locked underneath the kitchen sink. The fire extinguisher is located near the kitchen and is operable. Continued on 809-C Staff Room – The staff room was locked. Resident Rooms: The resident rooms were furnished with a bed, linens, night stand, lamp and chair. The rooms were neat and clean. Bathrooms: The bathrooms contained hand soap, paper towels, grab bars, trash can, and slip resistant mats. Water Temperature: The water temperature was tested at 11:40 am and was 115 Degrees F. Hallway Cabinet- LPA observed the clean linens were located in a cabinet. Surrounding Grounds: There were no visible hazards, and passageways were free from obstruction. The side gate of the house was closed and was not locked. Comfortable seating is also located in the backyard in a shaded area Smoke/Carbon Monoxide Detectors: The smoke and carbon monoxide detectors were tested at 11:55 am and were operable. Auditory Device: LPA observed the auditory device to the front door and the sliding glass door were turned off. During the visit, the caregiver turned on the devices. Under Title 22 General Regulations, the following citation was issued and recorded on LIC 809D. Type of visit: Case Management - Other

Licensing Program Analyst (LPA) Melissa Spaeth conducted an unannounced case management visit. LPA was greeted by two caregivers. LPA Spaeth stated the purpose of the visit was to conduct a case management visit since the facility will be sold to another Licensee. LPA Spaeth observed a third person at the facility and asked staff about the third person. The staff stated a family member was visiting them and confirmed the family member has not completed the finger print clearance process. LPA Spaeth called the Administrator, David Salvador and stated the visitor would need to leave the facility. LPA also explained to the staff members the visitor would need to leave as soon as possible. LPA observed the visitor left at 12:00 pm. LPA Spaeth reviewed the residents' files at 11:00 am until 11:30 am. LPA toured the facility at 12:00 until 12:15 pm. LPA observed gardening tools in the backyard and I instructed the caregiver to lock up the items. LPA reviewed staff files at 12:15 pm until 12:35 pm. Pursuant to Title 22 Division 6 of the CA Code of Regulations, a deficiency was cited (refer to LIC 809-D). Allegation investigated: Staff used inappropriate locks on the facility grounds

This is an amended copy of the report previously issued on 11/13/2024. This report supersedes reports previously issued. The findings for this complaint remain the same. On November 13, 2024 Licensing Program Analyst (LPA), Melissa Spaeth conducted a complaint investigation at the above facility to address the following allegation(s). LPA Spaeth was met by two caregivers. LPA Spaeth spoke to the Licensee, Nicole De Las Alas via phone call. LPA explained the purpose of this visit was to investigate the complaint and present findings. Continued on 9099C Unsubstantiated This is an amended copy of the report previously issued on 8/14/2024. This report supersedes reports previously issued. The findings for this complaint remain the same. LPA toured the facility at 10:45 am until 11:10 am. LPA interviewed five (R1-R5) out of six residents at 11:10 am until 12:15 pm. A resident (R6) was unavailable. LPA Spaeth interviewed two staff members (S1-S2) out of the four staff members at 12:30 pm until 12:55 pm. Regarding the allegation: Staff used inappropriate locks on the facility grounds. It’s being alleged a hotel style lock is used on the top of the door in addition to a regular lock. During LPA’s tour, LPA observed only a single lock on the front door. R1-R5 unanimously confirmed they are able to leave the facility through the front door without any issues. S1-S2 confirmed the residents are not hindered from leaving the facility through the front door. Based upon resident and staff interviews, the allegation is unsubstantiated. At 10:55 am, LPA observed the medication cabinet was not locked. LPA Spaeth informed the staff members the medication cabinet must be locked at all times. Licensing Program Analyst (LPA) Melissa Spaeth conducted an unannounced visit and was greeted by the caregivers. The Administrator Nicole De Las Alas was called. LPA stated the purpose of the visit was to conduct an annual inspection. The caregivers confirmed there are five residents. The facility is licensed for five (5) non-ambulatory residents and one bedridden resident. LPA Spaeth and the caregiver toured the facility at 10:00 am until 10:30 am. Common Areas – The family room, dining room, and kitchen are combined. The family room was furnished with comfortable seating and a television. The dining room area contained a dining room table and chairs. Kitchen - LPA Spaeth observed a two-day supply of perishable food and a seven day supply of non-perishable food. The knives were locked. The cleaning solutions were locked underneath the kitchen sink. The fire extinguisher is located near the kitchen and is operable. Medications: LPA observed the resident medications, first aid kit, and PPE supplies were safely locked in a hallway closet. Continued on 809-C Staff Room – The staff room was locked. Resident Rooms: The resident rooms were furnished with a bed, linens, night stand, lamp and chair. The rooms were neat and clean. Bathrooms: The bathrooms contained hand soap, paper towels, grab bars, trash can, and slip resistant mats. Water Temperature: The water temperature was tested at 10:40 am and was 110 Degrees F. Hallway Cabinet- LPA observed the clean linens were located in a cabinet. Surrounding Grounds: There were no visible hazards, and passageways were free from obstruction. The side gate of the house was closed and was not locked. Comfortable seating is also located in the backyard in a shaded area Smoke/Carbon Monoxide Detectors: The smoke and carbon monoxide detectors were tested at 11:30 am and were operable. LPA Spaeth reviewed the resident files at 10:45 am until 11:07 am and reviewed the staff staff files at 11:07 am until 11:20 am. LPA reviewed the medications at 11:30 am until 11:55 am.
