Licensing Program Analyst (LPA) Vincent Moleski arrived unannounced to deliver findings this complaint. LPA Moleski met with facility administrator Julie Nonu and explained the purpose of the visit. This investigation consisted of interviews and record review. LPA Moleski interviewed Nonu, two staff members (S1-S2), a resident (R1), and an ombudsperson. 22 CCR Section 87464(f)(6) identifies arranging transportation to medical and dental appointments as a basic service required to be provided by licensees. 22 CCR Section 87507(g)(3)(A)(1-3) indicates that licensees must identify in their admission agreements the payment rate for all basic services, including transportation to medical and dental appointments. Licensing Program Analyst (LPA) Vincent Moleski arrived unannounced to deliver findings on this complaint investigation. LPA Moleski spoke with facility administrator Julie Nonu over the phone and explained the purpose of the visit. Nonu said staff member Moria Gaunavou could sign this report in her absence. This investigation consisted of interviews, observation, and record review. LPA Moleski interviewed Nonu, a former resident’s responsible party (R1’s RP), two staff members of this facility and one staff member of a separate facility (S1-S3). R1 was admitted to this facility on 12/29/23. R1 was admitted to a hospital as of 1/30/24. Arrangements were made to have R1 transferred to a separate facility, where R1 was admitted as of 2/27/24. Licensing Program Analyst (LPA) Vincent Moleski arrived unannounced to open this complaint investigation. LPA Moleski spoke with facility administrator Julie Nonu over the phone and explained the purpose of the visit. When LPA Moleski arrived at the facility, he was allowed entrace by a resident (R1). LPA Moleski and R1 searched for staff, but found none. R1 did not know where staff members were. Three residents in the dining area were unsupervised during staff members' absence. LPA Moleski observed that a cabinet under the sink containing cleaning solutions and knives was left unlocked. A staff member (S1) appeared approximately 10 to 15 minutes later, and said S1 was busy in a resident's room caring for the resident (R6). LPA Moleski explained that an allegation was made that there is insufficient staffing at the facility. S1 said it was "definitely true." Licensing Program Analyst (LPA) Vincent Moleski arrived unannounced to deliver findings on this complaint investigation. LPA Moleski spoke with facility administrator Julie Nonu over the phone and explained the purpose of the visit. Nonu said staff member Latoya Dowe-Rose could sign this report in her absence. This investigation consisted of interviews, observation, and record review. LPA Moleski interviewed Nonu, a staff member (S1), two residents (R2-R3), and a resident’s responsible party (R1 RP). During an interview, S1 said that there was an altercation between R1 and R2 on the morning of January 3. R2 had been telling R1 how to eat breakfast, according to S1, R1 brushed R2’s hand away, which sparked a physical altercation, according to S1. S1 said S1 was not present during the altercation, but was busy changing linens in a bedroom.
