Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced on 09/23/25 to deliver complaint findings for the above allegations. LPA met with Staff, Flerida Manarang and explained the purpose of the visit. Allegation- Staff overcharged resident. -UNFOUNDED The department conducted record review and interviewed staff and relevant parties to investigate this allegation. Staff interview indicated that the facility charged community fee in the amount of $2000.00 when resident, R1 moved into the facility in August 2023 as R1 was waiting to be enrolled under Assisted Living Waiver Program (ALWP). Furthermore, it was learnt that the facility refunded this amount to R1s responsible party via cashier check on 09/05/25 once R1s responsible party brought this issue to facility’s attention. R1s responsible party confirmed this information to LPA during the complaint investigation and disclosed that this matter has been resolved. On 07/14/25, Licensing Program Analyst (LPA) Talwinder Bains arrived at the unannounced to do complaint investigation into the allegation listed above and met with administrator, Maria Susie Dizon and explained the reason of today's visit. During the investigation, the Department conducted interviews and reviewed documentation pertinent to the investigation. Based on interviews and records reviewed, the elevator broke on 07/09/25 and facility reached out same day to company who will repair it. Per facility records and interviews, repair company ordered the broken part and elevator was fully fixed on 07/22/25. Although the elevator was temporarily not in service, the plan to get residents downstairs was implemented and therefore the allegation is unfounded, as there was no harm caused to residents in care. Therefore, the allegation is UNFOUNDED. A finding of unfounded means that the allegation is false, could not have happened and/or is without a reasonable basis. Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced on 04/10/25 to deliver complaint findings for the above allegation. LPA met with Co-administrator, Suzanne Dizon and explained the purpose of the visit. The department conducted record review, interview with administrator and with witness to investigate this allegation. Record review indicated that resident, R1s responsible party had deposited pre-admission fee of $2250.00 for R1 to move in to the facility around 01/25/25 but R1 was hospitalized on 01/20/25 where passed away on 02/02/25 . R1s family notified the facility regarding R1s death on 02/03/25 and facility refunded $1125.00 to R1s family. During the complaint investigation, administrator’s interview reflected that they attempted to contact R1s family regarding the refund of remainder amount, $1125.00 after 02/03/25 but was not successful. Allegation investigated: HVAC system not working.

Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced on 01/24/24 to open the 10 day complaint investigation into the above allegation. LPA met with administrator, Maria Susie Dizon, and explained the purpose of the visit. LPA arrived and toured the facility and observed all the vents for the heating and cooling system. All the vents were operational. Checked all the rooms and all vents were working as well. LPA did not observe anything in disrepair. Administrator stated no one has reported issues with the air conditioner or any appliance in the facility. LPA interviewed 2 staff and 3 residents. Based on this information, the above allegation ‘HVAC system not working’ was found to be UNFOUNDED. The department has found that the complaint was unfounded, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. On 10/04/23, LPA Bains came to the facility to do complaint investigation on above allegation. LPA was screened upon entry including temparature. LPA met with administrator, Maria Susie Dizon and explained the purpose of the visit. Throughout the course of the investigation the department did facility observations and conducted interviews with residents and staff relevant to the complaint allegation. Throughout the course of the investigation the department reviewed documentation and conducted interviews relevant to the complaint allegations.
