Licensing Program Analyst (LPA) Mary Rico and Licensing Program Analyst Manager (LPM) Efren Malagon met with Licensee Kristine A. Mangente at Community Care Licensing Division (CCLD) Adult and Senior Care (ASC) Regional Office to deliver the findings of the above allegations. LPA Rico explained the purpose of the requested Office Visit. The investigation consisted of staff interviews and record review. For the allegation, Facility staff failed to provide resident's records to authorized representative. During staff interviews, S1 confirmed they had received a call, formal letters and via-email request from R1 authorized representative. S1 admitted they did not provide resident’s records to authorized representative because they had assumed they were not allowed to. In addition, S2 stated S1 was responsible to provide R1 records to authorized representative and was aware R1’s representative had contact facility staff members. Licensing Program Analyst (LPA) Ryan Gardner made an unannounced visit to investigate and deliver findings for the allegations listed above. LPA stated the purpose of the visit, was granted entry, and met with Caregiver Frederic Manuel. The investigation consisted of resident interviews and staff interviews. For allegation, Facility staff leave residents in soaked diapers: Interviews with the staff revealed that the resident’s diapers are changed on a as needed basis, but they are changed at minimum three (3) times a day. The number of times the resident’s diapers are changed varies from day to day depending on their urine and bowel movements. The staff denied leaving the residents in soaked diapers and stated that there have been no issues with the residents not getting their diapers changed. Interviews with the residents did not reveal information indicating that they were left in soaked diapers or any issues with incontinent care. Licensing Program Analyst (LPA) Ryan Gardner made an unannounced visit to deliver findings for the allegation listed above. LPA stated the purpose of the visit, was granted entry, and met with Caregiver Mildred Manuel. The investigation consisted of resident interviews, staff interviews, and document review. For allegation, Staff did not adequately feed a resident while in care: Interviews with residents and the staff revealed that the residents are fed three (3) meals a day including breakfast, lunch, and dinner. These meals include protein, vegetables, carbohydrates, and fruit. Additionally, the residents have snacks throughout the day in between meals. If the residents are not full with the amount of food provided for a meal, the residents can request more food. Additionally, the residents can request extra snacks at any point during the day. Interviews with the residents revealed the residents have not been denied food.
