Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to deliver the findings of the complaint investigation into the allegation listed above. LPA met with staff Fnu Lawrence and explained the reason for the visit. The investigation into the allegation, staff did not seek medical attention for resident in care in a timely manner, revealed the following. It was alleged that Resident 1 (R1) had a pressure injury, and no action was taken by the facility to have R1 treated by a wound care specialist. R1 moved into the facility on September 17, 2024. R1 had a stage 2 pressure injury at the time of move in. Witnesses interviewed verified this information. A review of R1’s records show R1 received wound care on September 17, 2024, November 27, 2024, and January 20, 2025. Witnesses interviewed could not verify if R1 received would care on any other days. R1 could not verify the days they received wound care. Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to deliver the findings of the complaint investigation into the allegations listed above. LPA met with Administrator Chet Khay and explained the reason for the visit. The investigation into the allegation, staff are not able to communicate with residents in an effective manner due to language barrier, revealed the following. LPA interviewed the Administrator and 4 staff members who reported no issues communicating with staff. LPA interviewed 4 out of 5 residents, who reported that they had no issues communicating with any of the staff. One resident was asleep and not interviewed. Two former staff members contact information is no longer valid and could not be reached for interview. The Administrator reported that all staff members and the 2 former staff members are bilingual and had no issues communicating with anyone. Allegation investigated: Licensee abandoned facility
On this Day, Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced visit to deliver complaint findings. LPA was greeted and granted entry into the facility by Caregiver Laurence Laurence and explained the reason for the visit. Administrator Chet Khay was available by phone. The Department received a complaint on 01/03/2024 and LPA Mendivil conducted the initial visit on 01/11/2024. LPA Mendivil interviewed staff and current Licensee. LPA Mendivil obtained copies of rental agreements, management agreements, and liability insurance. Regarding the allegation Licensee abandoned the facility, the investigation revealed the following: It was alleged that the current Licensee Hanh Dao has abandoned the facility. Based on interviews with Hanh Dao, the facility is currently operated by Administrators Ruth Khay and Chet Khay as they are in the process of applying for their own license. Allegation investigated: Facility staff do not answer the telephone
Licensing Program Analyst (LPA) Celine De Perio conducted an unannounced 10-day visit to the facility for the complaint received on 11/29/23 and to deliver the findings. LPA De Perio explained the purpose of today's visit, was greeted, and granted entry by staff on duty (S1), who contacted facility administrator (AD) Chet Khay. During the investigation, LPA De Perio toured the physical plant of the facility, conducted interviews, and requested copies of pertinent records reviewed. It was alleged that facility staff do not answer the telephone. During the physical plant of the facility, LPA observed that the facility phone is operational by observing the phone ringing, and staff answering it. A total of 6 interviews were conducted with staff and residents who did not corroborate with the allegation by stating that the facility staff does answer the phone, and that the phone is working.
