On today's date, Licensing Program Analyst (LPA) Rosie Quiroz conducted an unannounced visit for the purpose to conduct additional interviews and deliver findings regarding allegations listed above. LPA Quiroz was greeted by Caregiver 1 (CG1). Licensee (L) Thi Doan and Administrator (AD) Geraldine Doan arrived shortly after and discussed purpose of today's visit. The 10-day inspection visit was conducted by LPA Quiroz on 3/15/2022 and complaint follow up inspection visits were conducted on December 8, 2023 and March 28, 2024. During the course of the investigation, LPA Quiroz conducted interviews with interviewees consisting of staff and residents. LPA Quiroz conducted documentation review of resident roster, staff roster, physician reports, identification forms and Client/Resident Personal Property and Valuables for Resident 1 (R1) and Resident 2 (R2). On today's date, Licensing Program Analyst (LPA) Rosie Quiroz conducted an unannounced visit for the purpose to conduct additional interviews, review documentation and deliver findings for complaint allegation listed above. LPA Quiroz was greeted and granted entry by Caregiver Jovita Manaloto and called and spoke to Administrator (AD) Geraldine Lasala-Doan via telephone and discussed purpose of today's visit. (AD) Geraldine Lasala-Doan arrived during today's visit. The 10 day visit was conducted on 8/02/2021 by LPA Joseph Alejandre. During the course of the investigation, LPA Quiroz and LPA Alejandre conducted interviews with interviewees consisting of staff and witness. LPA Quiroz also conducted documentation review but not limited to resident roster, staff roster, physician report, identification form, needs and services plan and Admission Agreement for Resident 1 (R1) and Resident 2 (R2). Allegation investigated: Facility staff neglected resident resulting in unstageable wounds
On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility for the purpose of delivering findings into the allegation listed above. LPA was greeted and granted entry by facility staff after introducing himself and stating the purpose of the visit. Administrator Vivian Luis-Ortiz was notified of the visit and arrived later to assist. The allegation investigated was present to the administrator. An initial complaint investigation visit was conducted by LPA Michelle Reed on November 17, 2022, based on the present complaint, which was filed on November 16, 2022. The complaint was investigated by the Department and consisted of resident, staff and family member interviews, a review of facility and resident records, hospital records from MemorialCare Orange Coast Medical Center, records from Excellence Hospice Provider and a tour of the physical plant.
