An unannounced Complaint Investigation was conducted on this day by Licensing Program Analyst (LPA) Claudia Gutierrez regarding the allegations mentioned above. LPA met with Assistant Administrator (AAD) Janice Jabonero and House Manager Ana Cacho and explained the purpose of the inspection. Interviews were conducted with three facility staff and five of five residents, regarding the allegation the facility’s bathroom was not clean and was unsanitary. Three of three staff stated the bathrooms are clean and sanitary, and staff clean multiple times a day to ensure cleanliness. Four of five residents stated the facility bathrooms are clean and sanitary. One of five residents stated the facility bathrooms are not clean or sanitary as they have seen cockroaches there and in their bedroom. During today’s inspection, LPA conducted a tour of the facility, including two of two bathrooms. On this day, Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced visit to conduct a complaint investigation. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit. House Manager Ruby Cruz arrived shortly after. The Department received a complaint on 04/21/2025 and the initial 10 day visit was conducted on 05/01/2025. LPA Mendivil interviewed staff and residents and obtained copies of pertinent documents such as invoices for exterminator. Regarding the allegations that Staff does not ensure facility is free of pests, Staff does not ensure facility is free of mildew, and Staff does not ensure facility faucet is in good repair, the investigation revealed the following: Based on observation LPA Mendivil noticed small dead roaches behind a dresser in a residents bedroom and live roaches in kitchen pantry. Per interviews with staff and residents an exterminator came out yesterday 04/30/2025 and sprayed around the house. Allegation investigated: Staff hit resident

Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced complaint visit to deliver findings on the above allegation. LPA was greeted and granted entry into the facility and explained the reason for the visit. During the course of the investigation, LPA toured the facility and interviewed staff, residents and witness as well as reviewed and obtained pertinent documentation such as physician report. Regarding the allegation that staff hit resident, the investigation revealed the following: Four out of four staff as well as two out of three residents deny any physical abuse occurring at the facility. All but one resident interviewed deny seeing anyone get hit at the facility. LPA observed residents appeared to be taken care of and no visible bruising observed on any resident. Based on interviews conducted, LPA is unable to corroborate the allegation. Licensing Program Analyst (LPA) Lydia Martinez conducted and unannounced visit to the facility to deliver findings on the above allegations. LPA identified herself and discussed the purpose of the visit with Caregiver Alexandrea "Alex" Pacleb. The complaint was investigated by the Department. Administrator Adeline Moncera or Administrator Janice Jabonero were not available for today's visit. Cargeiver Pacleb and one Resident were present during today's visit. The other two Residents are in the hospital. During the investigation, interviews were conducted with facility Administrator and witnesses. Additionally, copies of facility file for Resident 1 (R1), photos, Unique Hospice and Palliative care records, Omni Wound Physicians records, and Vitas Healthcare records were obtained and reviewed. Witness 1 (W1) was caring for R1 in her home from 7/15/2020 to 2/25/2021. Services were approved from Unique Hospice and Palliative Care, Inc. from 8/25/2020 – 3/15/2021.
