Allegation investigated: Staff do not prevent a resident from causing harm to another resident

Licensing Program Analyst (LPA) Jose Tan conducted an unannounced initial complaint visit at this facility to investigate the above allegation. LPA met with Wellness Director Gina Osuna and explained the reason for the visit. LPA conducted physical plant tour at 9:35 AM, requested copies of facility documents relevant to the investigation at 10:02 AM and interviewed staff and residents between 10:15 AM to 1:00 PM. Regarding the allegation that Staff do not prevent a resident from causing harm to another resident, it was alleged that tenant, who lives above Resident #1 (R1), drilled holes in R1's unit and pumped a toxic substance into R1's unit and R1 believes the perpetrator is in conjunction with the assisted living facility and is working together to harm R1. On 11/04/25, at 8:10am, Licensing Program Analyst (LPA) Gina Saucedo arrived at the facility to conduct an unannounced, subsequent complaint visit and was greeted by Georgina Osuna, Wellness Director. LPA explained the purpose of this visit was to gather additional information and deliver findings regarding this complaint. On 10/10/25, LPA Jose Gary Tan conducted the initial complaint visit. On 10/10/25, LPA Jose Gary Tan obtained the following documents for the above allegations-Admission Agreement, Resident's Appraisal, Needs and Appraisal, Physican's Report and Resident Pre-Placement. On 11/04/25, LPA Saucedo asked for resident and staff rosters. On 11/04/25, At 9:05am, LPA Saucedo conducted a physical tour, conducted resident and staff interviews and delivered findings for this complaint. Licensing Program Analyst (LPA) Tuesday Cabiness conducted a subsequent visit and met with Administrator Celia Garcia. The reason of the visit is to deliver the final findings of the allegations mentioned below. The following was determined: Allegation # 1: It was alleged facility neglected resident resulting in infections. To conduct the investigation, LPA conducted the initial visit on 07/22/2022. Interviews were conducted and documents were obtained pertaining to the allegations. Due to the facility having a COVID outbreak at that time of the initial visit, LPA was not able to conduct a physical plant inspection, or interview resident # 1 (R1). LPA was able to interview the complainant, and review documents that were provided to LPA. From the interviews and documents, R1 was diagnosed with a fungal infection, and was seen by the podiatrist and a prescription was prescribed to treat the infection. LPAs met with administrator Jecery Ninonuevo and explained the reason for the visit. LPAs conducted physical plant tour at 10:02 AM, requested copies of facility documents relevant to the investigation at 10:25 AM and interviewed the administrator and staff between 10:43 AM to 12:00 PM. It was alleged that the reporting party (RP) as a representative of Resident #1 (R1) made a formal request at the facility on 03/23/23 to release all of R1's records to them. Formal request was made and sent via FED EX overnight on 03/24/23 at 4:30 PM. LPAs' interview with the administrator today at 10:43 AM revealed that based on her conversation with their corporate office, they have not yet sent the requested documents to the requesting party at this time.
